If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Field House is a registered care home to provide care and support for up to 33 places for people, mainly over 65 years of age, some of whom have dementia. The home is a large former domestic property, which has been extended and adapted for its present use. It is situated in the village of Eye close to the city of Peterborough. The village is near to the city of Peterborough. The home is set in large grounds and is adjacent to the local school. What did we do for this annual service review? We looked at information we have received from the home since our last inspection. We looked at the results of surveys we have received from five of the residents and from four of the staff and from one of the health care professionals who is involved with the home. We looked at the report of our last inspection that took place in October 2008. We looked at the Annual Quality Assurance Assessment (AQAA) that was completed by the Registered Manager. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. What has this told us about the service? In February we received a complaint, from a confidential source, and we asked the home to respond to this complaint, using the homes complaint procedure. We received a response, within the required 28-day period, although we were unable to read the response due to the format that this was emailed to us. We asked for this to be sent, via the post, as a hard copy, although we had no record of receiving this. The Manager has since responded to say that this was sent to us. In October 2009 we attended a meeting that was held under the local safeguarding procedure. The home was represented at this meeting, during which we saw the original written response to the complaint made to us, in February 2009; we found that this was satisfactory. All of the surveys that we received, from the staff, a healthcare professional and the residents, told us that the person knew about the homes complaints procedure. The AQAA told us that, within the last twelve months, the home has received two complaints, both of which were resolved within the 28-day required time period and neither of these complaints were substantiated. This tells us, and from our findings of how the home has responded to any complaint and allegations made against the home, that people can be confident that they will be listened to and action taken to any concerns they may have. We also consider that people should be safe from the risk of harm. We have received information, as required, such as notifications of expected deaths and falls. This tells us that the home is mindful of its legal responsibilities. Within the notifications we saw that there were three incidents reported to us of people who have sustained a fall: although one of these people had acquired a fractured leg the other two people had sustained no serious injuries. Given the nature and conditions of the people that Field House provide care to, this is a small number of falls occurring at the home. This tells us that the people should be generally safe from the risk of falls. At our last inspection of the home, of 21.10.08, we noted that the quality assurance Annual Service Review Page 4 of 7 systems of the home were very good. We were told, at the safeguarding meeting held in October 2009, that the home continues to maintain this standard: relatives of residents are routinely seen to ask them if they have any concerns about the home. As part of the homes quality assurance assessment we received the AQAA when we asked for it. We found the standard of the AQAA to be satisfactory, although there was some confusion with some of the standards: for example within Standards 1-6 there was evidence provided which was relevant to Standards 8 and 9. Notwithstanding such confusion the AQAA provided details of how the home is performing, such as what it does well and areas that the home has improved upon, within the last twelve months. This required information also told us areas where the home intends to improve upon, within the next twelve months. We received five surveys from residents, the majority of these had been completed, on behalf of the residents. All of these surveys indicated that the people were satisfied with the standard of care and social activities, such as the meals provided and with the cleanliness of the home. We received comments such as All management & staff (are) friendly & caring and The home has very caring and knowledgeable staff. They are very kind, friendly and helpful. We also received the following comments, They are very caring about residents wishes and happiness, and there is a family feel about the home. Less positive comments received were about the standard of the laundry as ..it needs to be organised better as sometimes clothes go missing or you get other peoples clothes. There were, however, more positive comments than negative ones, from these residents surveys. We received a survey from a health care professional, who also made positive comments about the home, saying,From my viewpoint and role...both staff and management have been excellent. At our inspection of October 2008 we discussed an ongoing concern: this was that some of the staff,who had English as not their first language, were sometimes difficult to understand. This issue was also discussed at the safeguarding meeting held in October 2009. The representataives of the home told us that that the staff are paid to attend college to improve their command of the English language. Whilst we regarded this as a positive action to take to improve this area of staffing, we reminded the homes representatives that the recruitment stage should be where applicants command of the English language is assessed. Field House told us that applicants are spoken to before the recruitment process progresses to a point of offering the person a position to work at the home. We received four surveys from the staff: all of these surveys said that the person was well supported by the home and were satisfied with their induction training and ongoing training. The AQAA told us that, of the twenty-one current staff, sixteen of these have attended a recognised induction training and all of these sixteen staff have a National Vocational Qualification, level 2 or equivalent, in care i.e. just over 76 . The AQAA also told us that one member of the catering staff and eleven of the care staff have attended training in safe food handling and fifteen of the care staff have attended training in the prevention and control of infection. This tells us that people should be in safe hands. We have looked the report of the last inspection of the home, in which there was a requirement made with regards to the storage of medication. Annual Service Review Page 5 of 7 to be taken, to meet this requirement, was by the 28th February 2009. The AQAA told us that the way medication is stored has improved, because there has been an Installation of an extractor fan in the medication storage cupboard and that there is also a new fridge for the storage of specimens, different to that for the storage of medication. People should be safe as the AQAA told us safety and service checks are in date for fire detection and fire fighting equipment, portable (electrical) equipment and hoists. What are we going to do as a result of this annual service review? We will continue with our inspection plan and we may carry out the next inspection of the home by 20th October 2010. We will continue to monitor information that we receive about the home.
