Conditions of registration: 1. The service should at all times employ a suitably qualified and experienced manager who is registered with the NCSC. 25th October 2004 Date of last inspection Brief Description of the Service: Dolphinlee House is situated on the Ridge Estate on Patterdale Road in Lancaster. The home is owned by Lancashire County Care Services (LCCS). The home is currently registered to take a maximum of 32 residents of both sexes who are 65 years or older. Dolphinlee House reopened in July 2005 after a long closure for major refurbishments. The home has now been extended to take a few more residents. LCCS is still deciding which category of clients to take when the registration is increased. The home has been tastefully fitted and decorated. There were eleven residents at the home at the time of the inspection. Some of them returned after spending many months at another home during the closure. They said that they liked their new home. All the facilities are good and modern. Accommodation is provided in single bedrooms and the majority of them have an ensuite facility. A rehabilitation unit is also available. This is an overview of what the inspector found during the inspection. This is the first of two statutory inspections to be carried out this year. The inspection was an unannounced one and took place on the 3rd November 2005 and lasted for 4.5 hours. The inspection was carried out against the National Minimum Standards for Older People. The inspection despite being an unannounced one was carried out in a nice atmosphere and with the full cooperation of the senior staff, the staff and the residents. During the inspection, some records were looked at and several residents and staff were spoken to. The residents were very positive about the care they receive and the way the staff treat them. One resident said “I am so glad to be back. They have done a good job.” There were 11 residents living at the home at the time of the inspection and there were 2 care staff, 1 senior carer, a cook and a domestic on duty. The number of staff on duty was well within the minimum level recommended. The staff were observed to be polite and kind when talking to the residents. What the service does well: What has improved since the last inspection?
The building has been extended and refurbished to a high standard. The area manager who was present during the inspection said that he is very pleased with the high standard of the building and the facilities. One resident who has returned to the home after the closure said that she is very pleased with her room and other facilities. She said that the home looked nice. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Pre admission processes are good. Prospective residents needs are fully met. EVIDENCE: All admissions to the home come via the Social Services Directorate (SSD) and a social worker is involved. The pre admission assessment for a new resident is done by the social worker in conjunction with the resident and/or the family. The assessment will include a brief history and will identify areas of needs and why residential care is needed. A visit by prospective residents and or their families is encouraged. A full assessment is carried once the new resident is admitted and continues all the time and regular reviews are carried to ensure that the level of care provided is appropriate. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 & 10 There is a good system in place to meet the health needs of the residents. Policies and training to care for the residents with respect are in place. Residents’ health needs are met and they are treated with respect. EVIDENCE: Two residents’ case files were examined and they clearly show that medical details of the residents were recorded. They include the name and contact of the GP’s. Other health professionals such as dentist, optician are also included if the resident has one. A brief medical history is recorded together with any medications the resident may be taking. If the district nurse is treating someone, this information is also written down. The district nurse keeps her own notes on the treatments she provides to the residents. The residents said that if they need a doctor, then one is sent for. They said that they prefer the doctor to visit them at the home as they are not fit enough to go to the surgery.

They said that they always knock before entering a resident’s bedroom. The residents said that as far as they are concerned all their rights as people are respected. One resident said “ It is lovely here and I can do what I like. The staff are good.” Policies regarding residents’ rights, privacy and dignity are available in the home’s policy manual. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 13 Policies and procedures to help residents maintain their social, cultural and religious needs are in place. Residents are able to practice their religious belief and maintain their cultural and social needs. EVIDENCE: The residents spoken to said that they are free to do what they want. They said that they can follow their religion and that priests and vicars visit the home on a regular basis. The residents said that the staff are good and that they would take them shopping and for walks if they wanted to. Some of the residents said that they prefer to spend part of the day in their rooms. Some of them were seen in their rooms when the inspector looked around the home. They said that they like the privacy of their rooms and that all the staff respect their wishes. They added that the staff will bring them a cup of tea to their rooms if they want. She said that the staff would play dominoes and scrabble with some of the residents. She said that as the number of residents in the home increases, then other activities would be introduced. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 There are policies and procedures to deal with complaints. Residents live in a safe environment and are not afraid to speak their minds. EVIDENCE: The home has a detailed complaint procedure which explains what people should do if they have a complaint. It says that in the first instance, people should complain to a member of staff unless the complaint is about staff. The next step is to speak to the manager or someone from LCCS. If this does not resolve the complaint, then the complaint can be passed to the Commission for Social care Inspection (CSCI). CSCI current address and contact telephone number should be included in the written policy. All complaints are recorded on an incident sheet. The home had received no complaints since the last inspection. The residents said that if they have any complaints, they would not hesitate to speak to someone about it. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home provides a safe environment and good accommodation for the residents. Residents live in a comfortable home. EVIDENCE: Dolphinlee House reopened in July 2005 after a long closure for major works. The home has been extended and extensive refurbishments carried out. Some of the residents who came back to the home were very complimentary about their new home and the facilities. They said that they like the new conservatory lounge and also the new furniture and fittings in their rooms. The home is currently registered to take a maximum of 32 residents who are aged 65 and over. The building has been extended to take more residents and an application to vary the registration has been received. The area manager said that LCCS is currently looking at what category of people they would like to take.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 30 The training of staff plays an important part in the home’s policy. A team of well-trained staff cares for residents. EVIDENCE: The person in charge said that all new staff are given an induction-training programme when they start work at the home. The induction training includes basic practices such as Moving and Handling, Food Hygiene and First Aid. The staff the inspector spoke to said that they have attended many training courses and found them beneficial. The staff said that they have attended the following courses: Abuse Awareness Health and Safety Medications First Aid NVQ The Commission for Social Care Inspection (CSCI) has recommended that 50 of care staff achieved the National Vocational Qualification (NVQ) level 2 this year. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 & 33 The management of the home is competent. Residents’ needs are fully met. EVIDENCE: Although the current manager at Dolphinlee House is very well experienced and qualified, he is not registered with CSCI to manage the home. The agreement for him to manage the home was a temporary one as the management is still deciding on the categories of residents for the home. The decision on which category of residents is admitted to the home would have direct effect on the person appointed and the qualifications and experience he or she has. The residents said that they are well looked after and that they can do what they want.
