This inspection was carried out on 11th November 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The residents appeared to be well cared for and those spoken to said that they were happy living at Fernbank. The atmosphere in the home was friendly and staff were observed talking to the residents in a friendly manner. The home was found to be clean and well maintained.

What has improved since the last inspection?

The manager said that the assistant manager has now been inducted in the policies and procedures about abuse. Good progress in care staff doing NVQ (National Vocational Qualification) continues.

What the care home could do better:

A copy of the document published by the Department of Health called `No Secrets` which gives information about the types of abuse and how to deal with them was not available at the home. This was recommended during the previous inspection. The manager needs to complete her Registered Manager`s Award by the end of the year, as this is a recommendation made by CSCI. The inspector found that a portable radiator used in the entrance porch was very hot to the touch and advised the manager to replace it with one, which is safer.

The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection The service is registered to accommodate a maximum of 18 service users in the category OP (older people 65 and over) Date of last inspection Brief Description of the Service: Fernbank is situated on Heysham Road in Heysham and is registered as a home for elderly people of both sexes who are 65 years old and over. The home is registered to take a maximum of 18 residents. Accommodation is provided in single bedrooms. None of the bedrooms have an en suite facility. The home has two floors and a stair lift is available to access the first floor. Residents can use the lift independently if they wish. The home is adequately fitted and furnished. A portable television is provided for those who want one in their bedrooms. A small garden area and a car park are available at the rear of the home. The home is close to all the amenities but still too far for most of the residents to access on their own because of their frailties. This statutory inspection was the second of two to be carried out this year. The inspection took place on 11th November 2005 and was an unannounced one. It lasted for 4 hours. The inspection was carried out against the National Minimum Standards for Older People. The inspection was carried out in a relaxed atmosphere with the full cooperation of the person in charge, the other staff and all the residents. The inspection took the form of looking at some of the records, talking to the residents and the staff and checking the building. There were 13 residents at the home and they said that they were well cared for and that all the staff were kind and helpful. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: The home does not provide intermediate care. The other core standard was assessed during the previous inspection and was met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7&9 Arrangements for assessing and meeting residents’ needs are good. Medications procedures are rigorous. Residents have defined care plans and their medications are dispensed in a safe manner. EVIDENCE: All the residents have a personal file containing their assessments and care plans. The case files of two residents were examined and they showed that they have been assessed properly and that care provided is according to assessed needs. The records also show that care provided is recorded and that regular reviews take place. The residents spoken to said that they receive good care and that the staff are kind. The staff the inspector spoke to were fully aware of the needs of the residents and said that they were involved in providing care according to individual assessments. Fernbank Retirement Home DS0000009699.V262353.R01.S.doc Version 5.0 Page 10 Comment cards completed by the residents during the inspection were complimentary about they care they receive and about staff attitudes. The inspector observed the care staff dispensing medications to the residents. This was done according to procedure. The records of medications were examined and they were found to be correct. An audit trail of the medications of one resident was done and this was found to be accurate. The manager was aware that medications following the death of a resident should be kept for a week in the event that a post mortem is carried out. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 & 15 Freedom of choice is an important part of the home’s policy. Nutrition needs are met satisfactorily. Residents have choices in their daily activity and are provided with good food. EVIDENCE: The residents said that they are able to do what they want within their limitations. They said that the staff would help them when they need something. The residents were observed moving around the home freely and asking assistance from the staff when they wanted to go to the bathroom. The manager said that the policy of the home is to encourage the resident to do as much for themselves as they want. She said that staff are aware when to encourage residents without forcing them to do something they cant. The residents said that the food served is good and that they get plenty to eat. A choice of meals is provided at meal times. On the day of the inspection, the lunch choices were lamb chops or fish fingers. The records of meals served were examined and they clearly show that a wide range of foods is available to the residents. The residents said that they could have a snack during the night if they are feeling hungry. They also said that they could have a drink when they want. After observing the staff serving lunch, the inspector has recommended that residents who are blind or have poor eyesight be told what food is placed in front of them. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The policies and procedures to deal with complaints are good. Residents live in a safe environment and are able to speak their minds without fear of reprisals. EVIDENCE: The home has a detailed complaint procedure which explains what people should do if they have a complaint. It is included in the Service User Guide and states that in the first instance, people should complain to a member of staff unless the complaint is about staff. The next step is to speak to the manager or one of the owners. If this does not resolve the complaint, then the complaint can be passed to the Commission for Social care Inspection (CSCI). The home has received no complaints since the previous inspection. The residents said that if they have any complaints, they would not hesitate to speak to someone about it. They said all the staff and the manager are very approachable and easy to speak to. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home provides a well-maintained and safe environment for the residents. Residents feel safe and protected. EVIDENCE: The building is well maintained and decorated. At the time of the inspection, one of the bedrooms was having new carpet fitted before a new resident arrives. The manager said that it is the policy of the home that when bedrooms become vacant, they are decorated and carpet replaced if necessary. All the bedrooms visited were found to be clean and well decorated. The residents said that they like their rooms and can bring as much of their personal belongings as they want. The front door is kept locked and all visitors are let in by a member of staff. This is to ensure the safety of the residents and the staff. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 28 Staffing level and skills are good. Residents feel safe and well cared for. EVIDENCE: Care staff who have achieved their NVQ level 2 and above are well over the 50 recommended by CSCI (Commission for Social Care Inspection). The manager said that all new staff are given an induction-training programme when they start work at the home. The induction training includes basic practices such as Moving and Handling, Food Hygiene and First Aid. The staff the inspector spoke to said that they have attended many training courses and found them beneficial. The staff said that they have attended the following courses: Abuse Awareness Health and Safety Medications First Aid NVQ The residents said that the staff are kind and helpful. They said that they feel safe living at Fernbank and that they are not afraid to speak to the staff. There is always a friendly atmosphere in the home and residents and staff seem to get on well together. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 & 35 The management of the home is competent. Policies are in place to protect residents’ finances. Residents’ needs are fully met. EVIDENCE: The staff said that the residents have complete freedom in their daily activities. They said that residents can do what they want and that they are available to speak or advice them when they want. The residents said that they are happy living at the home and that the staff are helpful Residents are encouraged to manage their own finances. Records of monies held by the home on behalf of some residents were examined and found to be accurate.
