The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home continues to offer a warm and friendly environment, with a staff team who have worked at the home for many years. Staff work well together as a team and provide good continuity of care. As seen from the above comments residents and their relatives are happy with the care provided by management and staff at Coote Lane.

What has improved since the last inspection?

An updated and revised service users guide has been produced, which give good clear information about the services and facilities offered by the home. Medication policies and procedures have been reviewed and there has been an improvement in the system.

What the care home could do better:

Assessment and care planning should include information about resident`s social, cultural, interests and hobbies back grounds. Staff who are foreign nationals should have POVA and CRB clearances prior to taking up employment.

The home is registered for a maximum of 20 service users of the category OP (Old Age, not falling within any other category). 23rd February 2006 Date of last inspection Brief Description of the Service: Coote Lane provides residential care, for up to 20 service users, male and female who are over the age of 65. Coote Lane is a purpose built home, situated in 5 acres of land within walking distance of the village of Lostock Hall. Accommodation comprises of 20 single rooms on two floors with lift access for the 1st floor, all with the required furnishings, and a sink. Two have en suite facilities. There are also 2 bathrooms, one shower room, four separate toilets, and three lounge/quiet areas. There are also seating areas outside, accessible through patio windows from the lounge. Fees for a place in the home are currently £380 per week. This was an unannounced site visit and took place over a five-hour period. A pre inspection questionnaire was completed by the manager. The inspector consulted care records and spoke to most of the residents living at the home. He discussed their care with them and visiting relatives. Comments received from residents and relatives were positive and were complementary about the care they receive. Their comments include: “They treat me very well”, “The care that I have received is excellent”, “I am very happy at Coote Lane”, Relatives comments included; “We are pleased with the level of care my relative receives in this home”, “The care provided by Coote Lane residential home is of the highest standard. The management and staff are at all times considerate and caring”. The inspector toured the building, spoke to individual staff, had discussion with management and consulted records and policies and procedures. Questionnaires were issued to residents and relatives. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3 Quality in this outcome group is good; this judgement has been made using available evidence including a visit to this service. The home has a good information and assessment procedures, which ensure resident’s needs are comprehensively assessed and appropriate care plans made. EVIDENCE: A new revised service users guide has been issued and gives clear updated information to residents, a copy of this has been made available to the CSCI. The assessments of four residents were looked at. A full pre admission assessment had taken place, which identified care to be provided including risk assessments. While the assessments contained good information about care to be provided the sections for social interests, hobbies, religious and cultural Coote Lane Residential Home DS0000049738.V302374.R01.S.doc Version 5.2 Page 9 needs had not been completed. This should be done to ensure that a person centred approach to care planning is developed. For residents who are funded by social services there should be a social workers assessment on file. None of the files looked at had these. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome group is good, this judgement has been made using available evidence including a visit to this service. Residents care plans are comprehensive and ensure health and personal needs are fully met, care is provided ensuring privacy and dignity is maintained. EVIDENCE: During the visit the files of four residents were looked at and their care discussed with them. Each resident had an individual care plan, which provided comprehensive information and detail to ensure that health and personal care needs can be fully met. Care plans are reviewed monthly, residents and their families are able to take part in the process. Care plans and reviews contained signatures of the resident or their representative. All these records were up to date and provided detailed information. Medication policies and procedures were looked at and records examined, these are well kept and been up dated. A risk assessment is carried out to ensure any resident wishing to self medicate is able to do so. There is a clear description of all drugs dispensed stating what they are for and when they should be given. All staff giving out medication have received training in this. A regular monitoring of temperatures in the medical fridge and medication storage room is carried out. Residents said that they are happy with the care they receive and said that staff are kind and considerate, their comments included; “They treat me very well”, “The care that I have received is excellent”, “I am very happy at Coote Lane”, Relatives comments included; “We are pleased with the level of care my relative receives in this home”, “The care provided by Coote Lane residential home is of the highest standard. The management and staff are at all times considerate and caring”. Good interaction between residents and staff was observed during the visit. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 Quality in this outcome group is good; this judgement has been made using available evidence including a visit to this service. The home provides residents with choice and routines are flexible, this ensures they are able to exercise control in everyday life. EVIDENCE: The home provides a good variety of activities for residents. The inspector looked at the activity folder, in which there was recorded activities provided, these include, dominoes, bingo, board games, entertainers, shopping trips, trips to garden centres arts and crafts. During the visit residents were enjoying a session in flower arranging. Two activity organisers are engaged for two hours each fortnight. Residents said that there is always something for them to do if they want to be involved. The home provides flexibility with regards to daily routines and each resident has their own preferred getting up time in the morning. The service users guide says; “We offer support and encouragement to older people seeking to maintain an independent lifestyle”. Menus showed that there is a good variety of food available and residents are asked what they like. Meals can be taken in resident’s own rooms if they prefer to eat in private. Residents told the inspector they are happy with the meals they receive. The inspector had a meal during the visit, this was of good standard and well presented. Residents are encouraged to maintain their own financial affairs as long as possible. Information regarding access to independent advocacy services is available in the home. Bedrooms are personalised and residents are able to bring items of furniture, pictures and ornaments with them when they are admitted to the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. There are good arrangements in place for residents to raise concerns and the homes policies ensure residents are protected from abuse. EVIDENCE: The home has a complaints procedure, which is accessible to residents and their families. Copies are available on the homes notice board and are contained in the service users guide. Residents said that they are aware of the procedure and feel able to raise any concerns with the manager who is always available in the home. The record of complaints is kept up to date and showed that complaints are dealt with appropriately and responded to within the required time. There is a protection of vulnerable adults procedure including a whistle blowing policy. Staff have received training in the protection of vulnerable adults and were able to tell the inspector about the procedures to be followed if they had any concerns. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome group is good; this judgement has been made using available evidence including a visit to this service. The home is well maintained and the environment is clean and comfortable. EVIDENCE: The have been improvements made since the last inspection and there is a programme of maintenance in place to ensure standards are always improved. There are plans in place to include another part of the building into the registration. There is a plan in place to ensure radiators are guarded and it is hoped to complete this during 2007. In the meantime risk assessment are in place to ensure more vulnerable residents are protected. Residents were happy with their accommodation and their rooms are personalised with their own ornaments and furniture. There are policies and procedures in place to ensure infection control and the home is kept clean and hygienic. At t he time of the visit the home was observed to be clean and comfortable. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30. Quality in this outcome group is good; this judgement has been made using available evidence including a visit to this service. Arrangements for staffing the home ensure there are sufficient skilled staff on duty to meet the needs of residents. EVIDENCE: Recruitment files for staff were looked at and showed that there is a thorough recruitment process in which references are obtained and Criminal Records Bureau checks carried out prior to employment of staff. One member of staff who had come from abroad had a work permit and clearances had been taken up for this to be completed. However there was some confusion as to whether CRB checks should be carried out, following advice the manager was told that all staff including foreign nationals must have a CRB clearance. Arrangements to do this were actioned immediately Staff records showed that new staff have a comprehensive induction and training to enable them to carry out their duties. A training matrix has been produced which shows staff have received the required training. Certificates of training were seen on staff files. Staff on duty were spoken to individually and said that they are well supported and are given the opportunity to take part in training. 50 of staff are qualified to NVQ 2 and more staff are presently completing this training. Rotas seen showed that there are always sufficient experienced staff on duty. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. The home is well managed, which ensures residents interests are protected and health and safety issues are promoted. EVIDENCE: There is a new manager in the home, who is applying for registration, she has nine years experience in providing care and has been deputy manager of the home for three years. There is good support from the responsible individual of the company owning the home and regular visits are made. Residents and staff are happy with how the home is run and there are clear lines of accountability. Resident’s views are taken seriously and resident surveys have been carried out. Residents meetings are arranged and the manager is in daily contact with all the residents. The home looks after resident’s personal allowances, these are kept in a safe and good records are maintained of any transactions. All the homes policies and procedures have been reviewed and updated. Health and safety is taken seriously and staff receive training in moving and handling, health and safety and first aid. All safety certificates and risk assessments are carried out and are up to date.
