Conditions of registration: Date of last inspection 7th July 2005 Brief Description of the Service: Farmborough Court is a large modern, 2-storey purpose built care home, which provides a base for a range of intermediate care services for older people who need convalescence or a rehabilitative stay. The Social Services Department is registered as the Provider of the personal care and accommodation provided at Farmborough Court Care Centre. The intermediate service is provided in partnership with Sunderlands Teaching Primary Care Trust, the Mental Health NHS Trust and the City Hospitals NHS Trust. There are 4 distinct units within the building that provide up to 54 places for older people, some of whom may also have mental health and physical needs. Each unit contains up to 14 bedrooms, a lounge, a dining room, a bathroom and a shower room. There is wide, level access into the building and wide corridors throughout the building. There are 2 passenger lifts to serve the first floor. All areas of the building are accessible by people with mobility needs. There is a driveway and car park to the front of the building. It is proposed that paved area to the rear of the building be made accessible as a patio/sitting area for the people who use this service. Farmborough Court is sited in a housing estate on the northern boundaries of the City of Sunderland. It is very close to a small number of local shops, club, church and local bus routes. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over part of one day in February 2006. At the time of this visit, the service had been dealing with an infectious outbreak that had affected several service users and staff. As a result no parts of the service users accommodation within building could be inspected, and discussions could not be held with service users or staff. Instead time was spent discussing the progress of the service with the Manager and examining some records. Over the 2 inspection visits this year, Farmborough Court has been assessed against all except two of the key standards, the vast majority of which are met. Those areas that could not be monitored for progress during this visit will be looked at as part of the next inspection. These area included care plans and teatime meals. There have been no complaints received about this service since the last inspection. What the service does well: What has improved since the last inspection?

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 & 6. Prospective service users have some written information about the service before staying here. Service users that are assessed and referred solely for intermediate care are helped to maximise their independence and return home. EVIDENCE: There is information for prospective service users about the different intermediate services at Farmbororugh Court. The information is brief but clear, and explains the type of service they will receive at Farmborough Court, how long they will probably stay there, and what happens to help them to return home. However the information does not describe where the building is or what the accommodation is like. Some people have said they were anxious before coming here and they have previously raised this point at group meetings, and at inspections. By the nature of this short-term service, people do not have the opportunity to visit Farmborough Court before their stay.
Farmborough Court DS0000040890.V256258.R01.S.doc Version 5.0 Page 9 The Manager stated at the last inspection that the Centre is to produce a photographic brochure so that prospective service users can see the style of accommodation and pictures of the type of therapies that they may receive. However it was stated that this brochure has been delayed due to other priorities. Farmborough Court Care Centre is the main provision base for a range of intermediate care services, including rehabilitation. This service is for older people who are recovering from illness, either from hospital or from home, who no longer need medical care but need support to regain daily living skills so that they can return home. This service involves the Community Rehabilitation Therapy Team, which includes Occupational Therapists, Physiotherapists and Re-ablement Assistants. The period of stay for this service is usually between 2–6 weeks. One unit provides 10 places for older people with mental heath needs who need rehabilitative support with their physical and/or mental health needs in order to return to live in the community as independently as possible. The period of stay for this service is usually 1–6 weeks. This service also involves RMNs (Registered Mental Nurses). Farmborough Court also provides convalescence care for older people who are medically fit to be discharged from hospital but are not quite ready to return home. This service allows people a stay of up to 2 weeks whilst further support can be put into place for their return home. Farmborough Court can also provide urgent accommodation for people receiving a service from the 24/7 Rapid Response Team. This service allows accommodation for older people who need short-term nursing and/or social care during an illness or following an accident. Farmborough Court Intermediate Services work alongside the Rapid Response Team and, where appropriate, will provide short-term accommodation (up to 48 hours) in order to prevent an unnecessary admission to hospital. All the elements of this service are free of charge to service users. The criteria for receiving such services is that the person must be over 65 years old, a resident of Sunderland and be assessed by the Intermediate Services Team as being potentially able to achieve rehabilitation. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 & 9. Service users have an individual plan of care, which they keep in their own rooms. Service users health care needs are fully met during their stay. Service users are supported to be responsible for their own medication, where appropriate, and are protected by the homes medication policies. EVIDENCE: At the last inspection the care plan format had been reviewed and was to be changed to improve the review of care plans during a service users stay. Care plans could not be viewed during this inspection as service users keep them in their own rooms, which were not accessible at this time. The people staying here get excellent health care screening by all the relevant health care professionals. There are Occupational Therapy, Physiotherapists and Registered Mental Nurses on site. There is a weekly GP visit and on-call service to anyone staying at the centre, and a Nurse Practitioner on-site. Rapid Response nurses, who are based at Farmborough Centre, can also provide advice as and when required. The service promotes continued independence and service users are assessed on admission for their ability to manage their own medication. This might

There is a standardised risk assessment and agreement form, which includes the service users in the decision. The situation is reviewed during their stay in case of changing abilities or needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): None of the above standard could be assessed during this visit. However these standards have been assessed over the past 2 inspections and will be examined again at the next inspection. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. Service users and their representatives have information about how to make a complaint, and these are taken seriously and acted upon. EVIDENCE: All the people who stay here have clear information about the complaints procedure in the Service Users Guide, which they keep in their own bedrooms for easy access. The complaints procedure is also available on cassette tape for people with a visual impairment or other reading difficulties. In this way all service users would know how to make a complaint about the service at Farmborough Court. There have been around 6 informal comments and complaints over the past year. These are recorded, acted upon and retained in service users own files. In this way it is evident that the service takes all complaints and comments seriously. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): None of the above standard could be assessed during this visit. However the key standards were assessed at the last inspection and will be examined again at the next inspection. EVIDENCE: There have been no changes to the premises since the last inspection. Farmborough Court was originally a purpose-built care home, and the building is suitable for its stated purpose. Level access is provided into the front of the building and across the two floors. Two passenger lifts enable access between the ground and first floors. The centre has on-site maintenance staff to attend to repairs, safety and maintenance checks, and this means that the building is well maintained for the people who stay here. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 & 30. Service users are protected and supported by the homes recruitment and selection procedures. Staff receive training to do their jobs. EVIDENCE: The Social Services Department follows a robust recruitment and selection process when appointing new staff. This includes application, shortlisting, interview, CRB disclosures, health declaration, and references. The Staffing Department holds personnel files. Individual staff files held confidentially at the service includes copies of the information required by the Manager. In this way, the Manager is assured that the staff providing care for service users are suitable to do so. There are very good training opportunities for staff. The Centre uses a number of training agencies including the Social Services Department, TCPT, Care Alliance, and local colleges. The Centre also takes advantage of the specialist knowledge of the professionals based on site, and the RMNs and therapy staff have provided some in-house training for staff. At this time the centre employs 52 re-ablement staff and 6 re-ablement officers. Of these, 19 have completed NVQ level 3 in Health & Social Care and Promoting Independence, and a further 6 staff are about to start this training. It is proposed that there will be a rolling programme of this training for all reablement staff. In this way the centre aims to ensure that all staff will have a professional qualification that demonstrates they are equipped to do their jobs.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 & 38. The service is run in the best interests of the service users, and their views are positively used to review the service. Generally, the health, safety and welfare of service users, and staff, are promoted and protected. EVIDENCE: There is a formal quality assurance system in place for Farmborough Court Care Centre. This includes a quarterly review of admissions and successful discharge. It also includes monthly monitoring visits from Social Services Department representatives; monthly health & safety audits; bi-weekly audits of care plan and assessment records; weekly kitchen audit; and any complaints and comments by service users and their relatives. The views of service users are sought via questionnaires following their stay at the centre, and by a 6 monthly follow-up questionnaire. There is an excellent response rate (about 80 ) to the service users questionnaires which means

Some former service users attend the Operational Group monthly management meetings. Also 2 service users are to develop a Service User Focus Group to represent service users interests and views at the quarterly reviews. In this way service users views are actively used to review and develop the service. Staff receive training in statutory health & safety matters including Moving & Assisting, Fire Safety, First Aid and Food Hygiene. There are health & safety audits of the building by maintenance staff. The premises could not be examined due to the infectious outbreak in the home. However the Manager did comment that there is not a call alarm point in the dining room in the Windsor Unit. This omission had only been noticed during a choking incident of a service user last year. This was clearly an oversight when the call alarm system was fitted. The Centre reported this matter to City Building Services for attention in September last year, but this equipment is still awaited.
