Conditions of registration: 1. Elderly persons over 60 years of age Date of last inspection 7th July 2005 Brief Description of the Service: Eden Court is a care home with nursing. The home provides care, nursing and accommodation for up to forty-five people. The home is purpose built. It has thirty-seven single rooms, 4 double rooms and 10 of these bedrooms have en-suite facilities. There is a large lounge, one quiet lounge, a small smoking lounge and a quiet sitting area on the upstairs landing. There is a large garden at the home where the residents are able to sit in the better weather. The home is situated within close proximity of the A58 Leeds Road, with access to the public transport links and local shops. Ample car parking is available at the home. This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried out over a seven and a half hour period and was the second inspection conducted during this inspection year. The inspector made a tour of the building and looked at a sample of the records kept by the home. The inspection was conducted with the manager of the care home. The inspector spoke with some of the residents, staff and a relative. What the service does well: What has improved since the last inspection? What they could do better:
The care records should clearly identify the residents’ changing needs. The present arrangements for the disposal of medication are not satisfactory and must be reviewed. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. The care needs of the residents are assessed prior to admission. EVIDENCE: The care records for one resident recently admitted into the home held the details of the care management assessment and the manager had completed a detailed pre-admission assessment. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Generally, the residents’ health and personal care needs are set out in their plan of care. The residents are treated with dignity and respect. The general management of medication is satisfactory, however the registered manager must review the current arrangements for the safe disposal of medication. EVIDENCE: The residents spoken with said that they are satisfied with the level of care provided by the staff. A relative spoke highly of the standard of care provided at the home and said she had been actively involved in the care planning process. During this inspection the inspector observed the residents being treated with dignity and respect. There has been a general improvement in the detail kept in the care records. Some of the care records looked at had been updated and reviewed. The inspector noted that the oral hygiene need of the residents is not clear. One resident had not been weighed for three months, the nurse on duty said this was due to a health condition but the care records did not indicate this. It is important that the care records reflect any changes so that when a risk is identified there is evidence that the appropriate action has been taken. The medication records for three residents were looked at and all could be reconciled with the medication records kept with the exception of one PRN (when required) medication. A discussion took place with the manager regarding the safe disposal of medication. The current practice adopted in the home is not acceptable and must be addressed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14. The residents are supported to make choices in their lives. EVIDENCE: The residents are supported to manage their own finances if they wish. The inspector was advised that the residents are able to bring personal possessions with them into the care home and, during a tour of the building, some residents’ rooms were seen and were personalised. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: This standard was not fully assessed on this occasion, however the residents spoken with, and a relative, said that they would feel comfortable to raise any concerns or make a complaint. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26. The home is generally clean and tidy but the manager must ensure that there are adequate facilities provided to safely manage clinical waste in the home. EVIDENCE: On the day of this unannounced inspection, the home was generally clean and odour free. There is ongoing maintenance and redecoration in the home, and there is a programme of renewal to replace all the furniture in the residents’ bedrooms. During a tour of the home, the inspector observed a clinical waste bag left open on the floor, this is not good practice and must be addressed. All clinical waste bags must be placed in a suitable container. A prescribed bath lotion and razors were left in a bathroom. This practice is not acceptable and poses a potential risk to the residents. (See standard 38).

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29,30. The home’s recruitment practice needs to improve to support and protect the residents. Generally, the home is able to demonstrate that the staff are trained and competent to do their jobs. EVIDENCE: The files for three staff that work in the care home were looked at in detail. Two of the records had gaps in the employees’ employment histories; this was identified during the last inspection and needs to be addressed. The organisation has an induction and foundation training programme for staff. The home receives some external support for the training of its staff in infection control, movement and handling and medication administration. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35,38. Quality assurance systems need to improve to ensure the home is run in the best interests of residents. Service users’ financial interests are safeguarded. The health, safety and welfare of residents and staff are not adequately protected. EVIDENCE: The organisation has a system for monitoring the quality of care provided by the home. However, this system needs to improve and include an annual questionnaire that seeks the views of the residents, their relatives and other stakeholders. The results of such surveys should be published with copies made available to the Commission for Social Care Inspection. The manager has

The money for three residents was checked and all could be reconciled with the financial records. The residents are supported to manage their own finances and they are provided with lockable space for any valuables. During a tour of the building the inspector observed poor movement and handling practice. The manager must address this to ensure the protection of the residents and the staff. All new staff should receive movement and handling training from a competent person prior to moving and handling the residents in the care home. The recording of hot water temperatures for bathing was discussed with the maintenance person. Razors and prescribed bathing lotion must not be left in the home’s bathrooms as it poses a potential risk to the residents.
