The home is registered for a maximum of 6 service users in the category LD (Learning disability) The registered provider must, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection Staffing must be provided to meet the dependency needs of service users at all times and will comply with any guidelines which may be issued through the Commission for Social Care Inspection 15th December 2005 Date of last inspection Brief Description of the Service: Darley Cottage is a care home for six adults with a learning disability. Located in a residential area in Chester the home is close to shops, pubs and other local facilities. It is also on the bus route to the city centre. Access between the ground and first floors of the two-storey building is via the stairway. Five of the single bedrooms are in the main part of the home; a separate selfcontained flat is located on the ground floor. One large lounge and a lounge/dining area are located on the ground floor. One communal bathroom/shower/toilet is provided, as is a separate toilet. Two of the bedrooms have en-suite facilities. Mobility aids are provided for a service user with mobility problems. Staff are on duty twenty-four hours a day to deliver care to service users. Darley Cottage DS0000006610.V309896.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. An unannounced visit took place on the 17, 20 and 23 November 2006 and lasted 5 hours. Val Flannery, Regulatory Inspector, carried out the visit. The visit was just one part of the inspection. Before the visit the registered manager was also asked to complete a questionnaire to provide up to date information about the service. Questionnaires were also made available for service user’s, families, health and social care professionals to find out their views. Other information received since the last inspection was also reviewed. During the visit various records and the premises were looked at. A number of service users and staff as well as the registered manager were spoken with and they gave their views about the service. Information provided by the manager showed that fees range from £569.26 to £678.70. Further information about fees and other costs can be obtained from the manager. What the service does well:
The pre-admission procedure, that includes the statement of purpose and service users guide, is available for prospective service users and their families. This will help prospective service users and their representatives by providing information about the service and will also help them make an informed choice about the home. There is a range of information available about service users that show their individual needs have been identified and plans are in place to meet these needs. Risk assessments have been carried out that show how service users can be best supported in the home and in the local community. These also ensure the service users are enabled to choose how they live their daily lives whilst at the same time maintaining their safety and independence. Service users are encouraged to make decisions about their chosen lifestyles, for example, where they spend their leisure time in the home, when they have their meals and make full use of available activities. Relatives are able to visit the home as they wish and are kept informed of events/accidents/incidents that affect service users. Service users’ have access to a range of health care

The complaints procedure has been provided in a format that makes it easier for service user’s to understand. The location of the home makes it easily accessible for relatives and other visitors. Six service user’s, three relatives and one health/social care professional comment cards were returned. Service user’s received help from staff when completing the comment cards. Comments included • I would have no hesitation in recommending this service • The staff are always so kind and helpful to the residents and nothing is to much trouble for them The service user’s comments were positive about the service they receive. The timescale for ensuring all new staff receive training appropriate to their roles could be improved. All parties should sign the service agreement between service users and MacIntyre. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1/2/4/5/ Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Information is available about the range of services offered by the home. This ensures prospective service user’s have the information to make an informed choice before making a decision about moving in. EVIDENCE: A copy of the statement of purpose and service user guide is available in the home. A copy of the service user guide is kept in individual service user files. The current service user group has not changed since the last visit. MacIntyre have a procedure that allows prospective service user the opportunity to ‘test’ drive the home before making a decision about moving in. This includes overnight stays, visits for meals and to meet service users and staff. Copies of tenancy agreements between service users and MacIntyre were seen on service users personal files. Two of the service users spoken with during the visit said they are ‘ fine’ and that they are able to do the things they want to do. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6/7/8/9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Plans of care are in place that show the level of assistance individual service users require with their daily lives. EVIDENCE: The care file titled ‘Care Package’, that included the Person Centred Plan, for one service user were seen during the visit. This showed how the service user wished to live her life and the level of support required from staff. Also seen were risk assessments that covered the home environment and the level of support required by service user’s with daily living tasks. Care plans are reviewed at least yearly with the service user and key worker. Family/care manager are involved if appropriate. Service user’s were seen making decision on, for example, what they wished to take with them for their lunch whilst at the day centre. Staff were seen advising service users on what clothing to wear because of changing weather conditions. Service user’s spoken with talked about their daily routines and what they like doing. They said they are helped by staff, where necessary, but they like to do as much for themselves as possible.

These set out the responsibilities of the organisation and service user with regard to living in the home. The agreement seen during the visit was not signed by the service user, her representative (where appropriate) or by a representative from MacIntyre. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11/12/13/14/15/16/17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service user’s daily routines are individually geared to ensure they are able to maintain fulfilling lifestyles, both in the home and in the local community. EVIDENCE: Service users spoken with said they ‘like living in Darley. They said they are able to use community facilities like shops, pubs and restaurants. One service user said he works in a fast food outlet and also does his own shopping. A list of daily activities for individual service users was seen during the visit. This included attendance at day care centres, art and craft classes and in-house domestic chores. The location and appearance of the home is in keeping with the local community. This also helps ensure easy access to local community events and services. Service user’s said they are able to maintain contact with family and friends. One service user said he regularly visits his parents, whilst another service

Service users also said they are involved in planning the menus which means they can choose food they prefer and like. A record was seen of the food offered to service user’s. The member of staff spoke with was aware of the responsibilities that go with the role of a support worker. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18/19/20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service user’s receive support with their personal and healthcare needs in a way they prefer. This helps ensure their wishes and choices are respected. EVIDENCE: Records were seen that showed service user’s healthcare needs are met including visit to doctors surgeries and that they are supported by staff to attend hospital appointments. Service user’s said they ask to see the doctor if they are not feeling well. A record was seen whereby a service user, following a seizure, had sustained an injury and was taken to the accident and emergency department of the local hospital for treatment. Other records seen showed that advice has been sought from healthcare professionals on managing challenging behaviour. MacIntyre have provided policies and procedures on the administration of medication. Service user’s medication is kept in a locked cupboard. A record was seen of medication administered, this was satisfactory. Three service users said they are able dress themselves and take a bath/shower by themselves with minimal staff support. Records were seen of

The member of staff spoken with during the visit was aware of the level of support required by individual service users with personal care tasks. Service users were seen approaching the staff for help with dressing or preparing food and for advice and guidance on personal issues. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22/23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Satisfactory procedures are in place to ensure service users are protected from abuse. Service users, and other visitors to the home, have access to a complaints procedure that enables them to raise issues of concern. EVIDENCE: A copy of the complaints procedure is available in the home. Details on how to contact the Commission for Social Care Inspection are included in the procedure. The manager and staff spoken with said the home has not received any complaints since the last inspection. A copy of the complaints procedure has been provided in picture format that makes it easier for service users’ to understand. MacIntyre have provided an Adult Protection Procedure, a copy of which is kept in the home. Included in the procedure is a copy of the government guidelines ‘No Secrets’. The member of staff spoken with said they knew about the complaints and adult protection procedures and what to do if a problem arose. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24/25/27/28/29/30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Sufficient private and communal space is provided to meet the needs, and suit the lifestyle, of the service users. EVIDENCE: The home provides a comfortable and safe environment for service users. It is well-maintained and provides sufficient private and communal space for service users. Service users are accommodated in single bedrooms that are, according to service users, suitable for their needs. One service user is accommodated in a self contained flat that is attached to the home. Two bedrooms were seen during the visit, these were individually furnished and decorated. A communal bath/shower/toilet is located on the first floor and a toilet on the ground floor. The self contained flat has its own bathing and toilet facilities. One bedroom has en-suite shower and toilet; another bedroom has en-suite shower facilities.

An assisted bath and walking aid is provided for a service user with mobility problems. On the day of the inspection the home was clean and free from unpleasant smells. The detached garage is the process has been converted to a games room; this has improved the in-house leisure opportunities for service users. Since the last visit the lounge and dining room have been re-decorated. The home employs a cleaner for two hours a day Monday to Friday. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31/32/33/34/35/36 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are supported by staff that are aware of their assessed needs and how these needs are to be met. EVIDENCE: The staff spoken with during the visit said the manager discusses their roles and responsibilities with them in one to one supervision and at staff meetings. They also said the majority of the staff team have worked together for a number of years. During the inspection staff were seen talking with and helping service users. Service users were comfortable in the company of staff. Staff said they are encouraged to attend training opportunities provided by the organisation. However, they said getting a place on the course could be difficult as there is a high demand for places from staff within the organisation. The two staff on duty had not received any of the core training since joining the organisation. The record of training seen included food hygiene, first aid, fire awareness and caring for service users. Five of the eight staff have achieved an NVQ level 2 or 3. At other times there is normally one member of staff on duty. A member of staff who sleeps in the home provides nighttime cover. The registered manager said that recently there has been a higher then normal rate of sickness amongst the staff. She also said staffing was, in their absence, provided by staff from within other MacIntyre services in Chester. An agreement has been reached between the Commission for Social Care Inspection and MacIntyre on a format for maintaining staff details in the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37/38/39/42/43 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The overall management of the home is good and is geared to improving the quality of life for the service users. EVIDENCE: The registered manager divides her time between managing Darley Cottage and another MacIntyre home in the Chester area. Since the last visit this arrangement has been formalised and she is now the also the registered manager for the other home. Staff spoken with said that they receive advice and guidance on any issues that may arise from the manager. The area manager makes monthly visits to the home and reports of these visits are sent to CSCI. The manager has worked for the organisation for a number of years in a senior capacity. Service users and staff said the manager is approachable, will listen to any concerns/worries and will take action to address the issues raised. Darley Cottage DS0000006610.V309896.R01.S.doc Version 5.2 Page 22 During the visit discussion took place with the manager on how the high level of staff sickness has affected her role as registered manager for both homes. Staff said the manager offers support and guidance on the delivery of care. She is keen to ensure the needs, and views, of service users are met. Health and safety records were seen that fire safety training, evacuation drills and fire safety checks are carried out. Also seen was a copy of the service record for the boiler, health and safety risk assessment and portable appliance tests.
