This inspection was carried out on 12th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home continues to offer very good quality care and support to the residents. Residents were keen to express to the Inspector that they are "very happy" in the home. Residents and staff interact well together and staff in the home have an excellent knowledge of each individual residents needs. Residents in the home are also very supportive of each other.

What has improved since the last inspection?

The staff in the home have developed more detailed risk assessments in respect of chains on doors and hot water temperatures.

The Service are aged between 18 and 65. The home is one of four small homes with a similar purpose, run by St. Anne’s Opportunity Centre Limited, a West Berkshire based care provider. It is a semi-detached house situated in a cul-de-sac, which leads to entrances to the local primary school, on the borders of Thatcham and Newbury.The purpose of the company is to facilitate the development of the individual to attain independent living skills and management of behaviour that can be challenging. This was a routine unannounced inspection, which took place over a 3.5-hour period. The Inspector spent some time talking to various staff member and the residents who were preparing to go away on holiday that day. A selection of records including resident and staff files were examined. This was a very positive inspection with considerable input from the residents. EVIDENCE: Individual service user records were examined. Each Service user has a detailed contract in place in respect of the terms and conditions of the home. The service user and other relevant persons have signed each contract. Service users spoken to on the day of the inspection had good knowledge of the basic terms and conditions of the home and informed the Inspector they would talk to staff if they had any issues. The service users also informed the Inspector that they discuss issues relating to the home at regular house meetings. JUDGEMENT – we looked at outcomes for standard(s) 11 Service users have opportunity for personal development. EVIDENCE: Individual service user records were examined, each service user has detailed individual plans in respect of work, education and leisure. These plans identify that service users have range of opportunities e.g. through local colleges, work placements and leisure activities to explore and pursue personal development goals. Service Users discussed with the Inspector the opportunities they have for personal development and some of the individually designed tools and strategies in place to help cope with areas of development were also discussed. Each service user has a “strength and needs profile” which has been developed with service user input to identify areas of development. There are also individual risk assessments in place to support and maintain development goals. Daily records detail progress alongside “programme reports” Following a recommendation made at the previous inspection, risk assessments have been updated in respect of chains on bedroom doors for the purpose of privacy. The service users informed the Inspector that they tend not to use the chains. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 21 The ageing, illness and death of a service user would be handled with respect and as the individual would wish. EVIDENCE: The Inspector spent some time discussing this issue with staff and examining records, this is an area of great sensitivity for some service users. The home has a policy in place, which is, clear and detailed. Staff have consulted appropriately with service users and other relevant person those who have made specific requests have these recorded. If relevant a professional assessment will be sought in respect of ability to make such a choice. JUDGEMENT – we looked at outcomes for standard(s) 34,35 and 36 Recruitment records are detailed and recruitment processes are in place to protect service users. Training is ongoing for staff to ensure competence. Staff are supported and supervised EVIDENCE: Copies of staff records are maintained in the home, samples of these were viewed by the Inspector. Records indicate that application forms and relevant checks and 2 references are undertaken. The CRB check details were sent to the Inspector immediately following the inspection. Prospective staff are interviewed and introduced to service users. Training profiles were seen for staff and training is regular and ongoing. Induction and Foundation training is undertaken. Certificates were seen for e.g. Fire, First Aid, Food Hygiene, Vulnerable Adults and N.V.Q’s. Sample supervision records were seen. Staff have regular supervision and supervisors have undertaken training. It was recommended that due to the fact staff work in different homes across the organisation and hence have supervision with more than one Manager on occasions that a list of all supervision sessions for each individual are detailed on a list. JUDGEMENT – we looked at outcomes for standard(s) EVIDENCE: Records and risk assessment in respect of the water temperatures were viewed. Water temperatures are recorded and are frequently above the recommended 43 degrees Celsius. The home has a detailed risk assessment in place for each individual based on capability. The Inspector spoke at length with each service user and they demonstrated sound knowledge of the care plan, risk assessment and practice processes in place to. This was in respect of a recommendation made at the previous inspection and will need to monitored on an ongoing basis.
