Conditions of registration: 1 - Up to 37 beds providing Nursing Care for older persons from the age of 60 years. 2 - Of the Older People accommodated, up to 9 may be in the category DE(E) Older People suffering from Dementia. 3 - Of the Older People accommodated, up to 3 may be in the category PD(E) Older People with the Physical Disability. Date of last inspection 16th November 2004 Brief Description of the Service: Coxhill Manor Nursing and Residential Home is an extended Elizabethan Manor House, situated on the outskirts of Chobham. The Service is privately owned by Coxhill Manor Limited and is run and operated by the Principals, Dr & Mrs Swift-Hook. Both Principals are closely involved with the day-to-day operation of the home. A Matron/Manager is employed to oversee the general running of the home. Care is offered for up to seventy-four older people in single and shared rooms, all with en suite facilities. There are extensive gardens and communal areas, all of which are actively used by the service users. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 5.75 hours and was the first inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2006. This inspection was carried out by Denise Débieux, Lead Inspector for the service. Mrs J. Varndell (Registered Manager) was present as the representative for the establishment. A tour of the premises took place. Five care plans, four staff files, criminal record bureau checks, and the staff rota were all inspected. Four of the service users were spoken to at length; with a further five service users and six on-duty staff being spoken with during the tour. This was a positive inspection. The inspector would like to thank the staff and service users for their time, assistance and hospitality during this inspection. What the service does well: What has improved since the last inspection?
The staff have worked hard to ensure that service users’ needs are appropriately assessed and that their care is planned to ensure that these needs are met. Much thought has been given to improving the layout and facilities on The Gables unit, with some successful changes already having been made. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 Each service user is only admitted to the home following a needs assessment to ensure that the home can meet the service user’s identified needs. The home does not offer intermediate care. EVIDENCE: The five care plans sampled during this inspection were seen to be based on comprehensive assessments of needs and clearly set out staff actions required to be taken in order to meet those needs. In conversation with a relative of one service user it was clear that she was confident that her relative is well cared for. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 10 Personal and social care needs and assistance are planned and were seen to be provided, where needed, in a respectful and sensitive manner. EVIDENCE: Service users spoken with were complimentary regarding the care they receive at the home. One service user stated ‘they are terribly kind here.’ One visiting relative confirmed that they felt their parent was well looked after. The home has introduced a new care planning system, on a trial basis, on one unit since the last inspection. The five care plans inspected were comprehensive and clearly set out actions which need to be taken by care staff to ensure that all aspects of the personal care needs of the service users are met. The home have now decided to introduce the new system to a second unit. The home have recently employed a new activities co-ordinator who is meeting with each service user with a view to tailoring individual activity care plans that take into account their present and previous interests. The inspector acknowledges the hard work this has involved and congratulates the staff on their success at implementing and working with the new system. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 14 Service users are encouraged to exercise choice and control over their lives. EVIDENCE: Service users handle their own financial affairs where appropriate. The inspector was advised that the home do not handle the financial affairs of any service users. The service users’ rooms seen all contained many personal possessions. One service user said how much she enjoyed having her mementoes from her previous house around her. A high degree of personal autonomy and choice was evident at this inspection. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 All required policies and procedures are in place to ensure that service users are safeguarded from harm or abuse. EVIDENCE: The home have a copy of the latest Surrey Multi-agency Procedure for the Protection of Vulnerable Adults. The senior care staff have all been on the course provided by Surrey Social Services with the information then being cascaded down to the remaining staff. The inspector was advised that all staff receive training in the basics of protecting vulnerable adults during their induction. All of the service users spoken with said they felt safe at the home. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The location and layout of the home is suitable for it’s stated purpose. It is accessible, safe and well-maintained with a pleasant and homely atmosphere. EVIDENCE: During the tour of the home the premises were seen to be well maintained with service users able to access all areas of the home and grounds. All service users spoken with expressed their satisfaction with the accommodation provided at the home. On the day of inspection the home was found to be warm and bright with a homely atmosphere and a high standard of housekeeping apparent. Improvements in the layout of The Gables unit have been made since the last inspection. One bedroom has been made over to provide a separate dining room with the previous lounge/diner now being used solely as a lounge. This has given the unit a far more spacious feel and provides the service users with a larger area to relax in, as well as increasing their freedom of movement.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30 The numbers and skill mix of the staff meets service users needs. The home has a comprehensive staff recruitment and training programme which incorporates all areas needed to ensure, as far as reasonably possible, that service users are in safe hands at all times. EVIDENCE: The staff rota inspected demonstrated that the staffing numbers and skill mix are appropriate to meet the assessed needs of the service users living at the home. The level of staffing on The Gables has been maintained at an increased level to ensure that the higher needs of the service users on that unit are met. Staff described how much less rushed the day is now on that unit, with time being available to do more activities with the service users. One service user stated ‘they are very good people here.’ Ten of the twenty eight care workers are qualified to National Vocational Qualification (NVQ) level 2 or above in care, with a further four care staff enrolled on the course. The inspector was advised that extra staff will be enrolled as places become available and the home expects to have at least 50 of their carers qualified to NVQ level 2 before the end of this year. The home have revised their recruitment procedure and now send a covering letter with all application forms saying that a full employment history with an explanation of any gaps must be provided. The files for the four latest staff members recruited were inspected and all were found to contain the required information and pre-employment checks.

Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 The home has a quality assurance and monitoring system in place that is based on seeking the views of the service users. EVIDENCE: The home’s quality assurance and monitoring system is based on seeking the views of service users and includes informal information gathering and formal surveys. There is a residents’ committee at the home that holds three monthly meetings which are chaired by members of the committee. The home have recently surveyed GP’s and other visiting health care professionals and is awaiting the return of the survey forms. Monthly monitoring visits to the home are carried out by the responsible individual as required.
