The category (PD) applies to a named service user for respite care only 24th May 2005 Date of last inspection Brief Description of the Service: Clova House offers residential care to 40 people over the age of 65 years. The home is a large three storey traditional detached house standing in its own well kept and attractive grounds on the outskirts of Ripon, with the town centre and market square within reasonable walking distance. Service users’ accommodation is on all floors accessed by both shaft and stair lifts. Most service users rooms have either a pleasant garden or rural view. This is an overview of what the inspector found during the inspection. This report relates to an unannounced inspection carried out on the 17th October 2005, which started at 9.45hrs and finished at 15.45 hrs. A tour of the home was carried out which included the service users’ private accommodation, a selection of records were looked at and time was spent observing activity in the home, talking and listening to service users, relatives and staff. The focus of the inspection was a number of key standards, inspecting the care records of five service users in detail to establish if they corresponded with service users experiences in the home. There were also discussions with the registered manager of the home. What the service does well: What has improved since the last inspection?
All of the requirements made at the last inspection apart from one have been implemented. Staff training is comprehensive and available to all staff. The homes manager has organised relevant courses involving other health care professionals such as the Community Dietician to undertake training regarding nutrition for older people. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,3,5 and 6 Prospective service users can be confident before moving into the home that their needs can be met. EVIDENCE: A pre-admission assessment is carried out and an assessment form is completed prior to admission into the home. A care plan from a placing or funding authority where applicable is also obtained. The information on files was comprehensive and details give good indication of the care needs of service users. Service users and their relatives are encouraged to visit the home prior to admission. Service users can stay for the day or have an overnight stay at the home. All service users or their representatives are to be issued with the organisations Terms and Conditions, which have been updated. The registered manager stated that the home does not provide intermediate care.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 The health care needs of service users are well met. EVIDENCE: Five service users’ case files were inspected, which contained comprehensive plans of care. Risk assessments covering areas of daily living were available and up to date. The health care needs of service users were recorded on their individual care plans that had been regularly reviewed. For those service users that are unable to self medicate the home operates a monitored dosage system, which was inspected. Medication was appropriately stored and records were accurately maintained. There were three service users being cared for in bed. One service user is bed bound, whilst the others are encouraged and supported to sit out. There was no evidence in their care plans or in the discussions held with the manager to suggest that the service users at this moment in time are requiring nursing intervention, as the home is meeting all of their care needs. The home’s manager is aware of her responsibilities if service users’ conditions change and

One visitor to the home stated that the home is very good and staff are always helpful. However a number of service users stated that overall staff are very good “always polite, but some can be sharp”. This was fed back to the home’s manager who agreed to investigate this matter and to report back to the Commission. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,14 and 15 Service users’ lifestyles within the home reflects their wishes and service users are encouraged by the staff team to continue with their individual interests EVIDENCE: Visiting arrangements are flexible throughout the day as this was confirmed by the comments made by service users and one visitor. There are regular activities most afternoons such as a game of snakes and ladders being played by a number of service users in the afternoon,whilst some service users had been out to a coffee morning and various entertainers are available. Mealtimes at Clova are flexible. All service users spoken with commented that the food at the home was very good. Menus had been displayed on the notice board but had kept disappearing. The manager was advised to look towards an alternative way of informing people such as a chalkboard. Menus are on a four weekly roster. In discussions held with the cook special diets are catered for. Service users are assisted with feeding where necessary. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 and 18 Service users are confident that their complaints and concerns are dealt with and are acted upon. EVIDENCE: The home’s complaints procedure is available to service users and visitors as it is on display in the main entrance to the home. Service users commented that if they had a complaint or a concern that they would speak directly with the manager. There are adequate policies and procedures in place to deal with complaints. There have been no complaints received by the Commission. The home’s adult protection policies and procedures are in place. 80 of the staff team have undertaken training in this area. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,23 and 26 Service users’ rooms are maintained well but communal areas are “shabby” and unwelcoming. EVIDENCE: The main entrance into the home requires re-decoration, which was made a requirement at the last inspection. This has not yet been completed. Which makes the main entrance into the home appear shabby and unwelcoming. The organisation should give priority to the re-decoration of this area. The home overall was clean and free from odours except for two areas. One of which was in the main entrance hall and one bedroom. This was discussed with the manager who stated that this would be addressed. Service users rooms have been personalised making them as homely as possible with their own possessions. The home has four bathrooms on the ground floor and 2 on the first floor. There is a passenger lift and stair lift available.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28 and 30 Service users receive a good standard of care from staff that are competent and trained. EVIDENCE: The staff rota showed that there are four care staff on each morning and afternoon. There is two care staff on duty each night. There is a separate rota in place for all ancillary staff. Staff training is ongoing with staff attending all the statutory training as required. Staff have had access to various training such as Safe Handling of Medication, Dementia Awareness, Infection Control and so on. The home’s registerd manager is completing training in the Management of Aggression. There are 6 staff that have completed NVQ Level 2 and above. 2 staff are to commence NVQ Level 2. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35,37 and 38 Service users’ health, safety and welfare are generally promoted and protected with one exception regarding fire precaution. EVIDENCE: Service users finances were inspected. The home operates the organisations procedures, which were being followed. Service users’ individual monies could not be inspected to ascertain they balanced as monies are held together in one account although individual records for all service users’ finances are maintained. Receipts are obtained for all purchases and two signatures are also obtained for all monies received and withdrawn. Regular audits are carried out by the organisation’s Regional Support Administrator. A number of health and safety records were inspected all of which were up to date and accurately maintained. However several fire doors were wedged open

Advice was given for the home’s manager to seek advice regarding the installation of appropriate systems from the Fire Safety Officer.
