Conditions of registration: Date of last inspection 8th August 2006 Brief Description of the Service: Eaton Lodge is an older style two storey detached premises situated near the town centre and sea front. The home is registered for 24 older people requiring nursing care and provides both single and shared room accommodation. There is a pleasant garden at the rear of the building with seating arrangements, and some parking space is available at the forecourt. The Registered Person Mr M Patel visits the home on a regular basis, and the Registered Manager has been in post for several years and is an experienced first level nurse. This is an overview of what the inspector found during the inspection. The quality rating for this service is 3 star. This means that people who use this service experience excellent, quality outcomes.
This report contains the findings of the home’s key inspection and takes account of information obtained from various sources since the last inspection of 08 August 2006, including an Annual Service Review dated 31 December 2007 and a visit to the home. The key inspection visit was unannounced and carried out on Tuesday 08 July 2008 between 10.00 and 15.30 hours. During the inspection the inspector spoke with several residents, the registered manager and staff on duty. Various records were seen and an accompanied tour of some areas of the home was made. Information contained in the completed Annual Quality Assurance Assessment (AQAA), was comprehensive and information and observations made at the time of the inspection visit has been used when completing this report. Three completed service user surveys together with one service user survey completed by a relative have been received providing positive comments about life at the home for example ‘completely satisfied with the home and the staff are brilliant’, ‘I find the staff very helpful and understanding’ and ‘nothing is too much trouble’. The findings of this inspection were discussed with the registered manager at the end of the visit, and overall indicate that this home provides sustained good quality outcomes provision for the residents and in line with CSCI ratings agenda this service has achieved an excellent 3* rating. What the service does well:
There is a well-trained, enthusiastic staff team that provides a high standard of care. The attitude of staff was positive, friendly and helpful and practices seen indicated a supportive and caring service is provided to meet the needs of residents. The registered manager is experience and competent, supported by a deputy manager and a stable staff team. The records of administration of medications were well recorded and up to date. The home has good links with other professional services. Residents are able to make choices about their daily routines and lifestyles, doing things at the times of their choosing. Residents were appreciative that the staff spent time with them individually; talking to them and being there for them, ready to listen if they have a worry. Residents confirmed they liked the food at the home and a varied and nutritious diet is provided with alternatives available. Food seen at the time of the inspection visit was well presented and looked appetising. The recruitment procedures and records seen are thorough, thus protecting residents at the home. Staff training is ongoing with a number of care staff having achieved NVQ Level 2, going on to undertake NVQ Level 3. This service has consistently provided good outcomes enabling this to be viewed as an excellent service in line with Inspecting for Better Lives. What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The homes Statement of Purpose and Service User Guide provide service users and prospective service users with the information they need to make a decision about moving into the home. The assessment process is thorough and service users move into the home knowing that their needs can be met and that their independence will be maximised and promoted. Standard 6 was judged as not applicable at this inspection visit. Eaton Lodge Nursing Home DS0000026091.V367423.R01.S.doc Version 5.2 Page 10 EVIDENCE: The Statement of Purpose and Service User Guide were seen, and contained information as required by Standard 1 and Regulation 4 and 5. The documentation was seen to contain an annual quality assessment report for the home completed by the registered manager. The AQAA documentation states each service user is admitted to the home following a pre-admission assessment to establish their individual needs. A copy of the Statement of Terms and a Brochure are provided. Full counselling is provided to relatives and relevant needs are discussed with the family/relatives/service user to cover all aspects of care as appropriate. This is documented in the care plan. A family history questionnaire is given for families to fill in to highlight relevant details about their past e.g. personal, social and vocational, home-life and leisure. This becomes a relevant part of therapeutic activity. A comprehensive completed pre-assessment documentation was seen for two of the most recently admitted residents. The documentation seen provided full information about the persons needs and supports the statement made in the AQAA documentation as stated above. It is not the general policy of the home to admit people on a short-term basis, and the registered manager confirmed that short-term care was only provided occasionally. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7,8,9,10 and 11 were inspected at this visit. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Residents benefit from care plans that show that person centred care is promoted, and that health needs are being met. Personal care is offered in a way to protect service users privacy and dignity. The systems for medication administration are robust with clear and comprehensive arrangements being in place to ensure service users medication needs are met. Staff are supportive and sensitive to the needs of service users at the time of their death, and they and their family are treated with all due respect. Comprehensive records including care plan, daily records, reviews, and risk assessments were contained within the folders. Additional documentation included for example admission assessment, care needs summary (first 24 hours), quality assurance and audit together with nutritional and dependency tools. Overall the documentation was seen to be person centred comprehensively completed and provided a clear audit trail of all actions taken. The AQAA documentation states attention is paid to pressure area care and all changes are documented. The information seen in the care planning documentation at the time of the visit confirms this statement. Medication records were seen and indicated that they were appropriately signed and up to date. Medication storage although not viewed at this visit, had previously shown that only medications appropriate to residents needs were stored. The registered manager said that discussion had taken place in relation to the purchase of a second medication trolley to aid staff in the administration of medication. The records indicated that residents have access to a range of professional services, and the registered manager gave for example a resident that had recently received the services of a speech therapist Residents confirmed that they are treated with respect and their dignity is upheld. Comments from service user surveys include ‘ I find the staff helpful and understanding’ and ‘always acts on requests’. The inspector observed a good rapport between staff and residents. This was also supported by comments by relatives/visitors to the home. From discussion at the inspection visit it was evidenced that the staff are supportive and sensitive to the needs of a resident who is dying, family may stay at the home overnight if they wish to, and every effort is made to ensure the resident comfort, and close contact is maintained with family and professional services. There was evidence seen of this at the time of the inspection visit. This service has consistently provided good outcomes enabling this to be viewed as an excellent service in line with Inspecting for Better Lives. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,14 and 15 were inspected at this visit. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Residents can be confident they will have satisfactory opportunities regarding lifestyle choices. Residents are supported to maintain contacts with families and friends and enjoy a balanced and healthy diet. EVIDENCE: All residents that the inspector spoke with at this visit spoke highly of the home, and about their different lifestyles within the home. Flexibility in routines was seen to suit individual needs. Activities are undertaken that suit the needs of the residents and includes reading and poetry, massage and gently exercise, one to one time, visits out to local shops/seafront. One staff member talked about taking a resident out to play Bingo at a local establishment. Eaton Lodge Nursing Home DS0000026091.V367423.R01.S.doc Version 5.2 Page 14 The AQAA documentation states that visitors are able to come at anytime and to stay for as long as they wish. They are able to make their own drinks and refreshments. Spouses, close relatives and friends are invited to stay for lunch at no extra cost. Visitors were seen arriving at the home during the course of the inspection visit and good interaction was seen with staff and residents. The home endeavours to maintain a close knit family community. The AQAA documentation states that spiritual and religious observance is maintained. The home organises for ministers to visit. A minister for the Church of England visits weekly and offers support when required. All residents spoken with commented on how they enjoyed the food at the home. Food records were seen, these had not always been completed and this was discussed with the registered manager who agreed to address this issue. The meals seen provided on the day of the visit, were well presented and looked appetising. People feel their rights as citizens are recognised and promoted, including fairness, equality, dignity, respect, and autonomy over their chosen way of life. This service has consistently provided good outcomes enabling this to be viewed as an excellent service in line with Inspecting for Better Lives. This judgement has been made using available evidence including a visit to this service. Residents and visitors know their complaints will be listened to and acted upon. Staff have good knowledge and understanding of adult protection issues, which protects the residents from abuse. EVIDENCE: The home has a satisfactory complaints system and residents feel that their views are listened to and acted on. The registered manager confirmed that there had been no complaints since the last inspection visit. No issues of concern or complaints have been received at the Commission office during the last twelve months. It was evidenced from talking with staff at the inspection visit that they were aware of the policies and procedures in place in relation to adult protection, and were able to talk through what action they would take should an issue of concern be raised. Staff have received training in relation to Safeguarding Vulnerable Adults and the registered manager confirmed that a further training session is booked for 04 August 2008. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents live in a clean and comfortable home that is continually improved by an ongoing programme of refurbishment and redecoration. EVIDENCE: The AQAA documentation states that the home is friendly, clean welcoming and safe. There are no offensive odours on entering the home. A hygienic and clean environment is maintained. These statements were confirmed during the accompanied tour of some areas of the home. Eaton Lodge Nursing Home DS0000026091.V367423.R01.S.doc Version 5.2 Page 18 It was seen during the accompanied tour of the premises that a number of bedrooms had been re-decorated, and re-decoration which is ongoing. Part of the communal area of the home has been re-painted. Work has been undertaken to repair part of the roof of the home. Many of the bedrooms were personalised to meet individual needs. Matching soft furnishings are purchased as necessary, and refurbishment is ongoing. Re-decoration of bedrooms is undertaken when rooms become vacant or at the residents convenience. A call bell system is in place, and it was seen in a number of the rooms viewed that the call bell was always within the residents reach. Residents said that staff were attentive and acted on any requests they may have. Adaptations are provided as required. A number of new beds have been purchased and these aid staff in relation to moving and handling and meeting resident’s needs. A further adaptation seen was in relation to equipment that was used to enable persons unable to get out of bed to have their hair washed. Appropriate equipment was seen in relation to the safe disposal of clinical waste, and both category bags for disposal of clinical waste was seen. The completed AQAA documentation confirms that all maintenance contracts are up to date and that the building complies with the fire safety requirements. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Resident’s benefit from the well-trained, enthusiastic and supportive staff team. The homes practice regarding the recruitment of staff ensures residents are protected. EVIDENCE: A stable and effective staff team in sufficient numbers are employed at the home. The staff rota was seen and indicated that there is sufficient staff on duty at all times to meet the needs of the current group of residents. Four staff files were seen at the inspection visit and these contained all documentation that would support that a thorough recruitment system was in place, including application form, references, CRB checks, copy proof of identity. Completed induction paperwork was seen in staff files. Eaton Lodge Nursing Home DS0000026091.V367423.R01.S.doc Version 5.2 Page 20 The AQAA documentation completed by the registered manager indicates that twelve carers have NVQ Level 2 and the registered manager said that 6 carers are going to undertake NVQ Level 3. Residents spoken with said that staff often comment on training that they have undertaken. The AQAA documentation states that a training schedule is maintained for every member of staff. Staff spoken with confirmed that training is ongoing at the home, and said that they had recently had fire training. One new member of staff said that they had recently undertaken training in Food Hygiene, Infection Control and updated Moving and Handling. Staff training is ongoing to ensure that staff develop the skills and knowledge enabling them to meet the needs of the residents. People are well supported by a staff team that recognises and responds appropriately to their diverse needs and human rights. This service has consistently provided good outcomes enabling this to be viewed as an excellent service in line with Inspecting for Better Lives. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,32,33,35,36 and 38 were inspected at this visit. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Resident’s benefits from good leadership from the manager who is well supported by the senior staff team in providing clear leadership throughout the home and residents can be confident that their home is well run. The health, safety and welfare of residents and staff are promoted and protected. Eaton Lodge Nursing Home DS0000026091.V367423.R01.S.doc Version 5.2 Page 22 EVIDENCE: The registered manager is experienced, competent and qualified to manage a registered home that provides nursing care. She continues to update and undertake additional training. The registered manager is aware of the national minimum standards and the regulations that underpin them and undertakes quality monitoring within the home, ensuring that residents and significant others are consulted. There is an open management style, which encourages the staff to play an active part in improving the service. The staff commented favourably on the positive impact that the management team has had indicating that there is good consultation and open dialogue. Discussions with residents also indicated that management provides a positive open and inclusive atmosphere. The Registered Manager reported at the inspection visit that currently the home does not hold any personal allowance monies for residents and therefore no written records are kept. There is a system in place at the home for the regular supervision of all staff with written records kept. The Deputy Manager is currently taking on this responsibility, and staff confirmed that dates are being booked. The AQAA documentation indicates that maintenance certification is up to date, and it was evidenced that the maintenance person is completing any work that requires undertaking. Currently one resident said he was painting her bedroom ceiling where the roof had leaked, prior to the roof being repaired. People enjoy as good a quality of life as possible because the home is managed in a way that takes into account and promotes their human rights and equality and diversity needs. This service has consistently provided good outcomes enabling this to be viewed as an excellent service in line with Inspecting for Better Lives.
