Conditions of registration: 1. 2. 4 named adults under 65 years of age Two named service users under 65 years Date of last inspection 6th September 2006 Brief Description of the Service: The Court Care Home is situated in a residential community and is close to all local amenities and is on a public transport route with easy reach to Birkenhead town centre. Accommodation for service users is provided on three floors. Communal space is situated on the ground floor and includes a lounge with access to the dining room. There is a serving hatch from the kitchen to the dining room. There is a non-smoking lounge and a separate designated smoking room. The dining room is accessed through the lounge. This is an overview of what the inspector found during the inspection. Information about The Court was obtained through a pre-inspection questionnaire and examination of the homes policies and procedures and supporting documentation. Discussion took place with residents, the Registered Manager and members of the staff team. A tour of the building also took place. Fees £490.00 - £623.00 per week. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are only provided with a service after their needs are assessed and they have been assured these needs will be met. EVIDENCE: An assessment of residents care needs is carried out to ensure staff have all the information they need on how to look after the them in accordance with their individual needs. Risk assessments have been completed to ensure residents safety and welfare, however, a review date had not been set, rather it was noted the risk assessments should be reviewed when necessary. A clear system of review must be set up to ensure staff have the most current information they need on how to ensure residents safety and welfare. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents health care needs are monitored and met so ensuring residents are treated in a dignified and respectful way. EVIDENCE: A documented plan of the support provided to each service user is in place and gives staff guidance and information on residents individual care needs. Resident’s health care needs are monitored with a record of their daily welfare being kept. The residents spoken to during the visit said they regularly see health care professionals such as their GP, chiropodist or dentist. The residents spoken to during the visit confirmed the staff treat them with respect and their privacy and dignity is always upheld. The residents spoken to said the staff are not bad, ok and very kind and helpful. One resident said the staff are better than anywhere she has lived before. The staff spoken to understood the need to treat the residents with respect and ensure their right to privacy was upheld. The Registered Manager said she intended to

Systems are in place to ensure residents receive their medication as prescribed by their GP and only qualified nursing staff are allowed to handle residents medication. The residents spoken to confirmed they receive their medication on time. Appropriate storage facilities are in place and all medication administration record sheets were up to date. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The routines within the home are flexible, which means residents, can exercise choice in their daily lives. EVIDENCE: A range of social activities is provided at the home and an activity organiser is employed for five hours each day. Residents social care needs are assessed and activities are provided in groups and on an individual basis. All residents have an opportunity to go out into the community. A religious minister visits the home each week and arrangements can be made for residents to attend church if they wish. During discussion residents confirmed they could go about their day as they wish with little restriction in place. They confirmed their friends and relatives can visit at any time. A varied menu is in place and residents spoken to said they enjoyed the meals and always had plenty to eat and drink. Systems are in place to ensure the kitchen is kept clean and tidy, which was the case on the day of the visit. Lunchtime was observed during the visit with the following issues being raised:
Court Nursing Home DS0000020944.V335127.R01.S.doc Version 5.2 Page 13 • • • • The dining room was bare and unwelcoming in that it had no homely touches such as pictures or ornaments etc. The dining tables were not set before residents sat down to eat their meal. Plastic picnic beakers were being used instead glasses. Residents were only given a dessert spoon to eat their meal rather than a knife and fork. A number of residents at the Court need assistance to eat and two staff were seen helping them in a polite and discreet manner. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): 16 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The systems in place for the protection of residents from harm and abuse are robust. EVIDENCE: The pre inspection questionnaire indicated the home has not received any complaints within the last 12 months and no concerns have been made to the CSCI. Staff spoken to were aware of the action they should take in the event of them receiving a complaint. All staff are trained on protecting vulnerable adults from abuse. During discussion they demonstrated an understanding of the different types of abuse that can occur and the action they should take in the event of them suspecting or knowing an incident of abuse had occurred. Each member of staff is given a pack of information when they commence employment with a copy of the adult protection procedures within it. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of the decor ensures people live in a comfortable and homely environment. EVIDENCE: There is a large garden at the back of the home which residents can use at any time, however, this area is untidy and did not provide a pleasant environment for residents to sit. There is an ongoing programme of refurbishment in the home were bedrooms and communal areas have been decorated. There are sufficient bathing facilities however some of the bathrooms whilst functional are bare and basic in appearance and require some attention.

Sufficient laundry facilities are in place along with systems to prevent the spread of infection. The laundry was extremely tidy and it is noted the laundry staff are clearly working very hard to ensure their area of work is kept to a high standard. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff recruitment process and training ensure suitably qualified and competent staff are employed. EVIDENCE: Staff are provided with induction training when they are first employed. This ensures staff have the information they need on how to look after the residents in accordance with current good practice. The pre inspection questionnaire indicated that staff have completed a range of training within the past 12 months and that further training is planned for the forthcoming the year. Many of the staff confirmed they have under taken a national vocational training qualification. All of the staff spoken to said they enjoyed their work and they are well supported in their role. They confirmed the Registered Manager and administrator is always available for advice and support when necessary. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People live in a home, which is run and managed to ensure that staff, friends and people who use the service feel valued. EVIDENCE: The Registered Manager has been managing the home for seven years. She is qualified to NVQ level 4, which is the recognised qualification for a manager of a care service. There are clear lines of accountability within the home for all staff. Court Nursing Home DS0000020944.V335127.R01.S.doc Version 5.2 Page 19 The pre inspection questionnaire indicated that a range of policies and procedures relating to the care of the service users and the running of the home are in place and have been reviewed and updated earlier this year. Systems are in place to ensure the ongoing monitoring of the service. Staff are supervised and monitored in their role and administrative systems are reviewed. The Registered Provider visits the home on a regular basis and carries out his own quality assurance assessment. A new and more comprehensive system of quality assurance has recently been introduced for the purpose of monitoring and improving the service provision. The administrator takes responsibility for the management of residents daily money. The Registered Provider manages other finances. A selection of financial records and money was inspected and noted to be in good order. Residents health, safety and welfare are promoted in the home. The pre inspection questionnaire indicated the equipment in the home such as the fire system, hoists, and electrical wiring etc is checked / serviced regularly; associated records and certificates are in place to validate this. During discussion staff confirmed they have completed a range of training in this area. A health and safety checklist is completed each week to ensure everything is in good order.
