This inspection was carried out on 13th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is set very close to Lancaster centre and is pleasantly situated in its own grounds. Daily routines for the people living at Hillcroft Lancaster are as flexible as they can be and designed to meet the residents` individual needs. Meals are good. All of the residents that were spoken to during the visit said that they were very satisfied with the quality and variety of meals that are provided. Comments from the residents were all complimentary. Carers are friendly and the residents spoken to said that the staff were all helpful and caring. The home is kept clean and hygienic.

What has improved since the last inspection?

The medication systems have been changed to a monitored dosage system, and this makes sure that written records of medicines for each resident are accurate and safe. Records of all meals that the residents have are now to be kept for three years, in accordance with the Care Homes Regulations 2001. This enables any person inspecting the records to confirm that a nutritious and/or special diet has been provided. Coded doors have been put in place to ensure the safety of residents. A new up-dated nurse call system is to be installed in March 2006 which. A new computer system administration of the home. has also been purchased to improve theGenerally there are plans to make a number of changes and improvements to the home and these will be looked at during the next site visit to the home.

What the care home could do better:

There are possible plans to re-site the laundry however, although there is a sluice situated immediately to the entrance to the laundry room, a hand-wash basin fitted in the laundry room would doubly ensure that hygiene was maintained.

The home is registered for a maximum of 18 service users within the category of Older People (OP) The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection Date of last inspection Brief Description of the Service: Hillcroft Lancaster is a Care Home with Nursing, situated close to the railway station and all the amenities of Lancaster centre. It provides full nursing care for 18 persons in ground and first floor accommodation. There is access for wheelchair and semi-mobile service users and a passenger lift is in place. All bedrooms are decorated and furnished to a good standard. There are currently two lounge/dining areas. There are adequate toilets and assisted bathrooms. Trained nurses are on duty at all times. The home is well maintained with well kept garden areas around the building. There is adequate parking space for visitors. This is the second of two unannounced inspection visits, scheduled from 1st April 2005 to 31st March 2006. The home has very recently changed ownership and all of the philosophies, policies and procedures are to be changed to those of the new owners, Hillcroft Nursing Homes Ltd. All of the people spoken to at Hillcroft Lancaster said that they preferred to be called residents. The inspection took place from 10am to 1pm and was carried out by the home’s designated lead inspector and an accompanying inspector. A tour of the home was done and this included some of the bedrooms, lounge and dining areas, laundry and bathrooms. No comment cards were received from residents or relatives, with one GP comment. The inspectors spoke with a number of the residents. There were no relatives available during the inspection visit. The manager, nurse in charge and care staff were spoken to. Everyone was welcoming and co-operative throughout the visit. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 The are comprehensive and clear care planning systems in place, which means the needs of the residents are met in a consistent and caring manner. EVIDENCE: A new care pathway system has been introduced into the home where a care menu of needs is done for each prospective resident on the pre-admission assessment visit; this is then developed into a care pathway for the new resident. The system means that identified needs in the care menu are transferred into a working document (care plan) or care pathway; these have also been introduced for existing residents. Care pathways are reviewed either monthly or as residents needs change. A number of care plans were looked at and these showed that the information is clear and that reviews take place. Staff on duty said that they were working with the new care pathways. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Residents are protected through all members of staff having abuse awareness training. EVIDENCE: Hillcroft Lancaster now have the Hillcroft Nursing Homes Ltd organisational established training programmes for abuse awareness. All staff are currently being assessed for abuse training needs. Copies of the General Social Care Council Code of Conduct for Carers are available for every member of staff. Nurses and carers said that they were aware of abuse issues. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): No judgement has been made. EVIDENCE: All of these standards were assessed on the previous inspection visit and each was met. There are currently plans underway for the redevelopment of the home, including refurbishment programmes. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 & 30 Staff are properly checked before being employed at the home and there are clear training programmes that make sure that staff have the information and skills to meet the range of care and support needed by the residents. EVIDENCE: Two staff files were looked at and these showed that there is a satisfactory recruitment procedure in place. The company’s training manager will be undertaking a full audit of training needs and appropriate training will be provided. Induction training is already in place, which includes a seven-day programme covering all aspects of care work. Existing staff have been provided with an introduction to their new parent company and an experienced senior care worker is giving support in respect of the new systems which have been introduced. Standard 27 was not assessed at this inspection, however one comment card raised an issue in respect of reduced staffing levels. This issue did not have any adverse effect on the residents at the home who were observed being attended to appropriately and in a timely way by staff who were competent, confident as they went about their business. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 & 38 The home makes sure that the personal and financial interests of the residents are protected in that their health and safety is promoted. EVIDENCE: Since the Hillcroft Company took over the ownership of this home, meetings have been held with both residents and relatives to offer reassurance and to inform them of the proposed changes which they hope to introduce. There are plans to arrange a relatives support group. This group will be made up of current relatives and hopefully, relatives of people no longer at the home and will provide support, information and social gatherings. The group will meet on a monthly basis, with input from the Directors, the General Manager and the manager of the home. These meetings are considered valuable and will provide an opportunity for relatives to meet with others, to share information and to Hillcroft (Lancaster) Nursing Home DS0000066641.V283626.R01.S.doc Version 5.1 Page 16 make sure “new relatives” are given support and help from someone who has experience in what it is like to see a loved one go into a care home. Quality questionnaires and residents meetings will also take place. There are procedures in place to ensure the resident’s financial interests are protected. The manager confirmed that no monies are held for any residents. The General Manager said that the quality system that is currently used at the company’s other homes is going to be introduced into this home. Feedback will be sought from residents, relatives and external healthcare professionals. The registered provider will also visits the home on a weekly basis and monthly reports will be sent to the Commission. The company’s mandatory training programmes is now underway for all staff that work at the home, and it is expected that all will be completed by 30 May 2006 A health and safety audit is being done and a plan of action will be developed for any issues that might arise. The accident book was examined and found to confirm to the Date Protection. Accidents were accurately recorded.
