Conditions of registration: 1. Admission of one named resident who is under the age of 65. Date of last inspection 4th August 2005 Brief Description of the Service: Chervil Cottage is a care home for up to 17 older people and is registered with the CSCI. The home is located in half an acre of landscaped gardens in the village of Brighthampton, about six miles from Witney. There are public transport links to Oxford and Witney. All service user accommodation is on the ground floor, and is in single rooms, although couples can share if they wish to. Many of the bedrooms have doors opening onto the garden, which offers opportunities for related hobbies and relaxation. Communal areas include a lounge, two separate dining areas and a conservatory. The home is family owned and one of the proprietors is the registered manager. The manager is supported by a local staff team, which improves the continuity of care for the service users. This is an overview of what the inspector found during the inspection. This inspection took place over one morning and early afternoon and was unannounced. This was the second inspection of the service since April 2005. The registered manager was on duty and there were 14 residents in the home, one of whom was in hospital. The inspector saw all areas of the home, spoke to staff and discussed issues with the registered manager. The inspector discussed life in the home with seven residents, and heard their views on the care and services they receive. Only those key standards not assessed at the announced inspection on 4th August 2005 were assessed at this inspection. What the service does well: What has improved since the last inspection? What they could do better:
The residents are at risk from scalding from the hot water in hot water taps and this needs to be quickly addressed. The radiators and hot water pipes need to be covered as soon as possible. New staff who have no care qualifications need to be registered on a certified induction course. A washable finish should be applied to the walls of the laundry room.

A pedal bin should be provided in the laundry for the disposal of gloves and aprons. Staff should attend infection control training. The registered manager needs to gain the NVQ Level 4 qualification. The plans to regularly monitor the quality of the service need to be carried out. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14. Residents are able to choose how to lead their lives within the home. EVIDENCE: The inspector spoke to 7 residents who told the inspector that they felt at home and were able to exercise choice about what they could bring into the home. One resident showed the inspector her room and said she was very pleased with it, and another said that he was able to bring his own bed with him. The registered manager said that relatives or care managers take responsibility for financial affairs when residents are no longer able to do so, and the home does not get involved in residents’ financial affairs. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26. Some measures need to be taken to improve infection control. EVIDENCE: The inspector saw the home’s laundry, situated near the kitchen, and as there are no hand-washing facilities in the laundry room recommends that gel be provided so that staff can disinfect their hands. Staff use protective clothing but it is not clear that staff have had training on the prevention of infection. The home should have an infection control policy giving current guidance for staff on how to prevent infection in the home. A pedal bin should be provided for the disposal of gloves and aprons. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38 was partly assessed at this inspection. The registered manager is skilled and experienced, but needs to gain the NVQ Level 4 qualification. The home’s quality assurance system is informal and needs to be formalised. Residents’ financial interests are safeguarded. Safety issues remaining from the previous inspection have not been addressed. EVIDENCE: The registered manager is also one of the joint proprietors, and is skilled and experienced in running the home. She intends to enrol on the NVQ Level 4 programme soon, but as she has not yet gained the NVQ Level 4 the inspector is required to grade the standard as ‘2’. Chervil Cottage DS0000013070.V284058.R01.S.doc Version 5.1 Page 15 The home’s quality assurance programme is informal and based on the good relationships between residents and staff. The registered manager sees residents regularly, mostly on a daily basis, and is able to quickly address any issues. The registered manager discussed plans for improving the quality assurance system, including residents and relatives meetings, and staff meetings. She also intends to introduce an annual survey of residents and relatives about all issues affecting the lives of residents in the home. The home does not manage residents’ financial affairs, and secure facilities are provided for residents’ valuables. The inspector noted that the temperature of the water in the hot tap in the bathroom near the lounge was too high, and presented a risk to the vulnerable residents. The registered manager must address this risk of scalding from hot water. The inspector noted that all radiators and hot water pipes accessible to residents are not protected. This was discussed with the registered manager, and although some risk assessments are in place a recommendation was made at the time of the last inspection and remains to be done. No STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP38 Regulation 13 (4) Requirement The risk of scalding from the hot water accessible to residents must be eliminated. Timescale for action 24/02/06 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. No. 1. Refer to Standard OP25 Good Practice Recommendations It is recommended that the radiators and hot water pipes are covered as soon as possible. This recommendation remains outstanding from the inspection of 4th August 2005. It is recommended that new staff, without qualifications, are registered on a TOPPS certified induction course to enable them to gain the knowledge to care competently. It is also recommended that staff are registered for NVQ in Care after the induction period to help gain the required percentage of qualified staff for the home.
