The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents receive a high standard of care which meets their health and personal care needs. Care staff are knowledgeable and well trained and have developed positive relationships with the residents they support. The atmosphere in the home is open and friendly and residents know their views are listened to. Meals are well planned and residents receive a varied and nutritious diet which they enjoy. The home provides comfortable accommodation which meets the needs of the people living there. The home is well managed and is run in the best interests of the residents living there. Procedures are in place to protect residents` financial interests and welfare.

What has improved since the last inspection?

Areas of the home have been redecorated and refurbished and the environment is pleasant for people to live in.

What the care home could do better:

The smoking room is unhygienic and is not comfortable for people to use. The work required to improve the room has been included in the plans for the continued refurbishment of the home. The procedure for storing dangerous chemicals had not been followed and residents` were placed at risk.

The service must at all times employ a suitably qualified and experienced manager who is registered with the Commission.for Social Care Inspection. 2. A maximum of forty (OP40) older people may be accommodated, Eight (8) of whom may have dementia (DE/E). 3. The staffing levels for the home must meet the Residential Forum Care Staffing Formula for Older Adults by 1st April 2004. 4. When single rooms of less than 12 sqm usable floor space become available they must not be used to accommodate wheelchair users, and where existing wheelchair users are in bedrooms of less than 12 sqm they must be given the opportunity to move to a larger room when one becomes available. Date of last inspection 16 September 2004 Brief Description of the Service: Elizabeth Welsh House is a care home registered with the Commission for Social Care Inspection to provide personal care and accommodation for up to forty residents. The home is owned and carried on by Cumbria Care which is a Cumbria County Council business unit. Mrs Evelyn Nelson is employed as the registered manager of the home. The home is located in a residential area on the outskirts of Carlisle. The home is close to local shops and services and on transport routes into the city centre. The property consists of a purpose built two storey building set in its own grounds. Accommodation is provided as forty single bedrooms, four of which have ensuite facilities. There are accessible toilet and bathing facilities close to all the accommodation used by residents. Shared space is provided in the form of three sitting/dining rooms, a separate smoking room and a large, comfortable entrance hall. The layout of the home meets the needs of the residents living there. The home provides pleasant outdoor areas with seating for residents. The inspection was unannounced carried out on 27th June 2005. The inspection focussed on how well the home meets the needs of the people living there. This was assessed by speaking to residents, visitors, care staff and the manager, observing activity in the home and reading a sample of the records which care homes are required to hold. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4, 5 and 6 Admissions to the home are well managed and residents know their needs will be met in Elizabeth Welsh House. Residents admitted for intermediate care receive specialised support to maintain their independence and to enable them to return to their homes. EVIDENCE: Residents’ needs are assessed, known to the staff who support them and are met by the services provided in the home. Residents are invited to visit the home before deciding to move there so that they can assess whether the services provided are suitable for them. Residents receiving intermediate care are admitted to the home for short periods and receive intensive support from a team of health care professionals to enable them to retain their independence and return to their homes. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Residents’ health and personal care needs are identified, clearly recorded and are met by the services provided in Elizabeth Welsh House. Residents are treated with respect and their privacy and dignity are protected. EVIDENCE: Residents’ health and personal care needs are identified in their individual plan of care and care staff provide the support that individuals require. Residents benefit from receiving health care from local and specialist health services. Medication is handled safely and residents are able to maintain responsibility for their own medication. Care staff treat residents with respect and take appropriate actions to protect their privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Residents make choices about their lives and the decisions they make are respected. Meals are varied and provide an enjoyable and nutritious diet. EVIDENCE: Residents choose how to spend their time in the home and whether to take part in the activities provided. Residents receive visitors as they wish and choose where to see the people who visit them. The decisions residents make are respected by care staff in the home. Residents have a choice of meal and are provided with snacks and drinks as they want them. Residents said the meals in the home are “excellent”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Residents know how to complain and that their complaints will be looked into and appropriate actions taken to resolve them. Residents are protected from abuse by staff awareness and procedures in the home. EVIDENCE: Residents are aware of how they can make complaints about the care provided. Residents said they have “no complaints at all” about the services provided and said, “Evelyn would look into anything if you ask”. Care staff are aware of the procedures to follow to protect residents’ safety and welfare. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22 and 26 Residents live in safe and comfortable accommodation which is suitable to meet their needs. Renovation of the home is ongoing and will, when completed, provide a pleasant and well-maintained environment for residents to live in. EVIDENCE: Elizabeth Welsh House has a programme for maintenance and refurbishment which was ongoing at the time of this inspection. Two lounges on the ground floor have been renovated to form one large, pleasant room which residents said is “lovely”. The home has attractive outdoor areas with seating for residents which they enjoy using during good weather. The home has a range of adaptations and equipment to maintain residents’ independence including a passenger lift to the first floor accommodation, grab rails, a call bell system and specialised toilet and bathing facilities. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 Residents benefit from receiving support from care staff who are competent, well trained and recruited using safe procedures. EVIDENCE: Care staff are knowledgeable about the needs of the residents living in the home and provide a high standard of care. Care staff have received training to give them the skills to meet the needs of residents and to carry out their duties safely. The home uses good recruitment procedures which ensure appropriate checks are carried out on new employees to safeguard residents in the home. Residents made many positive comments about the staff working in Elizabeth Welsh House and the standard of care provided. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 35, 36, 37 and 38 The home is well managed and residents know their views are listened to. Care staff receive appropriate supervision which supports them to provide a high standard of care. Although procedures are in place to protect residents, a dangerous chemical was not stored safely and residents were placed at risk. EVIDENCE: Procedures are in place to gather the views of residents about the services provided and residents are confident that their views are listened to. Care staff receive regular formal and informal supervision which supports them to continue to provide a high standard of care. The home has a good procedure for handling money held on behalf of residents which protects their financial interests. The procedure for storing dangerous chemicals had not been followed and residents were placed at risk. The risk to residents was removed before the end of the inspection.
