CQC found this care home to be providing an Good service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

All prospective residents are assessed prior to an offer of a place being made. Care plans are in place with some indication that they are updated monthly. There are suitably qualified staff working in the dementia care unit and we were able to observe positive interaction between staff and residents.

What the care home could do better:

Care plans should be completely up to date. A record should be kept of all professional healthcare visits and where intervention is required this should be requested immediately. This will ensure that any prescribed treatment is made available as soon as possible.Staff should ensure all pressure area care is documented and recorded properly and the tissue viability nurse is contacted immediately treatment is required. When supplementary documentation such as turning charts or nutritional intake charts are required these should be followed exactly and kept in a clean and tidy condition. When residents are admitted to hospital their rooms should be locked and made secure immediately to preserve the dignity and privacy of the resident in question. All residents rooms should be locked when residents are admitted to hospital to ensure all their personal items are kept safe.

The home is arranged over three floors with the top floor accommodating people with dementia. Care Homes for Older People Page 2 of 9 Brief description of the care home Express Care owns the home. They have other homes in England - including one other home in Cumbria. Mrs Jean Walker manages the home on their behalf. The home has been registered since spring of 2007. The home is situated in central Carlisle near to the Cumberland Infirmary. It is within easy travelling distance of the centre of town and its amenities. It is also within walking distance of local shops and amenities. There is a car park to the front of the home and a small garden area for residents at the front of the home. The home has its own grounds and accommodation is in single occupancy rooms. Information about the service is available in the Statement of Purpose that includes a copy of the last report. There is also a colour brochure. The cost of care ranges from #389.00 to #525.00 per week depending on need. Further information about the costs can be obtained from the manager or the company. Care Homes for Older People Page 3 of 9 What we found: We made this random visit as a result of concerns raised about the provision of health and personal care in Kingston Court. Two inspectors, Margaret Drury and Diane Jinks made an unannounced visit and were in the home for over 5 hours. During our time in the home we looked at a sample of health and personal care documents relating to various people living in the home. We found that whilst all residents had a plan of care the documentation and recording of incidents was not always up to date and there were some discrepancies and missing information. One residents care plan showed a body chart for pressure area care and entries documenting a grade 2 pressure sore to be referred to the tissue viability nurse on her next visit some 4-5 days later. There was not always records of professional healthcare visits even though visits from the doctor had been requested. We found that a turning chart for 2 hourly turns was introduced but turns were not always recorded at the correct intervals. The chart was written on a piece of paper covered in tea and coffee stains and just left on the bedside cabinet even though the resident was in hospital. One resident has special nursing needs and is cared for accordingly, with family agreement, as he was in danger of falling. We were unable to trace a risk assessment in respect manual handling even though it was necessary for staff to move this gentleman to his chair for meals.. We found that the room of one resident was left open for anyone to enter even though he had been admitted to hospital. This meant that anyone had access to all the personal and private possessions in the bedroom. Call systems to summon 10/08/2008 assistance must be easily accessible in toilets and en suite bathrooms at all times to people living in the home. The manager must ensure 10/08/2008 that there are easily accessible clinical waste bins in toilets and bathrooms and protective clothing for staff to use and so follow infection control procedures and prevent the spread of infection.
