This inspection was carried out on 15th June 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The staff interact very well with the residents placed. Staff are continually observed talking and spending time with residents. The cleaning staff ensure that the building is always clean and free from odour.

What has improved since the last inspection?

The staff within the service have recorded individual residents interests and activities in their care plans. However there must be a daily list of activities undertaken by each resident placed. The building has been assessed by an occupational therapist to ensure it meets the needs of the residents placed.

What the care home could do better:

The service must ensure it changes to use the monitor dosage system, and does not continue to secondary dispense medication. Medication should be signed for correctly. Staff employed in the service spend a lot of time interacting with residents. Various activities are provided. These must be recorded. It is then clear what activities individual residents have been involved in on a daily basis.The service must find a way of assessing residents` views in the home, and formalising a quality assurance system.

Residents will have a diagnosis of dementia or mental illness on admission Date of last inspection 21 January 2005 Brief Description of the Service: Edendale is registered to provide thirty one places for older people with a mental disorder, including Dementia. The building is near to the town centre and set within its own grounds. The service is divided into two houses with a separate staff group in each house the staff group rotates periodically. The inspection began at 9.15 and lasted for two hours and 30 minutes. A full tour of the building was carried out and each room was found to be clean and free from offensive odour. Documents for recruitment of staff were examined on the day of the inspection. Accident records within the home were viewed. The Staff rota was viewed. Menus were viewed. Sheets where administration of medication is recorded were examined. Residents appeared happy and settled. Staff were observed interacting with residents. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Edendale gains the appropriate information about a resident, so as to ensure the home can meet that persons needs. EVIDENCE: Edendale have had one new resident move into the home since the last inspection. The service had ensured that they had obtained the relevant information. A letter from the older persons mental health services, and letters from the consultant. Relevant information about the medication the person was prescribed. There was a community care assessment and community care plan in place. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 9 Care plans were up to date and identified risks were recorded apart from in relation to one residents changing needs. However staff were aware of the increased risk around this person. EVIDENCE: Care plans are up to date and recorded appropriately. Risk assessments are comprehensive and have relevant information in with regard to particular identified risks for residents. All accidents are recorded in individual care plans, and risk assessments are updated if necessary. Staff interviewed were aware of the changing needs of the one of service users. However the risk assessment needed to be updated and recorded. One resident requires continued nursing care, this was discussed, and due to mobility and progressing needs a more in depth level of nursing care is required. The service is currently assessing whether they can meet this residents needs and will obtain suitable equipment if necessary. Sheets which record the administration of medicines were examined on the day of the inspection. Significant gaps were evident in them. These were due to staff not signing for medication when it had been given. The service is still using the system of secondary dispensing medication. This is an ongoing issue. However the owners are currently awaiting the blister pack system from the pharmacy so the correct procedure for administration of medication will be followed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 The home provides adequate activities. Staff interact with residents very well, and involve them in day to day activities within the home. EVIDENCE: Activities were discussed in the home. Activities are being provided for residents. However these are not recorded appropriately. Activities carried out the week of the inspection were as follows: on Wednesday a Nostalgia group. This is when three people, visit the service they bring a keyboard and sing. Musical instruments are given to resident to play and join in. The group play for a hour in each house. On Friday a women plays a keyboard for one hour for a movement and music session. Again she spends one hour in each house. On Tuesday a visiting performer visits the service and plays the keyboard. Again he plays for one hour in each house. The hairdresser visits once a week on a Sunday. Staff will sit and talk to residents. Staff could relay information about what a resident enjoys and what their interests are. Dominoes and Bingo are played with residents if they wish, and staff will read newspapers to them. Staff will take individual residents to the local library if they wish. Four residents access the community independently. Other residents are taken for walks or out in car journeys with staff. Ice creams are served in the garden during the good summer weather. Seasonal activities are offered, such as pantomimes at Christmas. Staff ensure they spend time with residents who are less able to initialise contact with staff. However throughout the inspection, staff were seen with residents at all times. They were either spending time with residents in administration of self care, or involving residents whilst carrying out general care duties. The staff were observed initiating contact with residents and would always speak to them and never walk past a resident seated, unless they spoke to them. JUDGEMENT – we looked at outcomes for standard(s) 26 The home was clean and free from offensive odour. EVIDENCE: All areas of the two buildings were viewed on the day of the inspection. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 The service is on target to ensure that 50 of the staff are trained to NVQ Level 2 or above. EVIDENCE: On the day of the inspection on duty there were a senior carer from 8am – 5pm, and three carers from 8am – 8pm. One carer from 8am – 2pm, and a cook from 8am – 2pm. In the other house were two carers from 8am – 2pm, two carers from 2pm – 8pm and one care staff member from 8am – 8pm. One new staff member had been employed since the last inspection. The staff assured the inspector that POVA and CRB checks had been completed for this person, although they were not available to view on the day. The service has 24 care staff in total. Out of these 3 staff are starting NVQ level 2. Three have completed NVQ level 2. The service employees 6 nurses, there registration is not recognised in the UK. One senior carer has completed NVQ level 3.
