Conditions of registration: None Date of last inspection 8 March 2005 Brief Description of the Service: Farmborough Court is a large modern, 2-storey purpose built care home, which provides a base for a range of intermediate care services for older people who need convalescence or a rehabilitative stay. The Social Services Department iss registered as the Provider of the personal care and accommodation provided at Farmborough Court Care Centre. The intermediate service is provided in partnership with Sunderland’s Teaching Primary Care Trust, the Mental Health NHS Trust and the City Hospitals NHS Trust. There are 4 distinct units within the building that provide up to 54 places for older people, some of whom may also have mental health and physical needs. Each unit contains up to 14 bedrooms, a lounge, a dining room, a bathroom and a shower room. There is wide, level access into the building and wide corridors throughout the building. There are 2 passenger lifts to serve the first floor. There is a driveway and car park to the front of the building. It is proposed that paved area to the rear of the building be made accessible as a patio/sitting area for the people who use this service. Farmborough Court is sited in a housing estate on the northern boundaries of the City of Sunderland. It is very close to a small number of local shops, club, church and local bus routes. This is an overview of what the inspector found during the inspection. This unannounced inspection to Farmborough Court Care Centre took place over one day in July 2005. Around half of the time was spent talking with service users about their views of the service they receive and joining some service users for a tea-time meal. Around half the time was spent with the Manager discussing the progress of the intermediate services provided here, and looking at care records. A sample of bedrooms, bathrooms, lounges and dining rooms was examined. There have been no complaints received about this service since the last inspection. What the service does well: What has improved since the last inspection?
There are now 17 out of the 49 reablement staff who are training towards a suitable qualification in health & social care. This will help them to carry out

Since the last inspection nearly all the 54 places at the Centre have been filled. In this way the Centre has provided even more people with the chance to get fit enough to go home, and has prevented more people having to go into hospital or having to go into long term residential care. There are new care records being introduced. These will help staff to know how to support people to get fit and independent again, and for staff and service users to be able to track their progress. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 and 6. Prospective service users are given brief but clear information about the service before staying here. Service users are assessed and referred solely for intermediate care and are helped to maximise their independence and return home. EVIDENCE: There is information for prospective service users about the different intermediate services at Farmborough Court. The information is brief but clear, and explains the type of service they will receive at Farmborough Court, how long they will probably stay there, and what happens to help them to return home. However the information does not describe the where the building is or the accommodation, and some service users have said that they were anxious and did not know what to expect before coming to stay here. By the nature of this short-term service, people do not have the opportunity to visit Farmborough Court before their stay, so they dont know what it looks like. The Manager stated that this point had also been raised at a Service Users Meeting, and that the Centre is to produce a photographic brochure so that

Service users said that that sort of information will help to reduce peoples anxieties about coming to stay here. Farmborough Court Care Centre is the main provision base for a range of intermediate care services, including rehabilitation. This service is for older people who are recovering from illness, either from hospital or from home, who no longer need medical care but need support to regain daily living skills so that they can return home. This service involves the Community Rehabilitation Therapy Team, which includes Occupational Therapists, Physiotherapists and Re-ablement Assistants. The period of stay for this service is usually between 2–6 weeks. One unit provides 10 places for older people with mental heath needs who need rehabilitative support with their physical and/or mental health needs in order to return to live in the community as independently as possible. The period of stay for this service is usually 1–6 weeks. This service also involves RMNs (Registered Mental Nurses). Farmborough Court also provides convalescence care for older people who are medically fit to be discharged from hospital but are not quite ready to return home. This service allows people a stay of up to 2 weeks whilst further support can be put into place for their return home. Farmborough Court can also provide urgent accommodation for people receiving a service from the 24/7 Rapid Response Team. This service allows short-term nursing and/or social care for older people during an illness or following an accident. Farmborough Court Intermediate Services work alongside the Rapid Response Team and, where appropriate, will provide shortterm accommodation (up to 48 hours) in order to prevent an unnecessary admission to hospital. All the elements of this service are free of charge to service users. The criteria for receiving such services is that the person must be over 65 years old, a resident of Sunderland and be assessed by the Intermediate Services Team as being potentially able to achieve rehabilitation. In this way, service users are referred only for intermediate care and are supported to return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 10. The service users individual intermediate care needs are set out in a plan of care. Service users are treated with respect and their right to privacy is upheld. EVIDENCE: A new, improved care planning system is being introduced to guide staff in the exact goals/needs of the people who stay here, and how these can be met. Care goals will be evaluated, so that the service users progress can be tracked. By the nature of this short term service, care plans are brief and focussed upon rehabilitative goals in order for the service user to return home. Care files are kept in service users own bedrooms so that they have access to them at any time and have a sense of ownership of their own records. In the sample of care files examined, service users had signed their agreement to their care plans to demonstrate their involvement in drawing up their goals. Discussions with many of the people staying here at this time confirmed that they are treated with respect by staff, and that staff are very helpful. Over the course of this visit staff were observed to be sensitive and courteous in their support of the people staying here.

In this way service users have the opportunity to keep their bedroom door locked if they wish. It was clear that most people had chosen to keep their door open at all times to allow them easy access, but the rooms also have 2 lockable facilities in which they can securely store personal possessions and medication. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 15. Service users are able to continue their routines of daily living whilst being supported to maximise their independent living skills. Service users can choose from a range of nutritious meals and dining is a pleasant social occasion. More attention is needed to ensure a consistent quality of evening meals. EVIDENCE: The service respects the social, cultural and religious diversity of the people who stay here. The main aim of this short-term service is to get people back into their own homes and communities. There are daily activities and therapies provided within the service. These are based on peoples mobility and daily living skills in order that they can maximise their independent living skills for when they return to their own homes. During this visit staff prompted and encouraged individual service users with their mobility exercises. There were also group physical exercises sessions, and quizzes designed to promote service users memory and recollection skills. One service user stated, staff are helping me to get back to how I was so I can go home. “Another person commented, there are lots of exercises, and staff encourage us to get around and do things for ourselves”. Some service users also described being involved in washing up, clearing tables, and “going home with a therapist to cook lunch on my own cooker”.

Meals are provided from a centrally sited kitchen, and transported to the 4 dining rooms by hotlock trolley so that service users can be served their choice of meals to individual preference. Only one criticism of the service was received from several service users, and this related to the quality of the tea-time meal. Service users stated that this meal was often cold, tasteless and unseasoned. The service users commented that this was not the case with other meals, which they felt were of a better quality. The Inspector joined service users in one unit for the tea-time meal and found that their comments were founded. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18. The service has robust procedures and staff training to safeguard service users from abuse. EVIDENCE: As with all care services for adults in the City of Sunderland, Farmborough Court endorses the MAPPVA (Multi-Agency Panel for the Protection of Vulnerable Adults) policy and procedures. These are robust procedures for dealing with suspected abuse. All staff have training in MAPPVA procedures as part of their Induction training and then at least every 3 years. All staff will also have MAPPVA training as part of their NVQ training. In this way all staff would know what to do in the event of suspected abuse, and how to safeguard service users. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 21 & 26. Service users stay in a safe, well-maintained environment. There are sufficient toilet and bathing facilities for the people who stay here. The premises are clean, pleasant and hygienic. EVIDENCE: Farmborough Court was originally a purpose-built care home, and the building is suitable for its stated purpose. Level access is provided into the front of the building and across the two floors. Two passenger lifts enable access between the ground and first floors. The centre has on-site maintenance staff to attend to repairs, safety and maintenance checks, and this means that the building is well maintained for the people who stay here. Since the last visit 2 unused communal WCs have been converted for hoist storage. This means that the equipment can be stored out of the way and there is more room for service users when using the bathrooms. Work is

All of the bedrooms have en-suite toilets, so service users have private facilities when using their rooms. There are also communal toilets, shower rooms and bathrooms around each of the 4 units, so service users dont have far to walk from lounges to use these facilities. There were some personal continence equipment and labelled jugs in one shower room and this does not support the dignity of the people who stay here. There were used bars of soap in another shower room, which does not protect service users from potential cross infection. There were water temperature records in all shower rooms and bathrooms that were either out of date, or did not identify the unit or the facility, or recorded a temperature that is too low. In this way it was not possible to ascertain whether the temperature of water was correct for use by the people who stay here. Farmborough Court provides dedicated laundry staff and individual laundry bags to manage the laundry service for the people who stay here. The service users had many positive comments about the laundry service during this visit. One person said its tip top, and there are fresh towels frequently. Service users also said that the building is kept spotlessly clean. There are dedicated domestic staff who manage the cleaning programmes within the building and all areas of the premises that were examined were clean. All staff have had training in Infection Control. There are protocols in place for staff to support people with infections so that they receive a full care service without any risk of cross-infection to others. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 & 28. Service users needs are appropriately met by the numbers and skill mix of staff. Staff are being trained to demonstrate their competency to do their jobs. EVIDENCE: At the time of this visit there were 51 people staying at the service. During the day there were 12 Reablement Assistants (3 to each unit) and 2 Reablement Officers providing the care support service to people staying here during the day. There were also Occupational Therapists, Physiotherapists, Therapy Assistants, Registered Metal Nurses, and Rapid Response nurses in the service throughout the day to promote the therapy, rehabilitation and physical wellbeing of the service users during their stay. In the evenings there are 1 Officer and 8 Reablement Assistants (2 staff to each unit), as service users more time in the privacy of their rooms. During the night there are 4 night staff, one of whom is a Reablement Officer who leads the team. Discussions with service users confirmed that staffing levels are sufficient to meet their needs and to provide daytime therapy and support. Service users also felt that there were sufficient auxiliary staff to provide catering, housekeeping, and maintenance services. At this time, 17 of the 49 reablement staff are training towards

It is expected that they will complete this at the end of the year, and that around another 8 staff will start this course. It is intended that eventually all staff will be qualified to this level. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 35 & 38. Service users benefit from a well-organised service. They are responsible for managing their own finances, and are provided with lockable facilities to manage this. The health & safety of service users is promoted and safeguarded by protocols and practices within the service. EVIDENCE: The Registered Manager is a qualified nurse and has a number of suitable management and care qualifications. She has many years experience of managing nursing and social care services. She is responsible for the day to day running of the service within Farmborough Court and is assisted by a Deputy Manager. There is also a Community Rehabilitation Service Manager who is responsible for the management of the Occupational Therapy and Physiotherapy services. All service users’ finances are managed by themselves or their representative. The intermediate service is by nature a short-term rehabilitative arrangement and Farmborough Court does not take any responsibility for service users monies. All bedrooms have lockable facilities and all bedroom doors are lockable, so service users are able to maintain the security of their personal possessions whilst staying here. Staff receive training in statutory health & safety matters including Moving & Assisting, Fire Safety, First Aid and Food Hygiene. The maintenance staff carries out a workplace safety check health every month. At this time all maintenance and servicing checks of equipment are carried out. The issues regarding records of hot water temperatures must be addressed, as outlined in the body of this report.
