What the care home does well

The home provides good quality nursing care to service users as outlined in individual care plans. This is provided by competent staff in a caring and sensitive manner. The standard of accommodation is of a high standard with all but two bedroom having en-suite facilities. The meals are varied and the menus cater for a wide range of needs and choice. The activities offered meet the collective and individual needs of the service users.

What has improved since the last inspection?

Most of the requirements from the last inspection have been met. All service users have a needs assessment in place prior to admission. The standard of record keeping has improved. The records relating to fluid balance and outstanding employment documents are now in place. The fire safety records are now well maintained.

What the care home could do better:

Individual care plans are in place, which have improved since the last inspection. However some of these remain unsigned. All care plans must be signed by the service user or their designated representative following discussion.The care plans must also include the social aspect of care. The home has been without a registered home manager for eleven months. The acting manager resigned her post in August 2005. The organisation is currently making every effort to recruit a new manager and in the interim have made arrangements for senior corporate managers to share the responsibility for this post. This requirement is outstanding from the last inspection and has been carried forward.

One named service user under the age of fifty (50) may be accommodated. 7th April 2005 Date of last inspection Brief Description of the Service: Corrina Lodge is a purpose built home providing accommodation and nursing care for fifty-one service users. The home is located on the outskirts of Camberley in a residential area. The town centre, railway station and local amenities are within easy walking distance. Accommodation is provided on two floors, with a stairs and passenger lift access to the first floor. There are fifty-one single rooms, forty-seven of which have en-suite facilities including either a bathroom or a shower. There is an attractive communal lounge and dining room on each floor. The home stands in secluded well-maintained gardens that have sheltered terraced areas and raised flowerbeds. There is on-site parking available at the side and the front of the building. This inspection was announced and the second in The Commission for Social Care Inspection programme year 2005/2006. The inspection took place over 5.5 hours and was undertaken by Mary Williamson Lead Inspector for the service. Mr Rhys Evans, Sheena Wyllie, and Mrs. Mary Dannfald were present throughout the inspection. There was opportunity to talk with most of the service users, some in more detail than others. Five relatives were also spoken to. The feedback and comments received regarding the care being provided was very good. A tour of the premises was undertaken and records relating to the care of the service users and the management of the home were examined. On the morning of the inspection there was a story telling and music session taking place in the main lounge and this was well attended. During the afternoon a church service was organised and this was also popular with service users. There was a relaxed and happy atmosphere in the home. Staff were interacting in a professional and caring manner with service users and visitors. The home is well maintained, clean and tastefully decorated. Bedrooms are comfortable and have been personalised to reflect individual personalities. The catering arrangements are well managed offering a wide and varied choice of menu. The meals seen during the inspection were appetising, nutritious and wholesome. The number and skill mix of staff on duty met the assessed needs of the service users in the home. The management of the home is currently being shared by two senior corporate managers. Mary Dannfald the Regional Operations Manager is in the process on recruiting a home manager and is monitoring the situation daily. A requirement has been made in this report. The inspector would like to thank the service users, relatives, staff and management for their hospitality and positive contribution to the inspection process. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2 and 3. Prospective service users have the information necessary to make an informed decision about the home. The assessment process makes sure that prospective service users needs can be met. EVIDENCE: There is a statement of purpose and service users guide in place. All prospective service users and their relatives have access to a copy of this prior to admission to the home. This is currently being updated to incorporate the amalgamation of Westminster Health Care and Barchester Health Care Groups. Terms and conditions of occupancy are in place and were sampled during the inspection. These contracts include the room to be occupied, and the amount of fees payable and method of payment. A selection of these assessments were sampled and are satisfactory. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, and 10. Service users health and personal care needs are met by competent staff in a respectful and dignified manner. The administration of medication is in line with the homes medication policy. EVIDENCE: Individual care plans are in place, which have been developed with input from service users, relatives, health care professionals, and the pre admission needs assessment. A selection of care plans were sampled and contained detailed information regarding health and personal care needs. These must be expanded to include social care needs and must also be signed. All service users are registered with local G P’s who visit the home on a regular basis. Two service users stated that they have access to regular chiropody and dental treatment. An outside optician service also visits the home. Arrangements were being made with the local surgeries for service users to have their annual flu vaccine. There is also input from the tissue viability nurse. One service user stated that she had a pressure sore for a very long time prior to admission to the home and this has now healed since admission. They were observed to knock on service users bedroom doors prior to entering and explain procedures prior to undertaking them. Medication is managed and administered in line with the homes medication policy and The Nursing and Midwifery Councils Code of Professional Conduct. The required documentation was in place and all medication records are well maintained. There are currently no service users who self medicate. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, and 15. Service users living at the home are provided with a comprehensive programme of activities, which meets individual and collective needs. The catering arrangements are satisfactory and well managed. EVIDENCE: On the morning of the inspection there was a music and story telling session taking place in the main lounge on the ground floor. This was well attended and sounded very entertaining. The activities co-ordinator explained the monthly activities programme which she produces with input from the service users. This is varied and includes activities such as board games, music and movement, gentle exercise. Several service users stated that they are satisfied with the level of recreational activities provided particularly the celebrations, like birthdays and organised parties. Several service users have a daily newspaper and choose to read this in quietness. Individual activities are also organised to meet assessed needs. Two relatives stated that they are made welcome in the home at any time. They were both having lunch with their individual relatives during the inspection. They also stated that they are involved in the care planning process and regular reviews of care. The chef with input and feedback from the service users plans menus. Daily menus are displayed on each table for lunch and supper. The choice and range of food offered is wholesome, nutritious and appetising. Special diets are catered for. The comments and feedback from the service users regarding the food were satisfactory. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 There is a complaints procedure in place, and service users and relatives questioned were satisfied with this procedure. EVIDENCE: The home has a complaints procedure in place, which is part of the service user guide. This is available to all service users and their relatives on admission to the home. Relatives and service users questioned stated that they did not have to make a complaint but felt happy with the procedure in place. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24,25,and 26. Service users individual and communal accommodation is of a high standard, which is comfortable and well maintained. Ample toilet and bathroom facilities are provided some of which have been adapted to meet mobility needs. EVIDENCE: The standard of accommodation is very good offering all service users single rooms with all but two with en-suite facilities. Several service users and relatives stated that the accommodation is very comfortable. Bedrooms are personalised and there was evidence that service users are encouraged to bring individual items of furniture and their personal belongings from home with them. Corrina Lodge Nursing Home DS0000017600.V253623.R01.S.doc Version 5.0 Page 17 The home provides a comfortable lounge and dining room on each floor with an extra lounge area on the first floor for some of the younger service users who have different social needs. Facilities are in place to support the less mobile service user with access to a physiotherapist and occupational therapist when required. The home is clean, well maintained and free from offensive odour. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, and 30. The number of staff on duty during the inspection was sufficient to meet the assessed needs of the service users. The recruitment procedure in place protects the service users living there. EVIDENCE: The staffing arrangements in the home meet the assessed needs of the current group of service users. There were three qualified nurses and ten care staff on duty. There was also one housekeeper, three cleaners, a laundry assistant, two chefs, one kitchen assistant, one dining room assistant, an activities coordinator and an administrator on duty during the inspection. There was opportunity to talk with staff who all confirmed the recruitment process, and induction training received. Staff had a good understanding of individual and collective needs of the service users in their care. Three staff employment files were sampled and all the required documentation to safeguard the service users was in place. Some staff questioned stated that they have a job description and contract of employment. One RGN is the designated trainer for the home and undertakes all the health and safety training in house for all the staff. The operational manager stated that all new overseas staff undertake one week residential training in the organisations office in Oxfordshire prior to commencement of employment. Two staff confirmed this. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 34, 36, and 38. Currently the home dose not have a registered home manager. Leadership is being shared by two senior administration managers to ensure the safety and welfare of the service users. The health and safety of service users and staff are promoted. EVIDENCE: Since the last inspection the previous acting manager has resigned. On the day of the inspection the management of the home was being shared by Mr Rhys Evans, who was a former home manager at the home now working at senior corporate management level and Sheena Wyllie Special Projects Manager. They are also overseen by Mary Dannfald, who is Regional Operations Manager. This was followed up by a serious concerns letter following the resignation of the acting home manager in August 2005. A further requirement has been made following this visit. The Operations Manager informed the inspector that an advertisement has gone to press regarding this post. Qualified nurses supervise the care staff. This supervision needs to be formalised. The fire safety records were seen and are well maintained. All staff receive fire safety training during their period of induction. This is updated regularly and recorded. Contracts are in place for the maintenance of the fire fighting equipment. There is a designated health and safety representative and trainer allocated for the home. Staff receive induction and regular updating in these procedures. During a tour of the premises the inspector observed several mops and buckets stored in the toilet allocated to the kitchen staff. An alternative storage space must be found for these items in line with health and safety. Accidents are recorded and records are well maintained.
