Conditions of registration: 1. Two named individuals who are under the age category are allowed to reside in the home. 24th May 2005 Date of last inspection Brief Description of the Service: Elton Hall Care Home is a 76 bedded home for 43 older people and 33 older people with dementia. Elton Hall Care Home is a modernised stately home, set in its own extensive grounds, bordered by woodland and open countryside. The home is less than two miles from Stockton-on-Tees. The home retains many of its original features. There are large lounges and an oak panelled dining room. The building is centrally heated throughout. Access to the first floor is either by a five-person lift or one of the staircases. The home has a south facing sun terrace that overlooks the rose gardens. Elton Hall Care Home has both single and shared bedrooms, all of which have an en-suite with a toilet and washbasin. All bedrooms, bathrooms and toilets are equipped with a call alarm system. This is an overview of what the inspector found during the inspection. The inspection took place over five and three-quarter hours, with the inspector looking around the building, examining a number of records, speaking to six residents, two relatives, the acting manager, the regional manager, two members of the Care staff the maintenance man and a cook. This inspection looked at those key standards, which were not examined at the last inspection in May 2005. Any issues identified as requiring action are to be found at the back of this report. What the service does well: What has improved since the last inspection?
Elton hall has developed an improvement and refurbishing plan, which is currently in progress. There are also more regular staff and residents meetings and staff supervision all of which helps to focus on the delivery of high quality care to residents. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 & 15. Residents are encouraged to exercise choice and control over their lives. A varied and balanced choice of menu is provided in a pleasant dining room. EVIDENCE: Observation of several of the resident’s bedrooms confirmed that personal possessions and individualised rooms are encouraged. Residents are offered a key to their room on admission and for those residents with a level of dementia relatives are asked for their views. The Home does not manage the financial affairs of any residents but does hold money for the safe keeping for forty-two residents. Residents spoken with said that they exercise choice and control over their lives. Elton Hall Care Home DS0000059240.V267632.R01.S.doc Version 5.1 Page 11 Lunchtime was spent with four residents who normally sit together in the main dining room, which is well decorated with wooden panelling and a pleasant ambience. Lunch was either fish and chips or salad followed by syrup sponge and custard or ice cream. The residents said that the choice and quality of food was good and that at breakfast there was a choice of a full breakfast or a range of alternatives. Choice was also exercised on the upstairs unit for people with dementia where four people had a soft meal; one person had a salad with the remainder having fish and chips. The residents said that they discussed “food “ at the last residents meeting. The manager said that all residents are weighed on admission and that they use the “MUST” tool (malnutrition Universal Screening Tool). The local dietician is currently visiting one gentleman and two residents are having their diet fortified. Residents’ likes and dislikes were seen to be written on a sheet of paper pinned up in the kitchen. The use of large ketchup dispensers and plastic mugs detracted from an up to date acceptable image. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. Residents and relatives are confident that their views and concerns are listened to and acted upon by staff and managers, however the complaints policy and procedure must be strengthened to include information to inform residents/relatives of their right to complain to commissioning agencies such as social Services and Primary Care Trusts. EVIDENCE: The service user guide containing the complaints information is in every bedroom and all residents and relatives spoken with said that they were confident in staff and managers in dealing with any issues of concern that they may have. The information does not clearly state that residents/relatives have the right to complain to any commissioning authorities such as the Primary Care Trust or Social services. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26. Elton Hall is clean, pleasant and hygienic. EVIDENCE: Control of infection policies and procedures are in place together with domestic cleaning schedules. The manager said that bacterial dispensers were being piloted in three of the company’s homes. There were no unpleasant odours experienced throughout the home during the inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 & 38. Procedures are in place for the safe keeping of residents’ money held at Elton Hall. Residents and staff feel safe at Elton Hall and their health, safety and welfare is protected and promoted; however the home must ensure that all health and safety checks comply with timescales. EVIDENCE: Procedures and practices for the safe keeping of money were found to be appropriate. Money is held for forty-two residents; records and cash was checked for two residents and found to be correct. Appropriate systems are in place to help ensure the health and safety of residents including regular checks and maintenance. Various records were checked including gas safety, accident book, legionella check and regulation 26 recorded visits.

Yes STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP19 Regulation 23 Requirement Some of the exterior woodwork, with paint peeling, must be redecorated. The stained hall carpet must have the stains removed or the carpet replaced. The doors to a lounge and 2 WCs had peeling paintwork and the WCs had stained walls and ceiling, they must be redecorated. 2 en-suites had stained ceilings and 1 en-suite had grubby walls, they must be redecorated. The walls of a hallway and 2 bedrooms, damaged due to being scuffed by wheelchairs, must be redecorated. Note. The managers stated that the issues detailed above which are outstanding from the previous inspection will be dealt with as part of a three phase improvement programme currently in progress and to be completed by the summer 2006. A clear, simple, accessible complaints procedure must be given to each resident and displayed on notice boards around the home. It must inform residents/relatives of their right to complain to commissioning agencies of Social Services and Primary Care Trusts. 30/06/06 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. No. 1. 2. Refer to Standard OP28 OP15 Good Practice Recommendations By 2005, there should be at least of 50 staff trained to a minimum of NVQ Level 2. The home should review the appropriateness of items used at the dining table.
