Conditions of registration: Date of last inspection 27th April 2005 Brief Description of the Service: The home is a large former domestic property, which has been extended and adapted for its present use. It is situated in the village of Eye close to the centre. The village is near to the city of Peterborough. The home is set in large grounds and is adjacent to the local school. There is major building work under way to extend the premises. This will provide additional ensuite bedrooms and further communal space. Arrangements are in place to ensure that service users are protected from any potential hazards whilst the building work is being completed. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 20th September 2005. The inspector met with the Manager, to assess a number of standards. Standards not assessed on this occasion will be assessed at a later inspection in the year. Service users spoken to on the day of inspection were positive about the care they were receiving. Building work, to add further bedrooms and communal space is well underway and should be completed in November 2005. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,5 Service users have access to good information, and can make an informed choice regarding the home’s services. EVIDENCE: There have been no additions or updates to either the Statement of Purpose or Service Users Guide. The Manager stated that both of these documents would be updated when the building work has been completed to make sure that all information is correct regarding the services provided. Three care files of service users were assessed. There was pre assessment information in place received from the referring social worker and previous care homes giving detailed information regarding the service user’s needs. The manager stated that the home’s assessment documents will be further developed to accommodate the anticipated increased number of clients when the building work is completed. The manager stated that she would be contacting members of Personal Medical Services team, which is offered

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 Service users receive appropriate health and personal care to meet their assessed needs. Medication is administered and recorded accurately. EVIDENCE: Three service user plans of care were seen. Care plans provide some guidance for staff in how they are to meet the assessed needs of the service users. The manager said that care plans are reviewed regularly to show any changes but it was noted during the inspection that reviews of care had occurred but they need to be recorded in more detail. Medication records were inspected and found to be completed accurately. The service user files seen showed that health care services are available to ensure that service users receive adequate health care from a variety of professionals including district nurses, continence advisor, dieticians and GPs. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 Staff provide support to ensure that service users have access to activities appropriate to their needs. EVIDENCE: A number of service users were in the lounge and were happily talking with other service users and members of the care staff. There are no restrictions on visiting and service users can entertain their visitors in either their bedroom or the communal areas of the home. The manager acknowledged that activities and community contact needs to be improved. She stated that she will be developing activities in the home and a new post for Activities Co-ordinator is currently being advertised to ensure that programmes are implemented for both individual and group activities. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18 The home has a complaints process to make sure that service users have their complaints or concerns listened to and acted upon properly. EVIDENCE: The home has a complaints procedure including agreed timescales to make sure that all complaints are fully investigated and actioned appropriately. The home has received one complaint since the last inspection, which the manager has investigated and responded to appropriately The home has a satisfactory policy in place, in line with the Local Authority policies, to make sure that service users are protected from abuse. The manager stated that she will be attending key practitioner training in POVA issues which she will then cascade to the care staff team. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,23,24,26 The environment is suitable for the needs of those living in the home. EVIDENCE: Major building works are nearing completion to the rear of the premises, which will include further bedrooms and communal facilities for service users. The current premises have been secured to ensure that service users are safeguarded from any potential hazards. It is anticipated that the building work to the extension will be completed in November 2005. The manager stated that current service users will move to the new bedrooms. A new laundry, reception area and front entrance are due to be provided. Service user rooms and communal areas seen on the day of inspection were clean and tidy and the home was free from offensive odours. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28,29,30 The home’s recruitment policy and processes makes sure that service users are protected from harm. Training is provided to make sure that care staff are competent to deliver care to the service users they support. EVIDENCE: All staff are issued with contracts of employment, detailed job descriptions and clear details of their areas of responsibility. There have not been any new staff appointments since the last inspection. The manager stated that recent training in the home has included medication management, moving and handling, fire safety, personal care, dementia, challenging behaviour and first aid. Staff confirmed that they had received recent updates in training. The PMS team (Personal Medical Services) also continue to provide advice, support and training for healthcare issues on an ongoing basis. The manager said that two members of care staff have attained NVQ level 2 qualifications and two further staff have commenced NVQ level 2 training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,37 The home is well managed and the manager provides supportive leadership and guidance to staff to ensure that service users receive a good standard of care. EVIDENCE: The manager has now successfully completed a course in nurse training which will increase her knowledge and skills. She is now working towards an NVQ level 4 in Management. She has recently updated training in dementia care and protection of vulnerable adults. Care staff spoken to during the inspection confirmed that the manager provides a clear and inclusive style of management with support from the registered provider.
