This inspection was carried out on 8th February 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The Hawthorns offers comfortable, well-furnished and decorated accommodation to mainly local people who are assessed as needing residential care. A small staff team supports them who know in detail the needs and wishes of all the current fifteen residents. On the day of inspection there was a warm friendly and relaxed atmosphere between staff, and residents. Relatives and friends were seen to be visiting freely. Direct quotes from residents and relatives include the following; " The service is very good, we are really well looked after here. " " All the staff are good and if you need to see the manager about anything it gets sorted out." "Its very homely here and not clinical and the staff always welcome me with a cup of tea when I visit."

What has improved since the last inspection?

Since the last inspection in August 2005 staffing has been increased to provide more opportunities for activities and a number of bedrooms have been redecorated in line with the programme for decoration and re-furbishment, which has been developed.

What the care home could do better:

The manager and proprietor must address those issues requiring action which are detailed at the back of this report including continuing to develop the activity programme and training in Adult protection.

The home is situated close to community facilities. Accommodation for the 15 service users is provided in 11 single bedrooms and two double bedrooms. There are no ensuite facilities in this home. The rooms are appropriately decorated to the service users taste and they are furnished with their personal belongings.The home has two lounge/dining areas, which overlook the garden. One of the rooms has a television whilst the other is a quiet lounge. The kitchen is commercial in design and not accessible to service users although drinks/snacks are made for service users and visitors on request. A stair lift is available for those service users requiring assistance to access first floor facilities. The inspection took place over four hours, with the inspector examining a number of records; speaking to three residents, two relatives, the manager and a member of the Care staff. A tour of the building was carried out and requirements identified at the last inspection were re-visited. This inspection looked at those key standards, which were not examined at the last inspection in April 2005. Any issues identified as requiring action are to be found at the back of this report. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 and 14. Although there are some activities for residents these should be further developed. Staff encouraged and supported residents to maintain links with family and friends. Residents were able to exercise control over their lives. EVIDENCE: It was apparent from observation and conversation with staff, residents and two visitors that there was a relaxed and friendly atmosphere in the home. However a number of people said that there should be more opportunities for activities and particularly trips outside of the home. This should be driven by resident and staff consultation and supported by a programme of internal and outside activities. Routines were flexible and allowed residents to exercise personal choice and maintain control over their lives, subject to their individual plan. All rooms had an information book and this contains information, which encourages contact with family, friends and the local community. The manager explained that the local Vicar visits monthly and the Catholic Church visits weekly; also staff take residents to the local shops and library when staffing allows. Residents and visitors were seen to be freely coming and going to their own rooms as they wished and rooms were personalised with individual possessions. Only two residents have monies that are held safe by the Home on their behalf with the remaining residents either holding their own money or relatives hold it. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Appropriate policies and procedures are in place in relation to the protection of vulnerable adults and residents feel safe at The Hawthorns. Not all staff have yet received training/instruction in adult protection. EVIDENCE: The Manager said that eight of the fifteen care staff had undertaken Adult protection training. Whistle Blowing and adult protection policies are in place together with an Adult Abuse incident procedure; however the manager must ensure that all staff including ancillary staff receive an appropriate level of training/instruction. No incidents of adult abuse have been reported and residents and relatives spoken to on the day of inspection all said that they feel safe and protected by the staff at The Hawthorns. The money held safe on behalf of two residents is appropriately kept, with a recorded final balance, however it is recommended that this would be better protected from abuse if it was only released on two signatures. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Although the environment was not the focus of this inspection and the home was observed to be generally clean, well decorated and maintained there were areas of worn carpet, which need replacing. The provision of place mats and serviettes at the dining table would enhance the expected image as well as the resident’s comfort and convenience. EVIDENCE: Worn carpet, which must be replaced, was observed at the emergency entrance and on the stairs. It was observed that at lunchtime there were no place mats or serviettes on the dining table. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 and 38 Service users financial interests are safeguarded and the health, safety and welfare of residents is protected. However these areas would be strengthened with two signatures to release residents monies and bath water should not only be safe but also comfortable. EVIDENCE: Money is kept safe on behalf of only two residents with the remainder either handling their own or it is managed on their behalf relatives. The records and cash balances checked were found to be correct. The safeguarding of residents and staff would be strengthened if money was only released on two signatures. A number of health and safety records were examined to confirm that up to date maintenance/checks were in place relating to the gas safety certificate, portable electrical appliance and hot water temperatures. Two bath outlets were checked which recorded temperatures of 39 and 40 degrees which although safe may not be comfortable for all residents.
