This inspection was carried out on 1st February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This home has a consistent record of low staff turnover with the majority of the staff group having worked at the home for a significantly long time. However commitment to training is high and half of staff members are qualified to NVQ level 2 in care or above. The residents continued to praise the manager and staff for maintaining standards.

What has improved since the last inspection?

Four bedrooms had been completely refurbished since the previous inspection to provide a more homely and pleasant environment for residents. The work to ensure that residents signed their care plans had been completed in accordance with the good practice recommendation.

What the care home could do better:

The service user guide requires updating and a good practice recommendation has been made to cover this.

Registered for 25 Older People And 1 Older Person with Dementia Date of last inspection 22nd June 2005 Brief Description of the Service: Inglewood is a care home providing accommodation and care for up to 26 older adults, one of whom may have dementia. The home is an older property, which has been adapted for its present use. It is situated in the Denton Holme area of Carlisle, approximately one mile from the city centre. The accommodation for service users is provided on two floors, and there is a passenger lift and stair lift. There is one bedroom that is shared by two people, the remaining rooms are for single occupancy. A large number of the bedrooms have ensuite toilet facilities. There are bathrooms, which are equipped to assist people with a disability. There are two communal lounges and a dining room. People who wish to smoke can do so in the conservatory. The home has a pleasant garden to the front of the home with seating. There is a small car park. This was an unannounced inspection of the home, and during the inspection the Inspector talked with the Manager, residents and care staff, looked at a selection of records relating to the care of residents and the day to day running of the home. The Inspector looked around all parts of the home including communal areas and residents’ private rooms. Requirements made at the last inspection had been met. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2 Prospective residents and their representatives are given sufficient information to help them to decide whether to move into this home. EVIDENCE: The home has a comprehensive service user guide that is based on the statement of purpose. It contains clearly written information about the accommodation and what services are provided. The home’s complaints procedure and terms and conditions of residency are also included. However there are certain items that should be changed; for example the accommodation charges were out of date and the Commission for Social Care Inspection has replaced the National Care Standards Commission. The manager has been advised to amend these details. Prospective residents or their representatives are also given a copy of “The brief home guide” produced by Counsel and Care to provide them with additional information. Written contracts clearly stating the terms and conditions of residency had been issued when each resident moved into the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 9, Residents have been involved in drawing up their plan of care. Routine safe practices ensure the residents are protected from harm. EVIDENCE: The care plans for each resident were looked at during the previous inspection and were found to have sufficient information to enable staff to identify individual needs and provide appropriate care to meet them. However a number of plans had not been signed to show that the resident had been involved. This shortfall had been addressed since then and all plans had been signed by residents or their representative to show that they agreed with their care plan. The manager was interested about the benefits of person centred planning and said that she would like to learn more about the practice so that she could work with the residents to develop the plans further. The home has a policy to deal with residents who wish to continue administering their own medication. However, currently there are no residents who have elected to continue to do so. All medication was securely stored and records duly signed and up to date. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 Most residents were satisfied that the range of choices available to them allowed them to maintain adequate control over their lifestyle. EVIDENCE: Residents said that they were able to get up and go to bed at times to suit their preferences. Wherever possible they had been encouraged and assisted to maintain friendships and to pursue the social activities they had enjoyed prior to moving in. The manager believed that staff members had benefited from training and so were aware of the need to provide a range of options for residents to help them to maintain their independence. The residents confirmed that they had enough interesting activities to occupy them during the day. Most residents were happy to have their main meals together in the dining room although the kitchen staff accommodated any resident who chose to eat later, and this was seen to be the case. Choices were available at all meals. All private bedrooms had space for favourite and valued possessions for residents to bring with them to add to the furnishings provided by the home and the majority of residents had taken advantage of this opportunity. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 The home has a satisfactory procedure in place to ensure that the legal rights of residents are protected. EVIDENCE: The manager ensured that the electoral roll was kept up to date and that relevant information was given to residents to encourage them to vote in political elections. On previous occasions some residents had preferred to register for a postal vote and some residents had been assisted to go to the local polling station to place their vote. Staff members were aware of the local advocacy service that could be approached if residents were unable to speak for themselves or had no next of kin or relatives to support them. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26 This home continues to invest in a rolling programme of redecoration and refurbishment to provide a safe and comfortable environment for the people who live and work there. EVIDENCE: The manager undertakes a regular six monthly audit of the state of all rooms in the home to identify what work needs to be done to improve them. At the time of this inspection several bedrooms had been or were being redecorated and new carpets and matching soft furnishings had been provided. Residents appreciated the refurbishment and had been included in choosing colour schemes and patterns. The home has a policy of refurbishing all rooms that become empty ready for the new occupant. Residents are encouraged to bring in small pieces of furniture provided they do not pose a fire safety risk, and other favourite possessions. The staff in the home have developed a professional working relationship with health care professionals, referring cases for assessments and advice about mobility aids and adaptations to enable residents to be as independent as Inglewood Residential Care Home DS0000022693.V280148.R01.S.doc Version 5.1 Page 12 possible. There are sufficient bathrooms and toilet facilities for the number of residents living in the home. Some bedrooms have en-suite bathrooms. The three main bathrooms had been adapted to assist the less mobile residents to bathe in safety and as much independence as possible. In addition there are five shared toilets with hand washbasins. The home was warm, clean, tidy and well decorated in all areas. Residents said they liked the homely environment in all communal rooms and private bedrooms. Staff had been given training in infection control and this was also covered during staff supervision. Care staff were required to use protective equipment including rubber gloves and alcohol based hand rub to protect residents and themselves. All staff who routinely go into the kitchen had been given training in food hygiene. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 The procedures for staff recruitment are sound and offer protection for people living and working in the home. EVIDENCE: The staff turnover remained low with a core group of staff members who had remained loyal over the last 12 years or so. However whenever new members had been recruited all checks, including references, employment history and Criminal Record Bureau disclosures had been undertaken before they began their employment. The home has adult protection and whistle blowing policies in place to protect everyone living and working there. Training had been given to staff to back these up and staff knew who to contact and what to do if they suspected any form of abuse. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35, 36, 37, 38 The registered manager provides adequate direction, guidance and leadership for the staff so that residents are given consistent quality care. There are procedures and practices in place to ensure the health and safety and wellbeing of the residents, staff and visitors. EVIDENCE: The registered manager is an appropriately qualified and experienced manager in residential care. Individual supervision and appraisals ensure staff all understand what is expected of them. The manager provided evidence of her commitment to provide training courses for her staff and she said that she kept her knowledge up to date by sitting in on some training courses run by external trainers. Staff had been given training in specific areas to protect themselves and residents from harm. The training included moving and handling, infection control, fire prevention and what to do in the event of a fire, food hygiene and first aid. Staff members are supervised “on the job” and at Inglewood Residential Care Home DS0000022693.V280148.R01.S.doc Version 5.1 Page 15 handover sessions between the shifts. Matters were addressed as necessary to ensure safe and appropriate working practices. Records show that all care staff had been given formal supervision at least six times per year. The fire alarm and equipment tests had been carried out within the appropriate time scales and the entries into the fire logbook had been recorded correctly. The home uses an accountant to check and audit the accounts and in order to protect the financial interests of the residents there is a policy that money is not held on their behalf. Relatives, appointed representatives or social workers assist any residents who are incapable of managing their own financial affairs.
