This inspection was carried out on 16th February 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents stated they were very happy and settled living at Cloneen. One resident stated, `we are all satisfied with what we get here,` and another said,` nice place this is`. There was an open and flexible approach towards visitors who felt they were made to feel welcome and who commented that residents were encouraged to have as much control over their lives. As one relative commented, `my father can go to bed when he likes and can get up when he likes`. Residents` benefit from a well maintained environment with Health and Safety records up to date and staff receiving ongoing training in Health and Safety. The home was clean, tidy and free from odours.

What has improved since the last inspection?

Sine the last inspection the new owner has introduced an up to date leaflet for potential residents describing what Cloneen offers and how much that costs. The home has also completed a recent quality assurance survey, is positive in outcome, but could benefit from a summary report highlighting key points for distribution. Staff files provide supportive evidence that there has been an introduction of regular supervision.

What the care home could do better:

Currently the named Health and Safety representative is the new owner and this must be displayed on the Health and Safety poster. The home must also ensure the cellar door is always locked when not in use as this is a potential hazard. A number of tiles on the ceiling in bathrooms and passage way were cracked and in need of replacement to maintain a pleasant environment. The home provides `family style care` to residents, as one visiting relative commented, `It is like visiting one large family`, but the only private space for residents and staff and visitors to meet is the bedroom. There is also no office space with records kept in a lockable cabinet in the dining room. Although this was not presented as a concern for staff, residents and visitors consideration should be given as to how there could space provided for occasion when immediate privacy and confidentiality is required. Currently activity that involves trips out depends on the availability of the managers` car or the maintenance workers car. The home should provide and or have access to a mini bus that can ensure that residents who require wheelchairs have opportunity to trips out.

The house has been adapted to provide accommodation and residential care for 15 men and women over the age of 65. Accommodation is on three floors and presents homely, family style living. The house has 13 single bedrooms and one shared double bedroom. None have en-suite facility. Communal facilities available to residents are: one dining area and one main lounge. Kitchen is domestic in size and style. There is a private patio area used by the residents in the summer. This inspection was announced and was conducted by one inspector, three and a half inspection hours in total. A range of records examined, including staff files, policies and procedures and health and safety records. A group of residents interviewed after lunch, one resident in the privacy of her room, three staff members and a mother and daughter visiting a relative. A tour of the home also took place. Since the last inspection the home has transferred to new ownership, and the new owner has recently been registered with CSCI. The inspector was made welcome. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 Policies and procedures assured that at time of death, staff will treat residents and their family with care and sensitivity. EVIDENCE: Policies and procedures for handling dying and death are in place. Staff spoken to were able to confirm where these policies and procedures were kept and that they are linked to their induction. The home has had to deal with one resident death in the past 6 months and said that family and friends are involved. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 13 14 Resident lives would be enhanced by access to a mini bus to further the activities programme for those who require wheel chairs. Open visiting is encouraged and residents are able to maintain contact with family and friends. EVIDENCE: During discussion with residents, they described how they were able to make decisions about their daily lives and exercise personal choice. One resident stated, ‘you get what you want, I am fussy but they always find something I do like. My choice to have my bedroom door open, if I wanted it closed I can.’ A visitor to the home was able to state how her father can make his own daily lifetime decision about getting up and going to bed, ‘he can get up when he likes and go to bed when he likes, he also enjoys going out which he does’. On the day of the inspection a planned visit to a local school for children with disabilities was cancelled. This is a popular trip for residents. The manager commented that trips out are restricted to those residents who can manage to use his or maintenance workers car. The home should provide and or have access to a mini bus that can ensure that residents who require wheelchairs have opportunity to trips out. Some of the comments were: ‘We can visit any time and have the code for the front door’ (visitor) ‘I have two daughters who are able to visit any time, but always ring first to let me know’ (resident). The home provides ‘family style care’ to residents, as one visiting relative commented, ‘It is like visiting one large family’, but the only private space for residents and staff and visitors to meet is the bedroom. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Residents are adequately protected as the staff received training in adult protection and policies and procedures included displayed actions and contact details. EVIDENCE: Staff interviewed confirmed they had received training in adult protection. This included a ‘no secrets’ course run by Redcar and Cleveland College and their TOPS Induction. Policies and procedures were up to date, staff knew where they were kept and had access to displayed action/contact numbers should they have immediate concerns. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 29 Evidence from the day indicated that residents are in safe hands and are supported by robust recruitment practices. EVIDENCE: At the time of the inspection just over 50 of the staff had completed NVQ Level 2. Out of three staff interviewed, one had completed an NVQ 2 in care and another an NVQ 3 in catering. Two staff files were examined and contained the required information. In one of the files, the Criminal records Bureau check had been taken home by the staff member. This was returned on the day of the inspection. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 38 Arrangements for handling residents’ personal allowances are robust and health and safety records were up to date. EVIDENCE: A number of service and maintenance records were examined and were up to date, and staff confirmed that they are currently in the process of receiving Health and Safety training. However, the nominated health and safety representative must be displayed on the health and safety poster. A random sample of residents’ personal allowances and records examined highlighted that there were no discrepancies with the balance stated on the transaction sheet and the actual amount contained in the individual pouches.
