Conditions of registration: Date of last inspection 26th August 2005 Brief Description of the Service: Eastfield Farm Residential Home is a privately owned care home that is registered to care for and accommodate 25 older people, including those with dementia. The home is situated in a rural setting and local amenities are only accessible via public transport or car.The accommodation comprises of seventeen single rooms and four shared rooms - eleven of the single rooms and one of the shared rooms have en-suite facilities. The first floor is accessible via a stair lift. Communal space comprises of three lounges and a dining room - some of these areas have open views over the countryside. The garden is easily accessible for service users and in the summer benches, tables and parasols are placed outside. There is ample parking space at the front of the building. This is an overview of what the inspector found during the inspection. This inspection took place over 5 hours, including preparation time for the inspector. The inspection included a tour of the premises and examination of documentation, including care plans. The inspector spoke to several service users, staff and the registered manager. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9, 10 Service users are treated with respect at all times. Medication systems are in place and the procedure is followed correctly. EVIDENCE: There is a medication policy in place and this reflects the advice given by the Royal Pharmaceutical Society. Staff who administer the medication have completed accredited training on the safe handling of medicines. Medication was observed being dispensed and this was done appropriately and the records seen were accurate and up to date. They allowed for an audit of medication to be carried out. Medication is appropriately stored and controlled drugs have a separate storage area. Throughout the inspection, staff were observed positively interacting with the service users. The service users and relatives spoken with said that the staff always treated them with dignity and respect. One relative said that they could visit their relative in private if they wished but enjoyed sitting in a quiet lounge where staff could also interact with them. A health professional spoken with said that any visits were always carried out in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13. Service users can have visitors at any time. EVIDENCE: Several visitors were spoken with during the inspection and they all said that the staff made them feel welcome and there were no restrictions as to when they could visit. One visitor spends long periods of time with their relative and the staff were seen to include them both when offering refreshments. Service users said that their relatives could visit any time. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The home is clean and odour free. EVIDENCE: All areas of the home were seen and found to be clean, odour free and well maintained. Laundry facilities meet the requirements of this standard. Staff have undertaken training on infection control. In the rooms occupied by service users with a continence problem, the cleaning routine takes this into account and windows are left open to allow them to air. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38. The health and safety of the service users and staff is promoted. EVIDENCE: The staff spoken with during the inspection said that they had received training in back care, first aid, food hygiene, COSHH, and fire training. The equipment used in the home is serviced at the prescribed intervals. Health and safety notices were displayed around the building. However, the current form of accident record keeping does not allow for the service users personal information to be kept in a confidential manner. Information from the accident records is used in the care planning process. All staff can access Induction and Foundation training, this provides them with basic skills required to undertake the role of resource worker.
