What the care home does well

Charnley House provides a good service for vulnerable residents in a pleasant and secure environment. The residents feel safe and secure and enjoy organised trips to local places of interest and the theatre. Seasonal activities are arranged in addition to movement to music and social stimulation sessions. The home cares for the resident`s families and encourages them to remain involved in their relative`s care. There is a stable enthusiastic work force that works well as a team. The carers are respectful towards the residents, their families and also towards each other. The registered provider is committed to the National Vocational Qualification system. The majority of carers and housekeepers have achieved levels 1-3. There are good systems and procedures in place.

What has improved since the last inspection?

The building works have been completed to a high standard. The new bedrooms are above the minimum space standards and are provided with ensuite (wash hand basin and toilet) facilities. They are furnished to a good standard. A new walk in shower has also provided a choice of facilities and is proving popular with the residents. The dining room has been extended and refurbished to provide a pleasant eating area that can also double as a party room.

What the care home could do better:

Charnley House should stop using correction fluid to correct errors in any documentation.

The home is registered for a maximum of 41 service users to include: *up 41 service users in the category of OP (Old age not falling within any other category). *up to 41 service users in the category of PD(E) (Physical disability over 65 years of age). *up to 41 service users in the category of DE(E) (Dementia over 65 years of age). *up to 1 service users in the category of MD(E) (Mental disorder excluding learning disability or dementia over 65 years of age). The service should at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 15th September 2005 2. Date of last inspection Brief Description of the Service: Charnley House is a large, detached building located near to the centre of Hyde. The property has been adapted and extended over the years to provide accommodation for 41 older people, some of whom can have dementia, physical disabilities, sensory disability or mental health problems. The latest extension was completed in November 2005 when additional en-suite bedrooms and communal areas were provided. The accommodation is situated on three floors; there are 37 single and two shared rooms. 16 single rooms have en-suite facilities. In total there are 5 communal areas of differing sizes in addition to a conservatory. One lounge is provided for those people who choose to smoke. A dining room is located on the ground floor close to the kitchen. The home provides aids and adaptations to meet individual needs and there is a full passenger lift. Car parking is to the front of the building and there is a secure garden to the rear. This was the third unannounced inspection at Charnley House carried out over 6 hours. The manager who is also the registered provider is registered with the Commission for Social Care Inspection and was present through out the inspection. An investigation into a complaint from a relative was carried out on the 15th September 2005. Further details are contained in paragraph 16 within this report. Charnley House provides personal care for up to forty-one people over sixtyfive years of age. It is privately owned. In addition to teams of carers the organisation employs housekeepers, catering, laundry and maintenance personnel. An extension to provide additional en-suite bedrooms, shower room, communal areas and improve the dining room was completed in November 2005. The building work has been completed to a good standard. Furnishings and decoration in the original building were also upgraded at this time. The inspector spoke with three people who were visiting their relatives. Four residents two of whom had been recently admitted and two who had lived at Charnley House for a period of time assisted the inspector with their views on residential care. One newly appointed member of staff and the deputy manager were also interviewed during the inspection. Individual care plans and records were checked, care practice observed and a tour of the building was carried out. Comment cards were also left with the manager for relatives and residents to complete. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 4 & 5 Standards 3, 4 and 5 were inspected during the previous unannounced inspection (August 2005) when they were judged to meet fully with the standard. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8&9 The resident’s identified health needs are fully met by the various healthcare professionals. Standards 7 and 10 were inspected during the previous unannounced inspection (August 2005) when they were judged to meet fully with the standards. EVIDENCE: It was apparent during this inspection that resident’s heath care needs had been assessed as requiring a visit from their general practitioners. The consultations were carried out in the privacy of the individual residents rooms. The appropriate records were completed by the deputy manager and relevant information communicated to the staff when the doctors had completed their examinations. A selection of medication administration records (MAR) was examined. The deputy manager was advised to stop the use of correction fluid (Tippex) when errors had been made in the MAR sheets. The correct manner of changing the record in the event of error was advised. Charnley House DS0000005564.V274825.R01.S.doc Version 5.1 Page 10 Also discussed was the prescribing of some medications by individual doctors to be taken as required (PRN). It was the inspectors’ opinion that the doctors’ directions were confusing and could be open to misunderstanding. It was agreed that clarity would be sought from the doctor concerning the prescribing of PRN medications and the outcome fully documented. One resident was being cared for in bed. The carers were fully conversant with the resident’s condition and the manner in which to care for her. The community nurses are also involved with this person’s care and have provided specialist equipment to prevent pressure sores. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Standards 12, 13, 14 & 15 were inspected during the previous unannounced inspection (August 2005) when they were judged to meet fully with the standards. EVIDENCE: A visitor to the home commented that she thought there was no menu available to the residents as it had been in another home she had visited. It should be noted the daily menu was clearly displayed in the dining room. The menu documented the choices available at each mealtime. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The resident’s and their visitors were confident any complaints would be treated with respect and acted upon. Standard 18 was inspected during the previous unannounced inspection (August 2005) when it was judged to meet fully with the standards. EVIDENCE: A formal complaint was made to the Commission for Social Care Inspection in September 2005, by a relative of a resident. The complaint concerned action taken by a senior member of staff as a result of the resident falling in the home. An unannounced inspection was carried out to investigate the circumstances of the incident. At this time the complaint was found to be upheld. The home had however taken all reasonable steps to prevent the action taken by staff after such an accident from reoccurring. The complainant and the registered person were formally advised of the outcome at this time. During this inspection the visitors who spoke with the inspector confirmed they knew who they were to speak to if they were dissatisfied with the care. One person said she had made complaints and the staff had addressed them to her satisfaction. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 23, 24 & 26 The residents live in a safe and well-maintained environment, the home is clean and pleasant. The private accommodation is comfortable and the residents are encouraged to personalise their rooms. EVIDENCE: Since the previous inspection the work to extend and improve the environment at Charnley House has been completed. The finished result is to a high standard. All the additional bedrooms are above the minimum space standards and benefit from larger than average en-suite facilities. Many bedrooms have been nicely personalised and all are lockable. Two residents invited the inspector into their bedrooms they voiced their satisfaction with the accommodation and in particular the en-suite facilities. It was evident during the inspection that the resident’s were free to come and go within the building at will. Charnley House DS0000005564.V274825.R01.S.doc Version 5.1 Page 14 The registered persons husband carries out the basic maintenance and redecoration around the home. Based on observation there appeared to be no outstanding maintenance required at this time. At the last inspection two relatives complained that personal aids were not always in place and spectacles (in one case) had had to be replaced at great cost. The manager was required to ensure that the resident’s aids were in place at all times. This issue was discussed with the deputy manager during the inspection she was confident the staff were making efforts to ensure personal aids were in place and the visiting optician was naming all spectacle frames. Two visitors felt there had been an improvement since the last inspection. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 & 30 Standards 27,28,29 and 30 were inspected during the previous unannounced inspection (August 2005) when they were judged to meet fully with the standards. EVIDENCE: A newly appointed member of staff was interviewed during the inspection. He was aware of National Vocational Qualifications (NVQ) required to fulfil his role as a carer stating it was this training that appealed to him when he first read the advert at the job centre. The carer said he felt supported and enjoyed his work at Charnley House. New staff have been employed to reflect the increase in registered numbers of residents. They were integrated and inducted into a stable enthusiastic team of carers and housekeepers prior to completion of the extension. This situation provides a continuity that can only serve to benefit the resident’s and their relatives. The registered person is a member of the local training consortium which provides training opportunities for the staff in addition to the NVQ at levels 1 (for the housekeepers) 2 and 3. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected.
