Conditions of registration: none Date of last inspection 1st March 2005 Brief Description of the Service: Hilltop Manor is registered to residential personal and social care for up to 35 people over 65 years of age who may suffer from dementia. The property is a two storey building which has been extended and adapted for its present use. There is a lift to provide access between the two floors and pleasant secure gardens at the rear of the property. The home is situated in the village of Sherburn-in-Elmet, and is close to local services and amenities. The registered providers are Mrs Alison Green, Mr Allen Williams, and Mrs Claire Buckle. This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried out as part of the inspection process on 2nd August 2005. Both Mrs Green and Mrs Buckle were available to assist with the inspection and it was also possible to speak to 6 members of staff and many of the residents. I also spoke to a visiting nurse, the local doctor who has a lot of contact with the home and a relative of one of the residents. They all spoke very highly of the home and the quality of the staff and care in the home. The inspection also involved a check on the recommendations from the previous inspection, a tour of the premises, and a check on some of the records. The inspection took 8 hours which includes preparation time. The home was very clean, well decorated and furnished, and there was a pleasant atmosphere. The residents were in the lounges, talking and listening to music and the staff were busy in a variety of care duties. All the residents spoken to said it was a nice home and that the staff were courteous and helpful. The inspection showed that the home was very well organised and managed and that the staff were well trained and aware of their duties, and the residents were well cared for. What the service does well:
It was clear that the registered providers and manager work very hard to ensure that a high quality of care is provided for the residents. Training the staff to ensure that they can meet the residents care needs. It provides a safe and comfortable home for the residents to live. There is a nice atmosphere in the home and staff and residents work together very well. Residents said that they ‘ liked living at Hilltop Manor’. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3,6. Residents know that if they choose to live at Hilltop Manor they will be well cared for. EVIDENCE: The manager assesses all residents prior to coming into the home and they have an opportunity to visit and look around before being admitted. The home was able to demonstrate this following a recent admission. All residents have the information they need before they come into the home. All residents have a contract which gives the conditions of their stay in the home. The home is not registered to provide intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. The residents’ health and personal care needs are met and they are encouraged and supported to make choices about their daily lives. This helps them to have control over their lives in the home. EVIDENCE: All residents have good care plans which are used by the staff to ensure that appropriate care is provided. It was recommended that more detail should be included on how the objectives will be met. Staff meet regularly to discuss the care plans to ensure that consistent care is provided. . The home has good access to the local health professionals who visit regularly to ensure that the residents’ health needs are met. The local GP and a visiting nurse both assured the inspector that the communication between the home and themselves is very good which promotes good health care for the residents. The GP visits the home weekly and knows most of the residents. The nurse visits the home regularly and both considered that the care practices in the home were very good.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. Residents eat well and have a suitable range of activities both inside and out of the home and are part of the local community. EVIDENCE: Food is an important part of the residents’ lives and they help to choose the menus with the cook. Special diets are well catered for as are personal preferences as some of the residents were happy to testify. All the residents spoken to were happy with the food provided. Friends and relatives visit regularly and are welcome in the home. A visiting relative was pleased to report on the care in the home and the good communication between the staff and herself. A member of staff has the responsibility for organising activities and outings. She spoke enthusiastically about her role and plans for future events. The residents said they were happy with the level of activity in the home. One resident has a hobby and is given lots of encouragement and support to continue with it. Daily and local newspapers were in the home which helps the residents to keep up to date with events in the world and locally. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. The residents physical and emotional health needs are met. Residents are protected from abuse neglect and self harm EVIDENCE: All staff have been trained in the protection of vulnerable adults procedure and they were aware of their responsibilities, which safeguards the residents from abuse. Residents said that they would speak to the manager if they had any concerns and felt happy that she would help them. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,23,24,25,26. The home is clean and well decorated and furnished throughout, making it a comfortable place to live. EVIDENCE: There are four lounges and two dining rooms which provide comfortable space for a variety of activities or just quiet places to sit and talk or read. On the day of the inspection one dining room was being for a game of dominoes. Overall the standard of furnishings and decorations did make the home a pleasant place to live. Residents are encouraged to bring their personal items into the home to make their rooms feel homely. The home was clean and free from unpleasant smells and a good infection control policy was in operation.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30. The staff are well trained and well supported by the manager and this ensures that the residents feel supported and that the staff are aware of their duties. EVIDENCE: Staff training, related to the needs of the residents, is ongoing which ensures that the staff felt more confident in their roles and this is reflected in the levels of care provided. A senior member of staff has taken responsibility for staff training and the recording and monitoring of this and the level of support she offers were excellent. The benefits of this are mentioned above and demonstrated by the confidence given to the staff and the quality of care provided for the residents. Residents said that they liked the staff and felt comfortable with them. The staff work well as a team which makes it a happy place to work and staff said they felt valued. One member of staff said it was the ‘best place she had ever worked.’ It was clear from observation that the staff enjoyed their work, knew what they were doing and were very competent. The home prefers to employ staff on a part time basis which provides a greater degree of flexibility to meet the residents needs. The home has a robust recruitment procedure which ensures that the right people are employed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,36,38. There is clear leadership and guidance from the manager which ensures consistent quality care for the residents. This also means that the health, safety and welfare of the residents is promoted. EVIDENCE: It was clear from this inspection that the registered providers and manager work very hard to ensure that quality care is provided in the home. This is reflected in the recruitment and training of staff, the communication with healthcare professionals, the proper assessment of the residents, and the comfortable environment and pleasant atmosphere in the home. The residents views are taken seriously and their dignity and respect is promoted by good staff supervision and on going training.

Because there are so many outlets to check it was agreed that so many should be done each week.
