Conditions of registration: 1. The room known as `Goring` situated on the first floor of the home may only be used by service users if a documented risk assessment of their mobility capabilities has been undertaken and placed on record. 13th December 2005 Date of last inspection Brief Description of the Service: Foxleigh Grove is a care home with nursing. It provides nursing care to older people who do not suffer from a diagnosis of dementia and younger people with physical disabilities. Mr Mark Aram is the General Manager and one of the Registered Individuals. There are two Care Managers, Mrs Plant and Mrs Moxon; they share the responsibilities of the Registered Manager position. In addition, they are responsible for the clinical care provided at the home. Accommodation is provided over two floors that are serviced by a passenger lift. The majority of the rooms available are for single occupancy. The property is situated in the village of Holyport on the outskirts of Maidenhead. It has extensive gardens that are much appreciated by the residents. The train station is approximately 2 miles away and a bus service runs near to the care home. This is an overview of what the inspector found during the inspection. This key report contains information collected during an unannounced site visit to Foxleigh Grove made during a weekday by a locum inspector. Information held in the Service file pre- informed the inspection together with data provided by the Registered Individual. In addition, prior to the visit, Service User views had been sought in a survey “Have your say about”. Four forms were returned to CSCI; the comments were favourable and are included in the appropriate sections of this report. A tour was made of the premises and during the visit care practice was observed; Service Users were pleased to praise the service and the staff. Relatives who spoke to the inspector echoed these views. Records were inspected and the Inspector spoke to members of the family who own and operate the service including the General Manager, the two Care Managers and as many of the on duty staff as possible. The medication policy, procedure and practice have been reviewed as required following the previous inspection. On this site visit the practice of medication storage, administration and record keeping were found to be in order. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4 and 5. Quality in this outcome area is good. Service Users and their representatives have the information that they need to choose a home that meets their needs. Their needs are assessed and they have a contract for the service that is offered. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home produces a statement of purpose that contains detail of the service that is offered. During the visit Service Users informed the inspector that they had received enough information about the home prior to admission to enable them to make a decision .The “Have your say about” survey forms also confirmed that Service Users were satisfied with the detailed information provided for them. Trial visits were offered when appropriate. Service Users’ files contained evidence of assessment of individual need and each had a contract with detail of terms and conditions. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is good. Each Service user had a plan of care that included their health, personal and social care needs. The medication policies and procedures had been reviewed following the previous inspection; requirements had been met and the storage, administration and recording of medication are now satisfactory. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Each individual had a plan of care in place. The plans were generated by the individual’s assessed needs and were found to contain detail of the actions for care staff to take in order to meet need. Plans are regularly reviewed. Service Users are aware of the care plans. Staff provide support and assistance to meet health care needs; risk assessments for nutritional need, pressure area care and falls are in place. There was evidence that staff had promoted and achieved the healing of a chronic pressure sore. Referrals are made for medical and specialist nursing need when appropriate. Controlled drugs are only administered to Service Users when they are prescribed and supplied for them. There is an accurate record of wasted

The nursing staff in the home follow all relevant legislation and guidance with regard to Controlled Drugs. A pharmacist had inspected the medication system following the last inspection; the Responsible Individual should ensure that a pharmacist regularly inspects the system. Service Users reported in the survey that staff give care in a very professional manner. Observation during the site visit confirmed that staff treat Service Users with respect and kindness. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, and 15 Quality in this outcome area is good. Service Users enjoy the lifestyle available to them in Foxleigh Grove. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Routines of daily living are flexible in the home to meet residents’ needs. In the survey it was said by a Service User that staff “try to give happiness”. Arrangements are made for activities; during the visit a group of residents were enjoying a painting session in the conservatory. The organiser was aware of the different needs of each person and gave individual attention. Visiting relatives appreciated the staff practice of creating an album of photos for each person. Continued contact with relatives and friends is encouraged; a relative regularly enjoys having lunch with her mother in the home. The lunch was well presented and served appropriately with assistance given to those who needed help. The survey reported that the food was good; one person thought that it was rather bland but recognised that this might be the preference of most residents. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. Quality in this outcome area is good. Residents have access to a complaints procedure and are protected from abuse. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There was a complaints procedure available to Service Users and their relatives. No complaints had been made to the Foxleigh Grove or to CSCI. Service users are protected by the use of a staff recruitment policy and procedure that includes checks and references to vet new employees. There was evidence of good employment practice in the staff files. Staff receive training to raise their awareness of abuse issues and the action they need to take to protect Service Users. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 and 26 Quality in this outcome area is good. Service users live in an attractive safe well-maintained and comfortable environment that has been adapted to enable them to live as independently as possible. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Service Users live in a safe well-maintained environment; on a tour of the property there was evidence of routine repair and refurbishment of the attractive individual decoration. One Service User informed the inspector that the furnishings in her room had been designed to compliment the furniture that she had brought to the home. Equipment is fitted to assist staff to mobilise disabled Service Users safely and comfortably; a special hairdressing basin is provided and other aids are in use. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Quality in this outcome area is good. Residents’ changing needs are met by staff who have been trained and have the skills to provide care. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Service Users consider that there are sufficient numbers of staff employed to meet their needs. The staff are skilled and able to demonstrate that they are able to meet the assessed needs of the Servile Users who enjoyed continuity of care because there is a low turnover of staff. Many carers have given long service in the home and are experienced and given training but are reluctant to participate in training programmes for NVQ qualifications. This makes it difficult for the Service to achieve a 50 NVQ trained workforce. There was evidence that the procedure for recruitment of new staff was followed. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 36, 37, and 38 Quality in this outcome area is good. Service Users live in a well run and managed home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Service Users are in the care of a qualified management team; the Registered Individual is in daily charge of the home and the care managers who share the Registered Manger’s role are both qualified nurses and have completed the Registered Mangers award. Service Users feel confident because the owners of the service are directly involved in the management and day to day operation of the home. Staff are regularly supervised and the home is run in accordance with health and safety regulations.
