This inspection was carried out on 6th March 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Pre admission assessments carried out by the manager are detailed and help ensure that the home can meet the needs of the residents. The home has an in-depth care planning system with resident and/ or family involvement wherever possible. All healthcare needs are met, with an up to date record showing all professional healthcare visits and external appointments. Efforts are made to ensure that a nutritious and varied menu is provided, with a choice at each meal.

What has improved since the last inspection?

A new kitchen has been installed in Silverdale wing and new lounge carpet has been laid in the same wing. Nine of the bedrooms have been re-decorated.

What the care home could do better:

Discussions with the manager and staff confirmed that they are continually striving to improve the level of care and support given to the residents. Care is taken to ensure the running of the home is always in the best interests of the people living there. There were no requirements made at this inspection.

The service must at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. A maximum of thirty eight older people (OP38) of whom 26 have dementia may be accommodated (DE(E)26) The staffing levels for the home must meet the Residential Forum Care Staffing Formula for Older Adults by 1st April 2004. When single rooms of less than 12 sqm usable floor space become available they must not be used to accommodate wheelchair users, and where existing wheelchair users are in bedrooms of less than 12 sqm they must be given the opportunity to move to a larger room when one becomes available. One named person under 65 years of age with dementia may be accommodated (DE1). 5. Date of last inspection 14th September 2005 Brief Description of the Service: Dentholme is a care home operated by Cumbria Care, an internal business unit of Cumbria County Council. The home is registered to provide personal and social care for up to 38 older people, 26 of who may have varying forms of dementia, one being under 65 years of age. The registered manager is Mrs Catherine Watters. The home is situated in the middle of a housing estate close to the centre of Cleator Moor and all its amenities. Dentholme is purpose built on two floors, the upper being serviced by a passenger lift. Accommodation for residents is in four units, each with its own sitting and dining areas and kitchenette. There are thirty-eight single bedrooms, four of which have ensuite facilities. There are accessible toilet and bathing facilities close to all the accommodation used by residents. There is a large room on the ground floor that can be used for activities, visiting entertainers and parties. This room is also used as a venue for Cumbria Care internal training courses. There is a garden area, which is accessible to all residents and is used by them in the summer months. This was an unannounced inspection of the home and took place over one morning. It was the second inspection of the annual cycle and those standards not assessed during this visit were inspected and met during the previous inspection that took place on the 14th September 2005. During the inspection the inspector spoke with residents, staff, one of the supervisors and the registered manager. Documentation concerning the care of the residents and running of the home was examined and some parts of the home were looked at. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 The home’s statement of purpose and resident guide provide prospective residents and/or their families with sufficient information to enable them to make an informed choice about moving into the home. EVIDENCE: The home’s statement of purpose and other information outlining the facilities on offer at the home are made available to all prospective residents and their families. There are also copies on display in the hall. All residents are given a contract and terms and conditions. The information available allows prospective residents to make an informed decision about moving into the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 The residents benefit from knowing all their wishes will be met with dignity and sensitivity EVIDENCE: The home has a comprehensive policy outlining the procedure to follow on the death of a resident and the wishes, wherever possible, are noted on the care plans. Family members are offered hospitality and facilities at the home if they wish to remain with residents. Discussions with the manager confirmed that every effort is made to ensure all the wishes of the resident are met with care and sensitivity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 There is a sociable atmosphere in the home and residents benefit from being able to enjoy and follow their own routines. EVIDENCE: Discussions with the registered manager evidenced that, although residents meetings have been proposed on several occasions, they have not been well attended. There are monthly meetings held on each of the four units to discuss the needs of the residents and the most advantageous way of obtaining the opinions of those living in the home. There is a designated member of staff who is responsible for organising activities for the residents either on a one to one basis or in groups. Details of resident’s abilities are documented on the care plans and the activities are organised accordingly. A detailed record of all activities is kept and was made available for the inspector during the visit. Residents are free to choose how they spend their day and where they spend their time. Some were sitting in the lounges and others were in their rooms where the inspector was able to talk with them. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21, 22 & 25 The home offers comfortable and homely accommodation contributing to a good quality of life for the residents. EVIDENCE: Four of the bedrooms have en-suite toilet facilities and there are four communal bathrooms and ten toilets for use by the residents. These are situated on both floors close to the resident’s living accommodation. The home provides hoists and other specialist equipment in order that the independence of the residents can be maintained as long as possible. All the bathing and toilet facilities are suitable for those who may have a physical disability and there are handrails on the corridors to assist with movement around the home. There is central heating throughout and all rooms have natural light and ventilation. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Staff are well trained to ensure they have the competencies to meet residents’ needs. EVIDENCE: The staff team is both qualified and experienced in the care of older people. Cumbria Care provide an annual training plan and individual training needs are identified through staff supervision. Training courses completed include, infection control, in-house medication procedures, dementia care and health & safety. Fifteen members of staff are already qualified to NVQ level 2 and another one is due to start the course at the end of March. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 37 Residents benefit from the home’s policies and procedures that ensure their rights and interests are safeguarded. EVIDENCE: The home has a full set of policies and procedures that are put in place by Cumbria Care. These are updated regularly according to information received by the organisation. All members of staff at the home are asked to read three or four policies each month and these are then discussed during staff supervision. Emphasis is placed on those policies pertaining to the individual staff member and their role within the home. Discussions with the manager evidenced that the staff benefited from this process and that it was useful in their personal and professional development. This, in, turn means a higher level of care can be provided to those living in Dentholme.
