What has improved since the last inspection?

Following the last inspection two recommendations were included in the report. The home has put them into place. Both the time when the daily report was written and the temperature of the room where the medication was stored have been actioned and will be reviewed.

What the care home could do better:

During the inspection there were no identified requirements or recommendations. The manager is aware that standards can always be exceeded and the home will continue to strive towards achieving this.

The home may accommodate 43 service users in the old age (OP) category. Date of last inspection 29 March 2005 Brief Description of the Service: Jeanne Jugan Residence (Little Sisters of The Poor) Care Home with Nursing offers care and nursing to 43 residents aged 65 years and over. Care is offered to residents based on an ethos of Christianity with management and senior staff belonging to the religious order. The home is situated in the popular area of Fulwood, Preston. Local services are close by. The home is on a regular bus route, with the towns railway a short distance away. Accommodation is provided on ground and first floor levels, in a large detached Victorian property designed to care for the sick and dying since the early 1800s, and retains many of its original features and characteristics. There are 43 single bedrooms, 27 of which have en-suite facilities. This unannounced inspection took place over five hours and started at 10.30 on a day during a normal working week. The inspector spoke to seven residents, two visitors, a volunteer, three members of staff and the manager. Part of the inspection was spent on looking at care documentation together with the homes policies and procedures. The inspector also toured the home to assess the physical standards. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5 Residents move into the home having had their needs assessed and have been assured that these will be met. EVIDENCE: One resident’s care documentation, which had recently moved into the home, was looked through as part of the inspection. The resident had only been admitted on the basis of a full assessment undertaken by the Registered Manager of the home. The assessment format is a pre-printed template supplied to Little Sisters internally within the organisation. The assessment covers for example personal care and physical well-being, diet and weight, including dietary preferences; sight, hearing and communication. Each resident has a plan of care for daily living, and longer term outcomes, based on the assessment and Care Plan. All services offered by Little Sisters are demonstrably based on current good practice, and reflect relevant specialist and clinical guidance. Many of the residents have visited the home prior to admission either as a relative of a previous resident or as a day resident. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The health and personal care, which a resident receives, is based on the individual’s needs. EVIDENCE: The resident’s plan of care was seen to have been generated from a comprehensive assessment. The three care plans that were examined found that the plan set out in detail the action which needs to be taken by care staff to ensure that all aspects of the health, personal and social care needs of the resident are met. The Care Plan had recently been reviewed just prior to the unannounced inspection. The daily notes were well written and included good informative information. Nutritional screening is undertaken on admission and subsequently on a periodic basis, a record maintained of nutrition, including weight gain or loss, and appropriate action taken was seen to be recorded in the three files looked at. The manager was observed knocking on resident’s doors and speaking with the residents in a respectful manner. A member of staff was observed assisting a gentleman with his meals. This was being performed in a sensitive and dignified way. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The arrangements for the excellent social activity programme varies for each resident and residents are given special support and assistance to engage in the activities of daily life. EVIDENCE: The routine of daily living and activities are flexible and varied to suit resident’s expectations and preferences. Residents can exercise their choice in relation to leisure and social activities, food, meals and mealtimes and religious observance. Within the homes plan of care resident’s interests are recorded. The residents are given opportunities for stimulation through leisure and recreational activities. One resident told of how care staff had recently taken him out for a walk. Around the home were posters of forthcoming entertainment acts. One resident spoke ‘ I really enjoy the different people coming to entertain us’. The home has always welcomed visitors into the home and during the inspection it was observed that a number of visitors were present. Visitors are welcomed to join their relative at meal times and enjoy a meal alongside their family or friend. The main meal of the day was observed being taken and was much enjoyed by the residents at the home. The meal was taken in a congenial setting. Should a resident be out meals are kept to one side to be served at a more convenient time for the resident. One relative wrote that meals should be more varied in the evening; this was put forward as a suggestion to the cook. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17 Residents and staff have a robust and effective complaints procedure, which they feel able to use. EVIDENCE: The registered person has ensured that there is a simple, clear and accessible complaints procedure, which includes the stages and timescale for the process, and that complaints are dealt with promptly and effectively. There have been no complaints made to the Commission for Social Care Inspection since the last inspection. Residents spoken with had no complaints to make about the home, they were thoroughly happy with the care they were receiving at Little Sisters. Residents are free to exercise their legal rights and participate in the political process should they wish. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,24,25 The home sets out to offer a family – like philosophy of care that is interwoven between the style of home; it’s size, design and layout. EVIDENCE: The location and layout of the home is suitable for its stated purpose. It has undergone a refurbishment programme to the basement, reception and main lounge in recent months. The grounds, which include an attractive garden is kept safe, attractive and accessible to the residents. Residents spoke how they had enjoyed the good weather recently and sat out around the home. The home has a number of lounge spaces for residents to enjoy one of which is a smoker’s area. The home has a large dining hall for the majority of residents but also smaller dining areas for residents who require help with feeding. Some bedrooms have ensuite facilities. The home has a number of aids, hoists and assisted toilets and baths. Individual bedrooms were looked at and found to be furnished and equipped to assure comfort and privacy. Bedrooms were tastefully decorated and personal items had been brought in from the resident’s own home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30 Staffing levels and skills mix are of a high standard and meet the assessed needs of the residents. EVIDENCE: A staff rota showed which staff are on duty at any time during the day and night. The ratios of care staff to residents must be determined according to the assessed needs of residents. Domestic staff are employed and were observed during the visit. The home has a number of staff members, which have their National Vocational Qualification level 2 in care. It is written in the Care Homes For Older People Standards that a minimum ratio of 50 trained members of care staff excluding those members of the care staff who are registered nurses should be employed by January 2005. The home is on target to meet this standard. Four staff files were looked at during the inspection. All of the four files had references for the member of staff currently in employment at Little Sisters. All four staff had a current Criminal Record Bureau (CRB) certificate, which had been sought by the registered manager. The manager had also obtained a photocopy of the registered nurses pin card. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,36,37 The quality of care provided to residents in this care home is strongly influenced by the calibre of the registered manager. EVIDENCE: Residents, visitors and staff members were very positive in their comments about the homes manager and her style of management. Residents and visitors spoken to said they found the manager to be approachable, supportive and helpful. Staff members said they found the manager was supportive and provided a clear sense leadership. The manager had developed good systems to gather staff, residents and relative’s views and was hoping to introduce them as part of her monitoring of quality. Staff spoken to had a clear understanding of their role and what is expected of them during their shift. Inspection of records for residents was comprehensive, well written and up to date.
