The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service users and relatives who provided feedback spoke highly of the service. They had enjoyed the recent function `tea at The Ritz`. One resident stated that she was happy here but it wasn`t like her own home of course. One resident told the inspector that she had lived there for nine years and runs the book club, which entails her reading to some of the other service users. One visitor to the home stated that she was always made welcome and the staff was very kind.

What has improved since the last inspection?

The requirements from the previous inspection have been met. Service users or their representative now sign care plans and the home has now appointed a manager whose application for registration has been submitted to The Commission for Social Care Inspection.

One named service user under the age of fifty (50) may be accommodated. 22nd September 2005 Date of last inspection Brief Description of the Service: Corrina Lodge is a purpose built home providing accommodation and nursing care for fifty-one service users. The home is located on the outskirts of Camberley in a residential area. The town centre, railway, and amenities are within easy walking distance. Accommodation is provided on two floors, with a stairs and passenger lift access to the first floor. There are fifty-one single rooms, forty-seven of which have en-suite facilities including either a bath or shower. There is an attractive communal lounge and dining room on each floor. The home stands in secluded well-maintained gardens that have sheltered terraced areas and raised flowerbeds. There is on- site parking available at the side and front of the building. This unannounced inspection took place over eight hours and was the first key inspection to be undertaken in the Commission for Social Care Inspection year April 2006 to 2007. Lesley Garrett lead inspector for the service, carried out the inspection and the, acting registered manager, represented the establishment. A tour of the premises took place and the inspector saw some service users and spoke to some of them in more detail. A pre-inspection questionnaire was used to write some of this report and the home has been sent some service user survey forms (comment cards) but these will not be ready to assist with the report. Records were also sampled as part of the inspection process including care plans and policies and procedures. This was a positive inspection and the inspector would like to thank the service users, staff and manager for their time, assistance and hospitality during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Pre-admission assessments take place prior to a service user moving into the home. The home does not provide intermediate care. EVIDENCE: The general manager or the matron does all pre-admission assessments prior to the service user moving to the home. The inspector saw examples of these assessments in the service users individual folders and found them to be comprehensive. The home uses a recognised assessment tool and care plans are then generated from this assessment. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9, & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The service users individual plans of care are comprehensive and demonstrate that their health and personal care needs are met. The home has policies and procedures in place for the safe handling of medications and no issues around privacy and dignity were raised. EVIDENCE: The general manager told the inspector that the home is in the process of changing their care plans as the old system was too limiting for their needs. The home, therefore, has two systems in place and is gradually changing as new service users are admitted. The inspector saw examples of both systems and found them to contain a good variety of care plans and risk assessments which all showed evidence of monthly updates and the involvement of the service user or their representative. Both day and night staff completes daily notes. All care plans contain a tissue viability risk assessment, continence assessments and nutritional assessments. The opticians visit twice a year and on the day of inspection they were delivering new glasses for some of the service users. The chiropodist visits every six to eight weeks and the home also has access to the dietician and speech and language therapist. Due to the needs of a particular resident the respiratory nurse consultant visits regularly to support the home. The home uses the monitored dose system for their medications and the manager stated that they have good support from their local pharmacy that has audited the medicines recently. No gaps were noticed on the medication administration record and there were sample signatures of all registered nurses that administer the medications. Fridge temperatures are checked daily and the fridge was only being used for appropriate medications. All registered nurses have received medication training. The inspector saw a copy of the homes privacy and dignity policy, which was adequate, and this subject is also covered in the homes induction training. All rooms are single en-suite and the service users can see their guests in their own room. There are adequate communal facilities if they wish to use them. All visits by visiting professionals are conducted in the service users own room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. Arrangements for meeting resident’s social needs were a good standard and based on individual needs and expectations. Residents were able to exercise choice and control over their lives as much as they are able. Visitors were made welcome by staff. The presentation of food and arrangements for provision of meals was to a good standard. EVIDENCE: The home employs a full time activities organiser who had organised the previous week a ‘tea at The Ritz’ themed afternoon. Some of the service users were complimentary about this and said how much they had enjoyed it. There is a gardening team which a service user organises with the help of the activities organiser and enjoys being out in the greenhouse. Music therapy also takes place with one to one sessions with the younger service users. All activities are then written up in the service users individual plans and all will soon have an assessment plan, which the manager and activities organiser have devised. There are church services every week and a local school visits to entertain the service users with their singing and dancing. All visitors can be seen in private and there are no restrictions to visiting. The inspector saw evidence that service users were given choice in their daily lives. There is choice for every meal and also where they eat as meals can be taken in their room if they wish. Service users can choose their room dependent on availability. Choice of activities is also available. The inspector spoke with the chef who has knowledge of the dietary needs of the service users and special diets are catered for. The chef told the inspector that all soups and cakes are home made and the inspector saw this during the inspection. Fridge and freezer temperatures are taken and all food in the fridge is colour coded. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The home has policies for both complaints and protecting adults and they are both adequate and ensure that service users are protected. Protecting adults training has taken place for all staff. EVIDENCE: The homes complaints policy is available for each service user and is also displayed in the reception. The manager stated that there have been four complaints since the last inspection in September. All have been resolved and evidence was seen that the manager had investigated these concerns and responded to appropriately in the homes stated timescales. The home had two protecting adults issues in January 2006. One meeting has taken place. The Commission is now awaiting confirmation from social services that these matters have been satisfactorily concluded under the local authority safeguarding adult’s procedures. The manager and staff have received training in protecting adults and have a copy of the February 2005 Surrey Multi Agency procedures. There have been no further issues since January. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The home is well maintained internally and externally and is clean and free from offensive odours. EVIDENCE: The home has a full time maintenance man and there is a routine programme of maintenance. Rooms are decorated when they become vacant and if there is a need in between. The garden is well maintained and the home employs a gardener. There is also a gardening club and a service user has access the green house and can supply the home with flowers for the garden. The home was clean and free from offensive odours. The laundry is well maintained and has a sluicing facility on the washing machine. All service users have individual boxes in the laundry, which their clothes are placed in for delivery to their rooms. All staff had training in infection control and the home has plenty of soap dispensers and hand towels for use by the staff. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 & 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The staffing levels in the home are sufficient to meet the needs of service users. The home is able to demonstrate that service users are supported by trained and qualified staff that is able to carry out their role competently. The home ensures that adequate checks are completed prior to staff working in the home, but the manager needs to look at the application forms to ensure that there are no gaps in the employment record of staff. Staff training is carried out on a regular basis. EVIDENCE: The manager used a recognised tool to regularly assess the dependency level of the service users and the home is usually staffed to a higher level. The inspector saw the staff rotas. The manager stated that staff is helped by a hostess who does tea and coffees and helps at mealtimes. The home also has adequate domestic help and the laundry is staffed every day. The home enables staff to do their national vocational qualification training as the matron is an assessor for this qualification and two other staff is in the process of training to become assessors. The home does not use agency staff. The inspector sampled employment folders, and although all necessary checks were made prior to employment, there were gaps in employment history for some staff with no explanations given. This will therefore be a requirement at the end of the report. All staff receives induction training for one week and they all have their own individual training folders. There is a training plan for the year and mandatory training has taken place and more planned for example, manual handling, fire awareness, food hygiene, first aid and protecting adults. All registered nurses have also received resuscitation training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has appointed a new manager since the last inspection that is qualified for the post. The home has some quality audits in place but they need to seek the views of the service users and make this available for them. The financial interests of the service users are safeguarded and the health and safety of all service users are promoted and protected. EVIDENCE: The home has appointed a new manager since the last inspection and her application for registration with the commission is presently being processed. She has clear lines of accountability and is well supported in her role as her manager visits regularly and is always available by telephone. The homes senior manager responsible for that service completes monthly visits. The manager now needs to develop a quality audit system that seeks the views of their service users and relatives or representatives. The results should then be made available for current and prospective service users and this will be a requirement at the end of the report. Financial interests of service users are safeguarded and the sundries account is organised by the administrator. Adequate records are kept and the inspector sampled this. Manual handling training has taken place for all staff and it was stated that first aid training would also be available. Food hygiene training and infection control training has taken place and is on a rolling programme as new staff are recruited. The home has a full time maintenance man and he keeps records of all his regular checks including fire alarms and water temperatures. Environmental risk assessments are in place for safe working practices, which include the kitchen and laundry.
