The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

It is evident from the inspection and following discussion with the Manager that she takes great pride in the home and the care that the residents receive. The home continues to provide a high standard of care to its residents. Residents spoken to during the inspection complimented the Manager and staff team. One resident said, "This is a lovely home". The one relative spoken to during the inspection said, "This is a friendly and homely sort of place".

What has improved since the last inspection?

It was highlighted at the last inspection that resident care plans and risk assessments required further development. Examination of three plans of care during this inspection showed that minor improvements had been made, however, further development was still required. A long discussion took place with the Manager in respect of care plans.

What the care home could do better:

This was a very positive inspection with very few areas requiring improvement. As highlighted above care plans and risk assessments require further development. The lounge designated for those residents that wish to smoke needs decorating, flooring replaced and the chairs replaced. The homes reference request would benefit from being updated to ask the referee to confirm dates of employment.

This variation to age is for one named individual, until such a time as the placement is no longer required. 15th September 2005 Date of last inspection Brief Description of the Service: Church View Nursing Home is registered to provide personal and nursing care to forty-seven older people. The home is situated on Thompson Street in Stockton, and is close to the town centre and other local amenities. The home is a modern, two storey, purpose built facility. The home is divided into two units. The ground floor of the home accommodates a maximum number of twenty-three residents who require nursing care, and the first floor of the home accommodates a maximum number of twenty-four residents who require personal care. Each unit has a lounge, dining area and lounge designated for those residents who wish to smoke. All bedrooms are single in nature and provide ensuite facilities, which comprises of a toilet and hand washbasin. All bedrooms meet special requirements of National Minimum Standards. There is a car park to the front of the home and a pleasant enclosed garden/patio area to the rear, for resident use. This unannounced inspection was carried out over two days and lasted for a total of seven hours. Two residents were spoken to individually at length, two other residents were spoken to briefly, and a discussion took place with one relative. Staff were spoken to informally and a lengthy discussion took place with the Manager. Records examined during the inspection included, medication, complaints, staff training/recruitment and a number of policies and procedures. The Manager accompanied the Inspector on a tour of the home. Requirements highlighted at the last inspection in September 2005 were revisited. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The home’s medication policy/procedure ensures that there are safe practices in place for dealing with medication. EVIDENCE: The home’s system for storing, administering and recording medication was seen to be effective, storage is safe and complies with the relevant regulations and records of medication received at the home and given to residents were seen to be accurate. An appropriate system for disposal of medication is in operation. A record of controlled drugs is maintained. The Manager said that those residents who are able are encouraged to self medicate, a risk assessment is carried out on the resident to determine ability and safety, and lockable storage for medication is provided. It was highlighted at the last inspection in September 2005 that resident care plans and risk assessments required further development to ensure that they were specific and individual to the resident. Plans of care examined were still observed to be similar despite residents having very different problems and needs. A lengthy discussion took place with the Manager in respect of this. Residents spoken to during the inspection said that they were happy and that their needs were met. One resident when asked how good is the home at meeting their needs Said, “Good, and I am spoilt”, another said, “They look after me well”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 The home provides a wholesome, varied and appetizing menu that ensures dietary needs of residents are met. EVIDENCE: The home offers a four-week menu with an alternative choice available at each mealtime. Menus examined were wholesome and showed variety. Records were available to confirm that appropriate temperature checks are carried out on fridge, freezers and food. Records of food provided were available for inspection. One resident spoken to during the inspection said, “The food is spot on, there is lots of variety and choice”. It was evident following discussion with residents that relatives are also made to feel welcome for tea/coffee and at mealtimes. One relative said, “I was here for Christmas lunch, it was lovely. We had sherry and bucks-fizz with lunch”. Mealtime was observed to be pleasant and relaxed. Roast beef with all the trimmings was on the menu on the day of the inspection, however, some residents had opted for an alternative. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 The home has an effective complaints procedure, which enables residents to make any complaints they feel necessary. The home’s adult protection policy and staff knowledge helps protect residents from abuse. EVIDENCE: The home has a clear complaints procedure, details of which are also available in the statement of purpose and service user guide. Residents spoken to during the inspection said that the Manager and staff at the home are approachable and if they felt the need to complain then they would do so. One resident spoken to during the inspection said, “If the need arose I without doubt would complain and have confidence that the complaint would be dealt with effectively”. The home has an adult protection policy/procedure that informs of action to take if abuse is suspected. The Manager said that all staff receive adult protection training on induction and on a regular basis throughout their employment, records were available to confirm that this was the case. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26 The standard of the environment within this home is good providing the people who live there with and attractive, homely and comfortable place to live. Refurbishment of the lounge on the first floor designated for those residents who wish to smoke would further enhance this. EVIDENCE: A tour of the premises found the home to be well maintained internally and externally. The home is modern, spacious, welcoming and decorated in a homely and comfortable manner. The ground floor of the home accommodates residents requiring nursing care and the first floor of the home accommodates residents requiring personal care. Each unit has a lounge, dining area and a smaller lounge designated for those residents who wish to smoke. The lounge designated for those residents who wish to smoke on the first floor of the home was observed to be in need of re-decoration and replacement flooring. Chairs in this area were also observed to be worn and marked. Resident’s bedrooms are homely and personalised with personal possessions being evident. Residents bedrooms visited were well decorated and furnished. The Manager said that all baths, showers and sinks in the home environment are temperature regulated and that water temperatures are taken in these areas on a weekly basis. Records were available to confirm that this was the case. At the time of this unannounced visit the home was clean and hygienic with good facilities and procedures for handling laundry and preventing odours or the spread of infection. The home has a total of five hoists, many pressure relieving mattresses and other specialist equipment to assist residents and maximise their independence. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 The homes recruitment and selection process is robust and as such ensures that residents are protected. Staff receive appropriate training to ensure that care needs of residents are met. Appropriate numbers of staff are on duty. EVIDENCE: Duty rotas examined during the inspection confirmed that the home is working with the appropriate number of staff on duty. The Manager said that staffing levels fluctuate depending on dependency and resident numbers. Residents spoken to during the inspection said that they felt that there were sufficient staff on duty. The Manager said that 56 of care staff are trained to NVQ level 2 and 3 in care or equivalent with the majority of other staff registered and working towards their NVQ level 2 or 3 in care. Files examined contained a photograph, two references, one being from the last employer, proof of identity and appropriate POVA/Criminal Record Bureau checks. Both files examined confirmed that a thorough recruitment procedure had been followed. Both files examined were those of staff who had been recently appointed and contained evidence to confirm that induction training had been undertaken. A discussion took place with the Manager regarding the new induction standards that are to come into force in September of next year. Records were also available to confirm that a rolling programme of training is provided for staff working at the home. The Manager keeps a record, which lists all mandatory and other relevant training to the job that staff do. The record highlights each staff member working at the home, training received and training that is due. It was observed that the home’s reference request does not ask the referee to confirm dates of employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 The Manager works extremely hard to ensure that Church View Nursing Home is managed efficiently and effectively. EVIDENCE: The Manager, Denise McPhee is a first level Registered Nurse who has also completed an NVQ level 4 in Management. Denise has worked in the nursing and social care environment for many years. Residents and the one relative spoken to during the inspection spoke highly of the Manager, staff team and care that is provided. One resident spoken to said, “Denise is ideal and I like the nurses”. A relative said, “Denise is good, everyone is on first name terms it is very friendly”. Church View Nursing Home DS0000000160.V266626.R01.S.doc Version 5.1 Page 19 Standard 33 was not inspected in full, however it was highlighted at the last inspection in September 2005 that copies of reports from visits as required under Regulation 26 of the Care Home’s Regulations 2001 must be forwarded to the Commission for Social Care Inspection on a monthly basis, as yet no action has been taken to address this. A discussion took place with the Manager in respect of this who advised that she would forward the minutes of the monthly meetings to the Commission for Social Care Inspection on a regular basis.
