Date of last inspection 23/02/05 Brief Description of the Service: Castle Dene is a care home for older people and is registered under the Care Standards Act 2000 to provide accommodation and care for up to 27 elderly people. The home is situated in a village location. It is on the edge of the village, surrounded by a large grassy area and woodland and provides a minibus service to facilitate outings. The home offers a choice of lounge and dining areas and residents can choose to eat their meals in a dining room or their own rooms. Menus cater for the likes and dislikes of individual residents. Residents are able to personalise their own rooms and relatives and friends are welcome to visit at any reasonable time. This is an overview of what the inspector found during the inspection. This unannounced inspection commenced at 10.00 am and lasted for three hours. Residents, staff and the manager were spoken to during the inspection. Numerous records including care plans, maintenance records and menus were examined. A tour of the premises was carried out. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3 & 6 Terms and conditions of business for residents living in the home are in place. Resident’s needs are assessed prior to them moving to the home. EVIDENCE: Evidence was available to show that each resident has a written contract/statement of terms and conditions within the home. Three residents files were examined and they each contained an assessment, which had been carried out prior to the person moving into the home. The manager also carries out her own assessment, meeting with the prospective resident, either in his or her own home or in hospital. This service does not provide intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 8 The health and personal needs of residents are met in a way, which treats them with respect and promotes their rights and privacy. EVIDENCE: Three residents files were examined and they each contained the care needs, which were set out in a plan. Evaluations had been carried out within the given timescales. There was evidence to show that resident’s health care needs were being met, documentation showed the involvement of other healthcare professionals such as General Practitioners and District Nurses. Residents or relatives had signed the plans of care to show that they had been consulted on and agreed to the plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 15 A selection of social activities is available both in and outside of the home. The meals provided offer a balanced diet. EVIDENCE: The home has a minibus, which is available to take residents on trips out. Records showed that residents visit Redcar, Saltburn, and local parks, places of interest and attend events in the community. There are also in house activities, bingo, dominoes, available if residents wish to join in. The menus showed that there was a variety of food available for residents. There was also an alternative menu available fir the residents to choose from if they did not like what was being offered on the main menu. Meals are mainly taken in the dining rooms, which are pleasantly furnished and the tables set with the appropriate crockery and cutlery. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The home has a satisfactory complaints system in place. Training for staff in adult protection is required. EVIDENCE: The home has a complaints procedure in place. There had not been any complaints made to the home since the previous inspection. Staff have not received training in adult protection therefore the residents could be seen to be at risk. An immediate requirement was issued in respect of the training required. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The home is generally safe and well maintained offering residents a homely environment in which to live. EVIDENCE: There were maintenance certificates in place and up to date that showed equipment used in the home was safe and in good working order. The water temperatures in baths and showers are taken and recorded every week. On touring the building it was noted that there were no lampshades in bathrooms and toilets. JUDGEMENT – we looked at outcomes for standard(s) 30 Staff require training in fire safety to ensure residents are protected. EVIDENCE: There was evidence to show that staff receive aspects of training to enable them to carry out their roles. However staff require training in Fire Safety.
