The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Cranlea provides a pleasant homely environment for the people who live there. Independence is encouraged and residents live in "flats" with en suite facilities where a kettle, toaster and fridge may be provided to enable people to make snacks and hot drinks. Staffing levels are good and the overall quality of care is good.

What has improved since the last inspection?

A new system has been introduced to record and update daily information and to relate this to care plans. Satisfaction surveys have been developed which are used regularly to gather the views of people living in the home and their visitors.

What the care home could do better:

Activities are organised informally and more work is required to engage residents interests and to develop a range of suitable activities and pursuits. Alternative main courses are offered at meal times and people may choose preferences for breakfast and teatime. Menu planning should allow for an alternative dessert to be made available and a wider range of fruit should be offered to residents to allow for personal taste and diet.

The care home has a large lounge, conservatory and a dining room on the ground floor. There are a number of quiet seating areas, an additional lounge and two dining rooms on the first floor. In addition Cranlea offers a small library, a telephone kiosk and hairdressing facilities for residents. There are communal toilets located on each floor and residents have access to two bathrooms and two additional assisted bathrooms in the home. Cranlea is located in Kingston Park to the north of Newcastle with access to local shops, churches, a GP surgery and public transport. Cranlea is a residential care home, which provides a service to 39 people. This report is of an unannounced inspection undertaken on 10th May 2005. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1-5 Cranlea meets the standards required in offering people a choice of home. People who require information are given a comprehensive brochure which clearly sets out statement of purpose, costs, the services provided and the complaints procedure. A full assessment is undertaken by the manager and social worker prior to admission. People are given the opportunity to visit the home and to share a meal with residents. EVIDENCE: One woman spoken with during the inspection confirmed that she had been given a choice of home and that her family had helped her to choose Cranlea. She had been given a copy of the information brochure and said that her daughter dealt with all the paperwork and had the contract and forms. She commented that she had found the adjustment difficult and would have preferred to remain at home, however the staff had been very kind and were helping her to settle in. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-11 Cranlea meets the standard required in relation to the health and personal care of residents. Each resident has a care plan, in which health, personal and social care needs are recorded. There is a comprehensive policy to deal with medication and some people take responsibility for their own medication, which is recorded in care plans. EVIDENCE: A cross sample of care files was examined during the inspection and confirmed that care plans are comprehensive and relate to assessment. Residents and carers are encouraged to have input to the development of care plans and needs are reviewed on a regular basis. Care plans examined reflected health care needs of residents and referral to health professionals. A new system of daily records has been introduced to improve information gathering. The medication procedure have recently changed and is currently under review. Staff are trained in drug procedures and administration and records examined were accurately maintained. Cranlea has a policy relating to death and dying and people are given the opportunity to indicate their wishes in advance. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 The social, cultural, religious and recreational interests are not being fully met. People are encouraged to exercise choice in daily routine and meals and visitors are welcomed to Cranlea. EVIDENCE: On the day of inspection local radio was playing loudly in the lounge whilst a number of people were reading and one woman was engaged in sorting activities. There was no evidence of any organised activities other than bingo, which is provided three times per week. V.E. Day occurred during the week of the inspection and may have offered the opportunity for reminiscence and collective activities however this was not marked in any way. Staff were heard talking to residents about changes in Newcastle and of memories of the past informally, and offered choice in playing board games. There is a computer in the lounge for residents but this was not being used by anyone. Residents spoken to said that they were bored by the lack of activities offered and one woman said that she would like to go out to the local shops but that this was not possible as the staff were busy. A home activities questionnaire has been developed and as a result extra sessions of bingo have been introduced. Visitors are encouraged at Cranlea and residents are encouraged to exercise choices and to go out with visitors. There is a four week menu cycle and people are given a choice of main course, however there were no yoghurts available on the day of the inspection as an alternative to the milk pudding offered and the fresh fruit available was limited. One woman commented that she bought her own fruit as “they never have satsumas”. Cold drinks are available in the dining rooms and fluid charts are maintained where required. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16-18 Cranlea meets the standards required in providing a complaints procedure. Information regarding advocacy is available for residents. There is a policy in place to deal with abuse and a commitment to training to promote awareness amongst staff of abuse and whistle blowing. EVIDENCE: Cranlea has a complaints procedure and records examined and discussion with residents confirmed that this is active and that people know how to make complaints. Cranlea has a comments book and compliments are recorded and passed on to staff. Postal voting was taken up by a number of residents during the recent election and information relating to advocacy services is displayed in the foyer. There is a whistle blowing policy and policies are in place to protect residents from abuse. Residents commented that they felt safe in Cranlea and that the staff were very good and listened to people. Staff spoken to confirmed that they had attended training and had an awareness of the needs of vulnerable adults. Staff confirmed that they could approach the management team and that regular supervision takes place where concerns may be raised. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19-26 Cranlea provides a safe, well maintained environment for residents and communal facilities are of a good standard. There are sufficient toilet and washing facilities to meet the needs of residents and specialist equipment is provided where required. Individual’s rooms are arranged to suit their needs and tastes and the building is clean and comfortable. EVIDENCE: There is a large lounge area downstairs, as well as a choice of smaller seating areas located on the first floor, and there is a pleasant conservatory and garden to the rear of the house. Residents have a choice of dining rooms on the ground and first floor and people are encouraged to bring their own furniture and to arrange their rooms to suit them. All rooms in the home have natural light and ventilation and window restrictors are fitted. There are facilities available for people to have a fridge and kettle in their rooms following appropriate risk assessments. There is a passenger lift available for people with mobility difficulties. Cranlea is a pleasant clean environment, which is free from offensive odours. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27-30 Service user’s needs are met by the numbers and skills of staff. Training is well regarded and actively promoted. There is a satisfactory recruitment policy in place to safeguard service users. EVIDENCE: There are 28 care staff of whom 5 are seniors and staff are organised into 5 staff teams operating to a keyworker system. 14 staff are qualified to NVQ level 2 and there are a further 7 staff currently working on developing NVQ skills. There is a commitment to training within the home and all staff are encouraged to participate in relevant training. Regular supervision is provided to all staff. Cranlea has additional kitchen and domestic staff on site. Staff files examined confirmed that satisfactory recruitment procedures are in place and interview records are kept. References are sought for all new employees and the required checks have been followed up and records kept. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,36,37,38. There is a commitment amongst management and staff to meet the needs of service users. There is a satisfactory management structure in place to safeguard the well being of people who live there. Staff participate in regular supervision and records are maintained. EVIDENCE: The manager has considerable experience and is fully qualified to manage the home. Mrs Coulson holds NVQ level four in Care Management and the Registered Managers Award in addition to Assessors and Verifiers Awards. The manager was not available on the inspection day due to training commitments, however subsequent conversations to follow up issues arising from the inspection confirm that she is fully aware of principles of good practice and is committed to applying this in the management of her staff Cranlea B53-B03 S439 Cranlea V221782 100505 Stage4.doc Version 1.20 Page 17 team and is keen to discuss and implement suggestions arising during the inspection process. Staff are provided with regular supervision and satisfactory records are kept. Quality Assurance questionnaires have been developed and are used at 6 monthly intervals and comments books have been introduced to determine the views of residents and families. Cranlea has administrative input and files are generally well maintained. The administrator has recently returned from maternity leave and has identified some gaps in recording and maintenance of administrative systems which she is working to rectify.
