Conditions of registration: People with dementia over the age of 55 years of age to be accommodated within the 32 bedded unit for older people with dementia. These beds are not registered for the provision of continuous nursing care. Date of last inspection 13 January 2005 Brief Description of the Service: Hollie Hill is a purpose built home that was opened in July 1996. Built to a high specification, it provides each resident with an individual room with its own en suite toilet and hand wash facility. The home is tastefully decorated throughout and residents are encouraged to bring their own pictures and small pieces of furniture to make their rooms more homely. Each section of the home has its own dining facilities and separate lounge. Bathrooms, showers and additional toilets are located throughout the building and all are fitted with special aids. The philosophy of care is based on 6 key principles: privacy, dignity, independence, choice, rights and fulfilment. The home is registered primarily for two groups of service users: 30 Older people with or without nursing care needs, and 32 people over 65 with dementia who do not need continuous nursing care. These two groups are accommodated in two separate units. Hollie Hill aims to ensure that service users will receive the level of care that their own situation requires, to satisfy physical, personal, social, spiritual and emotional needs. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 16th June 2005 over a period of 6 hours. During the inspection some of the time was spent talking to the residents, staff and relatives. A selection of comment cards were given to service users with pre-paid envelopes provided. A tour of the building was also undertaken and a number of care plans were examined. Time was spent observing the care provided to residents in the home. The home have reviewed the audit system with regard to medication. The requirement from the previous inspection that an accurate record is maintained of medicines stored and administered has been met. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3,4 &5 Service users are unaware of the terms and conditions that apply to their stay within the home. Service users are appropriately assessed prior to admission and have the opportunity to visit the home prior to admission. EVIDENCE: There is still no statement of terms and conditions available to service users. The company are aware that this issue has been outstanding for some considerable time. Examination of an assessment of a person who had recently been admitted to the home, confirmed that comprehensive assessments are undertaken and it was confirmed that he had been invited to visit the home prior to admission. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9,10 &11 The care planning system is inconsistent, with some not providing the information that staff would need to satisfactorily meet the needs of service users. EVIDENCE: 6 care plans were reviewed during the course of the inspection process. 2 did not contain plans to meet all of the needs identified in the assessment and had not been reviewed and evaluated on a regular basis. Where appropriate, plans contained some evidence of input from other health care professionals. Several staff have undertaken specialist training in the area palliative care. Company policies are available for staff to consult and specialist services are utilised when required. No concerns were raised during interviews with service users, staff or visitors and no adverse observations were made during the inspection process. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 24, 25 & 26 Hollie Hill was purpose built and therefore meets the needs of the people who live there. The overall quality of furniture and fittings is good and redecoration is carried out on a rolling programme. EVIDENCE: The home was purpose built and meets the room sizes as stated in the National Minimum Standards. During the tour of the building and whilst talking to people in their bedrooms, the inspector saw that service users could bring in their own furniture and belongings should they wish to do so. The family of one service user recently admitted to the home confirmed that they had been given a choice with regard to the redecoration of the room. One relative commented that the temperature of the upstairs lounge in the nursing unit was hard to control, being either too hot or draughty. There was a range of specialist equipment seen around the home and bathrooms were noted to be appropriately equipped.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28 & 30 Staff are generally trained and competent to do their job, although the home have not achieved the standard for the ratio of care staff qualified to NVQ Level 2. The procedures for the recruitment of staff are robust and offer protection for the people who live there. EVIDENCE: The staff files reviewed indicated that all of the required recruitment checks are now made before a new member of staff starts to work in the home. After talking to staff and reviewing records it was confirmed that recent training has taken place in the areas of care planning, nutrition, food hygiene, fire safety, first aid, moving and handling and ‘Protection of Vulnerable Adults. The company have now implemented their own programme to ensure that the appropriate numbers of staff are qualified to NVQ 2. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34 & 38 The home has a Registered manager in place who is positive towards the provision of a good standard of care delivery to service users. The home regularly reviews its performance through seeking the views of service users, staff and relatives. EVIDENCE: The Registered manager confirmed that she has commenced an appropriate management qualification and hopes to complete this soon. Feedback from service users, staff and relatives confirmed that the manager is approachable and friendly. There was evidence that quality assurance systems are in place with regular consultation at home and company level taking place with service users. There were up to date maintenance certificates available to demonstrate that the health, safety and welfare of service users are promoted and protected.

STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP 2 Regulation 5& Schedule 4 Requirement The home must ensure that each service user is provided with a statement of terms and conditions at the point of moving into the home (or a contract if purchasing their care privately). (Previous requirement timescale of 31/03/05 unmet.) Comprehensive assessments must be undertaken upon admission and care plans formulated for all service users.
