This inspection was carried out on 27th April 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Field House provides a caring, respectful and homely environment for service users. Service users are encouraged to personalise their rooms to meet their individual choices and preferences. Visitors are welcomed to the home at all times. Cultural preferences are observed at all times.

What has improved since the last inspection?

At the last inspection no new requirements were made and the home continues operating in a manner that meets the assessed needs of the service users.

What the care home could do better:

The care planning processes are being developed to provide more detailed information and the manager is actively researching examples of good practice with the assistance of local healthcare specialists. The premises will benefit from the extension currently being built to the rear of the premises. This will provide improved communal space for service users, and better kitchen and laundry facilities. There will also be new office space, which will provide improved storage and work space for the manager and care staff.

It is situated in the village of Eye close to the centre. The village is near to the city of Peterborough. The home is set in large grounds and is adjacent to the local school. There is major building work under way to extend the premises. This will provide additional ensuite bedrooms and further communal space. Arrangements are in place to ensure that service users are protected from any potential hazards whilst the building work is being completed. Field House Version 1.10 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 27th April 2005.The home continues to offer service users a caring residential experience. Service users spoken to on the day of inspection were positive about the care they were receiving. Building work, to add further bedrooms and communal space is well underway and should be completed in September 2005. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 &3 Assessments prior to admission are in place to make sure that that the care needs of new service users can be met by the home EVIDENCE: Three care files of service users were assessed. There was information about the service user’s needs and how Field House could meet them provided by the referring social worker and previous care homes. There have been no additions or updates to either the Statement of Purpose or Service Users Guide. The Manager stated that both of these documents would be updated when the building work has been completed to make sure that all information is correct regarding the services provided. Field House Version 1.10 Page 8 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 &9 Care provided at the home is generally good. However the home needs to improve the current format for care plans so staff have sufficient information to satisfactorily meet service users needs. EVIDENCE: Three service user plans of care were seen. Care plans provide some guidance for staff in how they are to meet the assessed needs of the service users. The manager said that care plans are reviewed regularly to show any changes but it was noted during the inspection that dates for reviews of care need to be added. Medication records were inspected and found to be completed accurately. The service user files seen showed that health care services are available to ensure that service users receive adequate health care from a variety of professionals including district nurses, continence advisor, dieticians and GPs. The manager said that she was reviewing the care plan process and it that she has been researching new documents with advice from the Personal Medical Services team, which is offered through the Greater Peterborough Primary Care Trust. The manager said that the revised care plans would be Field House Version 1.10 Page 9 implemented to coincide with the anticipated opening of the home’s new extension in September 2005. Field House Version 1.10 Page 10 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 13 Service users enjoy a friendly and inclusive atmosphere in the home and are able to choose how they wish to live. EVIDENCE: A number of service users were in the lounge and were happily talking with other service users and members of the care staff. There are no restrictions on visiting and service users can entertain their visitors in either their bedroom or the communal areas of the home. Field House Version 1.10 Page 11 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The home has a satisfactory complaints process to make sure that service users and their representatives have their complaints or concerns listened to and acted upon properly. EVIDENCE: The home has a complaints procedure including agreed timescales to make sure that all complaints are fully investigated and actioned appropriately. The home has not received any complaints since the last inspection. The home has a satisfactory policy in place, in line with the Local Authority policies, to make sure that service users are protected from abuse. It was observed that care staff spoke to service users in a friendly and respectful manner. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 25 & 26 The environment of the home provides service users with a safe, comfortable, clean and a well-maintained place to live. EVIDENCE: Major building works are under way to the rear of the premises, which will include further bedrooms and communal facilities for service users. The current premises have been secured to ensure that service users are safeguarded from any potential hazards. It is anticipated that the building work will be completed in September 2005. Service user rooms and communal areas seen on the day of inspection were clean and tidy and the home was free from offensive odours. Field House Version 1.10 Page 13 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 & 30 The agency’s recruitment policy and processes makes sure that service users are protected. Training is provided to make sure that care staff are competent to deliver personal care to the service users they support. EVIDENCE: All staff are issued with contracts, detailed job descriptions and clear details of their areas of responsibility. One new member of staff’s file was seen and it contained appropriate information including two references and evidence of a satisfactory CRB check. The manager stated that recent training in the home has included medication management, moving and handling, fire safety and first aid. The PMS team (Personal Medical Services) also provide advice, support and training for healthcare issues on an ongoing basis. The manager said that two members of care staff have attained NVQ level 2 qualifications and two further staff have commenced NVQ level 2 training.
