This inspection was carried out on 19th April 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The manager has had many years experience in running the care home. This home is one of a group of three and a training co-ordinator is employed for the group to ensure that training needs are addressed. The standard of care is good and the home has a happy atmosphere.

What has improved since the last inspection?

The fabric of the home is being well maintained, so that all areas are clean, odour-free and pleasant to use. Several bedrooms have been redecorated, with minor inconvenience to the residents. A new passenger lift is being installed, to be completed later in the summer. The staff and residents have been involved in recent discussion regarding the menu and meal choices and the new menu will be started at the end of this month.

What the care home could do better:

There is little that can be done better, the home is well run and the majority of the residents are happy with their care. The residents will be happier when the proposed menu changes are implemented. The management and staff should continue to maintain the high standard of care that the home provides.

Date of last inspection 23.09.05 Brief Description of the Service: Carleen Nursing and Residential Care Home is registered to provide nursing and personal care for 33 people, no more than nine of which may be service users requiring only personal care. Service users must be over the age of 50 years. The Home is also registered to admit service users who are terminally ill or who have physical disablilities. Carleen is a large house that is in a residential area of Fareham and is close to local amenities. The home has a ground floor extension and provides accomodation over two floors, which are connected by a passenger lift. There are two sitting rooms and a dining room on the ground floor. There are eleven single and eleven shared bedrooms, eight of which have ensuite facilities. The home has large, well tended, front and rear gardens which are accessible to the residents and there is ample parking space for staff and visitors at the front of the building. This unannounced inspection took place over one day. The matron/manager was away but the deputy and other staff members assisted the inspector. The inspector was able to look around the home, talk to staff and residents and see records, policies and procedures. Most of the residents seen during the visit also spoke to the inspector, as did several members of staff. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 The admission process is well managed, providing potential residents with clear information about the home, the services it provides and the type of care available. EVIDENCE: The home uses comprehensive pre-admission assessments before admitting potential residents to the home, ensuring that they can meet their assessed needs. These assessments are then used to plan the personal and health care plans for the resident. The home has provided CAPS or intermediate care beds, but is this service is not currently available. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Residents are well looked after in respect of their health, personal care and social needs. Revision of the care plans is done regularly and the quality of the record keeping is good. EVIDENCE: Three care plans were looked at and found to contain comprehensive information, which is supported by the associated documentation and evidence of regular reviews. The plans contain the individual health, personal, nursing and social care needs of each resident and reflect the quality of the care given. It was apparent during a tour of the home that the residents are treated with respect and their right to privacy is upheld, there are screens and curtains in shared rooms and staff were seen to knock on doors before entering rooms. Residents that were spoken to were happy with their care. Medication policies and procedures reflect the actual practice of the home but there is no evidence to indicate that the nurses have received recent training or up-dates in medication administration. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 There is a variety of activities available to the residents in the home. Visitors are encouraged, with no set visiting hours. The menu is being changed and offers variety and choice daily, with all specialist diets catered for. EVIDENCE: There is a timetable available in the hallway, which outlines all the activities available within the home, including musical exercise sessions, bingo and singalongs. On the afternoon of the inspection there was a talk and slide show given by staff from a local zoo, which was very popular. When spoken to some residents expressed dissatisfaction with their meals, the menu choices and presentation of meals. The manager and the chef have addressed this matter and a new menu is planned to start shortly. A new heated trolley has been obtained to ensure that meals stay hot when taken to the residents who are not eating in the dining room. Three cooked meals are available daily, as are snacks and fresh fruit when required. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The arrangements for protecting residents and responding to their concerns are satisfactory. EVIDENCE: Residents and staff comments showed that people felt comfortable discussing their concerns with the manager and staff. A copy of the complaints procedure was displayed on the notice board, containing all the relevant and up-to-date information required. There has been one anonymous complaint to the home since the last inspection and the manager dealt this with. The home has an adult protection procedure. Staff are undertaking adult protection training. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 36 and 37 The home is managed very efficiently. The positive way the home is managed would be enhanced if staff received regular supervision. EVIDENCE: Residents, staff and visitors made positive comments about the management of the home, feeling that they were all valued members of the team. It was a requirement of the last inspection that staff supervision sessions must be held at least six times a year and that a record must be kept. This has started but not continued; so various enabling methods were discussed with the deputy manager. The home has a property book, which records the resident’s possessions on admission and is updated as necessary.
