Conditions of registration: 1. The service may admit persons between the ages of 60 - 65 years. Date of last inspection 12th July 2006 Brief Description of the Service: China Cottage is a care Home registered to provide personal care and nursing care for up to 33 persons in the category of older adults. The home is situated in the village of Carcroft, which is about six miles from the town of Doncaster. It is a two-storey building with 29 single and 2 double bedrooms. There is a passenger lift to both floors. Accommodation is provided on both floors. The lounges, dining areas, kitchen and laundry facilities are all located on the ground floor, there is also a hair dressing facility. There are patio and lawn areas at the rear of the home and car parking spaces to the front. The fees are from £380.62 to £426.30 per week. This information was provided on the 10th July 2007. This is an overview of what the inspector found during the inspection. A limited number of the National Minimum Standards were examined at this inspection (with emphasis on the ‘key standards’), and the previous requirements. The senior person present at the inspection representing the providers was Mrs S Thomas-Blount the registered manager. The inspector spoke to people who use the service, the staff and visitors to the home. Personal and communal living areas were inspected. Recording systems including risk assessments, care plans, menus, complaint files and staff files were examined. The history of the service was examined prior to the site visit. This included telephone contacts, letters and notifications. What the service does well: What has improved since the last inspection?
The system of care management continues to develop and evolve. The home’s owner and manager continued to demonstrate their commitment and positive attitude to staff training induction and development of the care team. Each service user has a written contract/ statement of terms and conditions with the home. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Pre admission assessments demonstrated that individual needs had been planned for before people moved into the home. People who required care services and their families were given all the information they required to make a choice of whether China Cottage was the home for them. Individual written contracts detailing terms and conditions of residence provided clear information for people who use the service. Case records and discussion with staff confirmed that any specialised equipment that may be required is obtained before anyone is admitted to ensure that people’s health and wellbeing could be supported. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. People who lived at the home and their relatives were involved in care planning and setting measurable targets for care. Care staff were proactive in ensuring that the health care needs of people were adequately addressed. Staff attitudes and approach to care was based on respect for the person and this helped to safeguard and promote the rights and dignity of individuals. Staff were working to the home’s policies for the administration of medication, which promoted the wellbeing of people who lived at the home. China Cottage Nursing Home DS0000053683.V345379.R01.S.doc Version 5.2 Page 11 EVIDENCE: The care plans were well compiled and complete they promoted dignity, privacy and encouraged independence. They contained appropriate information to enable people who were not familiar with the individual to meet their care needs. These had been monitored at regular intervals; some plans had been amended within the timescale in response to changed care needs. Risk assessments had been recorded and evaluated to promote safety of people who use the service. Risk areas had been identified and planned for such as: poor appetite, risk of falls, episodes of confusion and disruptive behaviours, all had plans for staff to follow. Care records show that staff had good communication with the local hospitals, primary care trusts and local GPs. Specialist equipment had been obtained and used to maintain people’s wellbeing such as pressure relieving mattresses. Relatives were observed to visit freely and continue to assist with care of their loved ones as they wished. Family members spoken to confirmed their involvement in the planning and provision of social, physical and psychological care. They said they could have access to care plans whenever they wanted. People who lived at the home and relatives spoken with confirmed that staff respected their privacy and dignity. An example of this was staff knocking on bedroom doors and waiting to be invited to enter. Staff were observed to interact with people appropriately and with obvious empathy for their needs. People living at the home spoke well of their key workers and ‘named nurse’, who they felt provided them with invaluable help. Good interactions were seen with staff this helped to maintain a relaxed and happy atmosphere at the home. All were offered personal care in the privacy of their own rooms or in bathrooms. People were observed to be dressed appropriately this enhanced their self-confidence and dignity. Staff were observed administering medications and helping the people to take their prescribed medicines. Records were correctly maintained and medicines stored safely. The home had a range of policies and procedures to provide guidance for staff and maintain the safety. None of the people were responsible for their own medication although this facility was available. Relatives praised the staff and services provided, “nothing is too much trouble, they are most helpful and ring me and keep me fully informed, they took my mum to hospital and stayed with her I was so relieved, they have endless patience I don’t know how they do it, I’m very happy with the care my mum China Cottage Nursing Home DS0000053683.V345379.R01.S.doc Version 5.2 Page 12 gets here, they remember the little things, she wears her jewellery and makeup as she always has”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. Social and leisure activities are organised by the activities coordinator and other staff, all spoke favourably about their involvement in the activities. The home was providing meals and snacks, which met both the nutritional needs and food preferences of the people living at the home. EVIDENCE: The religious and cultural needs of people were documented, the records showed how these could inform basic lifestyle issues such as diet and dress, and how staff, sometimes in conjunction with families acted to meet these. People were encouraged to choose their own clothes to wear each day, decide when to bathe, where to sit and select their own meals. The meals provided appeared well cooked and presented. Mealtimes were unhurried with extra portions available as required. Portions were adjusted to each person’s likes and diet needs. Care staff also helped people to exercise their food choices at meal times, and those who had difficulty with eating were offered appropriate assistance People living at the home were encouraged to eat healthily. Plentiful supplies of food and fresh fruit were available. There was a four weekly menu with records kept of each persons food consumption. Alternative meals were available if people did not like the planned meal. Visitors to the home confirmed that there was an open visiting policy and that they were encouraged to maintain regular contact with their relatives. Visitors spoken to said they are encouraged to come and see their relative or friend and are always made to feel welcome. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. People who use the service experience good quality outcomes in this area. The staff and management operated an effective complaints policy to protect service users. The policy and procedures for adult protection was in line with the local multi-agency approach. Staff had been trained to implement these procedures to protect people from all forms of abuse. EVIDENCE: There was a complaints’ procedure, it was accessible to all people who live at the home and any visitor. A copy was displayed in the entrance hall. This procedure was referred to, in the statement of purpose and the service user guide. All said that they could complain, if they had reason to do so. They commented that they were confident that management would address their concerns seriously. Three complaints had been made since the last inspection. Records checked showed that they were appropriately dealt with. The recording system has been improved in accordance with requirements made at the last inspection.

Staff confirmed that they had been given training on issues relating to adult protection. There had been no reported incidence of abuse at the home. People who lived at the home confirmed that staff were caring and that they were very satisfied with the delivery of care. They said, “I haven’t any complaints” and “what have I to complain about - I can’t think of anything,” “I am very happy and satisfied with everything here”. Bedrooms were clean, comfortable, and overall well decorated and furnished to meet their needs. Good hygiene standards were maintained and this helped with the control of infection and made the home more pleasant. The owners had created a good environment for people living at the home. And continued to redecorate and refurbish the home. A range of colours and homely features had been used to individualise and improve the appearance of the bedrooms. A number of families had assisted with personalisation of bedrooms with favourite items and memorabilia. The maintenance record book identified that any repairs discovered had been dealt with promptly. The homes gardens were accessible to people who live at the home whenever the weather was suitable. The building and its surroundings were generally well maintained, thereby enhancing its appearance and facilities. People who use this service experience good quality outcomes in this area. This judgement has been made using available evidence including interviewing and observing staff as they worked. This judgement has been made using available evidence including a visit to this service. The numbers and skill mix of staff were sufficient to meet peoples’ needs, in particular, their nursing needs. Staff had received training to meet general and specific needs. Appropriate support and guidance was provided to new staff, enabling them to safely care for people who lived at the home. Staff files included the required information. The home operated a recruitment policy that promoted people’s protection. EVIDENCE: The numbers of trained nurses, care workers and ancillary staff on duty was sufficient to meet the needs of the people. The three staff files inspected were detailed and complete. They contained CRB and POVA checks, job application, references, copies of induction and training undertaken.

They confirmed that not only were they well supported in their work but actively encouraged to train and develop personally. Staff had undertaken statutory training and updates i.e. basic food handling & hygiene. Training had been provided to increase staff awareness of dementia care and adult protection this promoted good practise and people’s rights. Staff received regular supervision and appraisal from the manager this ensured they were equipped to provide a good standard of care. The home is well organised and managed with the health, safety and welfare of people was promoted and protected. EVIDENCE: People said they were satisfied with the way the home was managed. The registered manager was described as being ‘ very able, honest and professional’. She was a first level nurse with wide range of experience.

This was a way to air their views on a range of issues relating to life at the home. People and their relatives commented that use of the homes customer satisfaction questionnaire was a good way of helping the home to further improve its services. The manager also completed regular audits on specific issues such as the environment. The outcomes of these surveys and audits were used to improve the quality of care for people who lived at the home. Visitors to the home stated that they had ready and easy access to the homes management and that they felt confident in them. Staff said there was always a senior member of staff on duty at the home with advice and support readily available. Responsibilities were shared between senior members of the team. The manager monitored case files and the personnel files to ensure policies and procedures were adhered to, this enabled her to identify where improvements were needed. Staff files contained evidence of completed statutory checks protecting vulnerable adults. The manager maintained a system to ensure staff training needs had been met. This enabled her to ensure staff had the necessary skills to provide the care and service that people required. People were assisted to manage their personal monies by either family or staff. The monies held and records maintained were correct. Health & Safety at Work risk assessments had been reviewed to maintain continued safety for people living at the home, visitors and staff. Statutory checks and servicing of equipment were undertaken and records maintained. The Environmental Health and Fire Prevention Services inspection reports were positive confirming that the home provided a safe environment.
