Conditions of registration: 1. The home is registered for a maximum of 28 service users to include: Up to 14 service users in the category of DE(E) (Dementia over 65 years of age). 1 service user in the category MD (Mental disorder under 65 years). Up to 13 service users in the category OP (Older person over 65 years of age not falling within any other category). 30th August 2005 Date of last inspection Brief Description of the Service: Howard Court is registered to provide social and personal care for up to 28 older people, 14 of whom may have varying forms of dementia. The home is owned by Howard Court Care Home Ltd and managed on a day to day basis by Mrs Jean Beattie. It is situated in the centre of Brampton, a market town approximately eight miles from Carlisle, and close to all amenities and public transport. Residents accommodation is on two floors, the upper being served by a passenger lift and a stair lift. On the ground floor there are bedrooms, a large lounge/diner, a quiet lounge, toilets and a bathroom. On the first floor there are two small lounge/diners, bedrooms bathrooms and toilets. There is also a ground floor conservatory providing further communal space for the residents. There is a small courtyard to the front of the building and limited parking space. This is an overview of what the inspector found during the inspection. This was an unannounced inspection of the home and the second one of the yearly cycle. The visit took place over one morning. During the inspection, time was spent talking with the manager, care staff on duty and the cook. The inspector discussed the care plans, other documentation and records to do with the day-to-day running of the home and the care of residents. Time was spent talking with some of the residents in the lounge and in their own rooms. Those standards that were not inspected on this occasion were assessed and met during the previous inspection that took place in August last year. What the service does well: What has improved since the last inspection?
Internal decoration has meant that some rooms have been completely refurbished whilst others have been redecorated. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1&4 Prospective residents are given a statement of purpose and contract and terms and conditions that provide sufficient information to enable an informed choice to be made about moving into the home. EVIDENCE: The home has a comprehensive statement of purpose that is made available to all prospective residents and/or their families. The information contained in the document is sufficient for the resident to make an informed choice about whether or not to move into the home. All residents are also given a contract and terms and conditions outlining the facilities on offer at the home. All prospective residents and their families are invited and encouraged to visit the home to look around, meet the staff and others living in Howard Court. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 Residents benefit from a policy that ensures their wishes after death are met with dignity and respect. EVIDENCE: Although the care plans were not inspected in detail during this visit the manager did advise the inspector that the recommended work on upgrading the plans had not started. She does, however hope to look at the format in the near future in order that a more in-depth plan of care can be provided for each resident. The home has a policy to deal with the death of a resident that is easy to understand, with the wishes of the resident documented on the care plan wherever possible. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Residents benefit from being able to express their wishes about how to spend their time and are given the choice about whether or not to join in any organised activities. EVIDENCE: Discussions with the manager evidenced that the residents were not really interested in attending residents’ meetings and that they preferred “ to go with the flow.” The residents confirmed this when speaking with the inspector. It has been found that the best way to get residents’ opinions is through questionnaires and one to one discussions with the activities co-ordinator. JUDGEMENT – we looked at outcomes for the following standard(s): 17 Service users benefit from the availability of an advocacy service should this ever be required and the opportunity to take part in local and general elections. EVIDENCE: The home has a policy outlining the rights of those living at Howard Court and what is expected of the staff working there. This policy includes information about the use of advocates and when this service should be used. Details are on display about an organisation called “Care Aware” a professional organisation available for help and advice. Those residents who wish to vote in any of the elections are given the opportunity via the postal vote system. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 & 25 Residents benefit from a warm and friendly environment in which to live. EVIDENCE: The inspector was able to inspect a sample number of bedrooms and found them to be pleasant and suitable for their stated purpose. They were all nicely decorated and personalised with pictures and photographs from the residents’ own homes. Some have been completely refurbished or redecorated since the last inspection. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Staffing arrangements are appropriate, ensuring the needs of the residents are met. EVIDENCE: There is a full complement of staff, some of whom are trained to NVQ level 2 or 3. There were five members of care staff on duty on the day of the inspection, plus the manager. These staff, plus the cook and domestic staff were able to attend to peoples needs, keep the home clean and provide meals and snacks during the day. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 36 & 37 There is clear leadership, guidance and direction to staff to ensure residents receive consistent good quality care. Residents benefit from a relaxed style of management and approach. EVIDENCE: Discussions with the manager and care staff during the inspection evidenced that they work together as a team to ensure the residents receive a consistently good standard of care. The residents said the home was “a nice place to live in “ and that everyone “did their best to look after them”. One of the newer residents has been able to bring their dog and this is proving very popular with the other residents. Staff supervision takes place every 2 months with records available for inspection. The home has a full set of policies and procedures that are made available for the staff to read any time they want to.
