Conditions of registration: 1. 2. 3. The service must at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. The home is registered for a maximum of 26 service users in the category of OP (old age, not falling within any other category). The staffing levels for the home must meet the Residential Forum Care Staffing Formula for Older Adults. Date of last inspection Brief Description of the Service: Holmewood offers accommodation and care for up to 26 older adults. The Home is decorated and furnished to a good standard and is pleasant and comfortable. All of the bedrooms are for single occupancy, but one can be used for two people sharing if requested, they are spacious and individually decorated and furnished. All of the bedrooms have an en-suite toilet and bath or shower. The Home has two large and two small lounges and a dining room. There is a passenger lift, a platform lift to assist with three stairs, handrails, grab rails and a range of equipment to assist people in their day-to-day lives. The Home is set in large well-maintained gardens, including a vegetable garden and has seating and easy access. This is an overview of what the inspector found during the inspection. This inspection was an unannounced visit, which took place over one day. The visit included a full tour of the home and discussions with people living in the home and some of their relatives. The manager was spoken to and a member of the care staff. The visit included looking at a sample of the records kept in relation to the people living at the home and the staff working at the home. At the time of the visit there were 23 people living at the home. What the service does well: What has improved since the last inspection? What they could do better:
The home has neglected the staff training and staff supervision due to other pressures. The manager is aware of this shortfall and has started to address the matter. Staff supervision has commenced and this will the help the manager to develop a training plan to ensure that all staff have appropriate training and refresher training to further meet the needs of the people living at the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3 and 5 The home has a thorough assessment and admission process, which includes trial visits. This ensures that service users and their representatives know that the home is suitable and will meet their requirements. EVIDENCE: Service users and their representatives are given information about the home in order for them to make an informed choice. At the time of this inspection the manager was reviewing and updating this information. The files looked at during the inspection contained detailed assessments of the needs of each service user. The assessments had involved the service user, their family members where appropriate, senior staff from the home and the service user’s social worker. Service users were able to visit the home prior to admission to ensure as far as possible that the home was suitable. Reviews had taken place at the end of the first month in order to confirm that the home was suitable and meeting the needs of the service user. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 The personal records of people living at the home include detailed plans of their care needs with recorded goals and outcomes. This helps to ensure that staff are provided with the information they need to care for people appropriately. EVIDENCE: The care plans looked at during the inspection contained detailed information in respect of the individual care needs of each service user. The plans included risk assessments and moving and handling assessments, which had been reviewed each month. Appointments to and visits from other healthcare professionals (for example district nurses, opticians and chiropodists) were recorded. This shows that service users have access to specialist services when necessary. Following an assessment, people living at the home are able to take responsibility for their own medication if appropriate and the home would provide them with a secure place in which to store their medication. Records relating to the administration of medication had been accurately maintained. Medication was stored securely and in a well-organised fashion. The home benefits from the services of a very supportive pharmacist, who

During this visit, staff were observed caring for the people living in the home. It was very evident from their actions and conversations that the staff were fully aware of the needs of the residents ensuring that privacy and dignity were maintained at all times. The home has a private room with a telephone and computer with the web cam facility for use by service users. This has been installed to enable service users to keep in touch with their relatives and friends. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The home provides people living at the home with a range of activities to help meet their social, recreational and spiritual needs. EVIDENCE: There was a range of activities available for service users to participate in if they wished to. A programme of the activities arranged for the week was posted in the reception hall. People living at the home and their relatives said that they enjoyed trips out in the mini bus to local places of interest or shopping. The residents said that they could choose to join in the activities if they wanted to and did not feel pressured to do so if they didn’t want to participate. Relatives and friends of service users said that they could visit the home anytime and were always made to feel welcome and were offered refreshments by the staff. Religious services were organised on a monthly basis from the various local churches. Service users were also able to receive individual visits from their own religious leader. Several people living at the home said that they had made lots of friends at the home and that everyone was nice and friendly. The weekly menus were looked at during this visit. A well-balanced and varied diet was provided. Residents said that the food was very good and that there was plenty of choice. They felt that if they wanted something different, then

If service users wanted to they could have their meals in their own room or in the dining room, which was clean, bright and cheerful. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17 Service users and their relatives are aware of the home’s complaints procedure and feel confident that any concerns or suggestions they may have would be listened to and dealt with by the manager. EVIDENCE: The home has a satisfactory complaints procedure and a copy of this is given to service users and/or their relatives when they come to live at the home. The home had not received any complaints at the time of this visit. There was a system in place to monitor complaints, suggestions and compliments as part of the homes quality assurance programme. Service users and their relatives said that if they had a problem they could speak freely to the manager or other care staff. They felt confident that if they did experience any problems that they would be listened to. Nothing was ever too much trouble for the staff at the home. The home had access to advocacy services and the manager assists service users and/or their relatives to obtain these services if needed. Where service users had a social worker, records showed that they were involved in reviewing their care needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, 22 and 26 The home provides service users with a safe, pleasant, clean and wellmaintained environment. EVIDENCE: The home is very well maintained and provides a comfortable and homely atmosphere. Service user’s rooms are furnished to a high standard and have en-suite facilities, including either a bath or shower. Service users may personalise their own rooms if they wish. Many people had brought items of their own furniture, pictures and ornaments with them into the home. There are two large lounges, a dining room and smaller more private lounge that are used by people living in the home and their visitors. There are several communal bathrooms and toilets within the home with specialist equipment available to help maximise the independence of the people living at the home. The home was clean, pleasant and hygienic. Housekeepers are employed at the home to ensure that the environment is kept clean at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 and 30 A robust recruitment and selection process is in use at the home to help ensure that appropriate staff are recruited. The home employs sufficient numbers of staff with various skills in order to meet the needs of the people living in the home. Staff training needs required reviewing and updating in order to develop an appropriate training plan. This will help to ensure that staff have the skills and knowledge to help them provide good quality care to service users. EVIDENCE: Care staff, housekeepers, kitchen staff and the handy man were on duty at the time of this visit. The manager, general manager and deputy manager were also available at the home. People living at the home said that staff were very kind and caring and that they would go out of their way to make sure that they were well cared for, happy and everything that they needed. A relative of a service user said that the current manager was fantastic. Her mother had lived at the home for five years and they had noticed a great difference in the standards at the home. If service users required assistance it was provided promptly and in a caring and dignified manner. The current manager at the home is an experienced manager. She has not undertaken the registered managers award as she is preparing for her retirement later this year. The deputy manager was said to be taking over as manager and as part of her preparation she is undertaking the registered managers award. This will help to improve her management skills and ensure

The manager did not have a current training and development plan for the staff working at the home. She was in the process of meeting with each member of staff in order to identify any training shortfalls. Eight members of staff have gained National Vocational Qualifications (NVQ) in care. One of the housekeepers had also obtained an NVQ award. Some training had been booked to take place in the near future. This included First Aid, Fire Training and a specialist training lecture on Parkinson’s disease. One member of staff was spoken to during the visit. She had previous experience of working in care. She was very happy working at this home and said that she had been provided with suitable training in order for her to carry out her tasks safely. Two staff personnel files were looked at during the inspection. They were found to contain details of all the necessary checks that must be undertaken to ensure that staff are suitable for employment in the care home, especially when working directly with vulnerable adults. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, and 36 Experienced and competent persons manage the home and this helps to ensure that the home is run in the best interests of service users. EVIDENCE: The manager is experienced in the management of care homes. A deputy manager who is currently undergoing training in order to obtain the registered managers award assists her. A quality assurance programme was in progress at the time of this visit in order to obtain the views of people living at the home, their relatives and people working at the home. A specialist company has been employed to assist with the evaluation of this programme. The manager was aware that the supervision of staff had not been carried out as frequently as it should have been. A staff supervision programme has been devised and the manager was working hard to ensure it kept to the plan. People living at the home are encouraged to manage their own finances where possible. This is sometimes with the assistance of a relative or friend. Small

This was stored securely with appropriate records being kept. Tests and checks of equipment used in the home were regularly carried out and records were kept. This helps to ensure that the home is a safe place to live and work. The fire officer had made a recent visit. He had made a requirement at the home.
