The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Training opportunities for staff is very good ensuring they receive the skills and competencies to provide support for the residents. Comments from staff included, "Training is excellent". Also, "All staff including cooks, cleaners and management attend three days mandatory training each year to update moving and handling, fire safety and other courses". And, "The company pay for us to attend training access is good". One resident said, "All the girls know what they are doing". All surveys returned from relatives and residents commented positively on how the home is run and standard of care given confirming the support and care of residents is a priority. Comments from relatives included, "My husband is a patient at Lake View with dementia, staff are always aware of my feelings andcaring towards my wellbeing as well as my husband". And, "I am very impressed with the standard of care ". The home has a settled staff team with few staff leaving since the previous inspection ensuring relationships develop between staff and residents and staff have a good knowledge of resident`s needs. One staff member spoken to said, "A lot of us have been here for years, we all get along well." Residents spoken to confirmed the positive comments, one said, "Its like one big family". The home carries out very clear and precise assessments of need in the different areas of care they provide service in with detailed care plans developed which take into account the residents wishes and needs. The management team are all qualified and experienced people, and show a commitment to, and a pride in, their work, taking into account the diverse needs of their residents. One relative of a resident in the dementia part of the home commented, "I was involved in every decision about the care for my husband". A resident spoken to said, "Yes I get involved in the reviews of my care every month". A staff member said, "Thorough initial assessments are completed by experienced nursing staff before admission".

What has improved since the last inspection?

The manager is always improving forms and systems to make them more detailed or easier to follow, ensuring the care is consistent and continues to improve the home for the residents. Since the last inspection the manager continues to update and refurbish some bedrooms and communal areas so that residents continue to live in a wellmaintained and comfortable environment. The home has a designated social activities person who is responsible for arranging daily activities and outings to suit each individual`s interests ensuring the residents are motivated, stimulated and able to follow there hobbies as much as possible. A member of staff spoken to said " We have a lot of trips out in summer, also regular meetings arranged so that residents can request activities they enjoy". One resident spoken to said, " Its good we have a person to arrange things for us".

What the care home could do better:

Radiators must be of a guaranteed low surface temperature or guarded properly so that the risk to residents is removed ensuring there safety is maintained. Since the previous inspection work has been carried out to ensure radiators are safe however guards for all the radiators would protect the residents. There remains a recommendation for the manager to complete the necessary qualifications recommended by the National Minimum Standards in management and care to run and manage a care home. Discussion with the manager confirmed training is ongoing to complete the course.

This home is registered for a maximum of 51 service users to include: Up to 25 service users in the category of OP (Old age not falling within any other category). Up to 26 service users in the category DE (Dementia) 2. 3. Up to 4 service users in the category of PD - (Physical Disability) The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. Staffing must be provided at all times to meet the dependency needs of service users and will comply with any guidelines which may be issued through the Commission for Social Care Inspection regarding staffing levels in care homes. 22nd March 2006 Date of last inspection Brief Description of the Service: Lakeview is a residential and nursing home providing 24-hour care and accommodation for fifty one residents of either sex over the age of sixty-five. The registration includes a specialist unit for residents suffering from mental health problems associated with dementia and four residents with a physical disability over the age of fifty. At the time of inspection fifty residents were living at the home. Lakeview is owned by European care (UK) Limited, whose headquarters are based in London. Responsibility for the day-to-day running of the home is that of the registered manager, Mrs Margaret Parker. There is an area manager who visits the home regularly to provide managerial support. The home is located near the village of Withnell, in a rural area requiring transport to reach the nearest shops and other facilities such as the post office, local pubs, and other town amenities. The home is set in extensive grounds with woodland views and views of a lake owned by a different company. The home provides accommodation on two floors, served by a passenger lift. All rooms are single and thirty-one have an en-suite facility. There is a statement of Purpose/Service user Guide, which is given to all prospective residents. The fees for the home range from £366.00-£705.00 per week. Additional variable charges are made for hairdressing, newspapers, some toiletries and chiropody. This was an unannounced inspection that took place on the 6th of March 2007 over a period of approximately 6.5 hours. The Inspector spoke to the manager, senior nurse, six staff, eight residents a relative visiting the home, and a group of residents in the lounge area. As part of the inspection process the inspector used case tracking as a means of assessing some of the National Minimum Standards. The process allows the inspector to focus on a small number of people living at the home. All records relating to these persons are examined and the rooms they occupy are looked at. Other residents are invited to pass their opinions to the inspector if they wish. Survey’s were sent to relative’s and residents for there views on how the home is run four completed questionnaires received from residents and four from relatives none were returned from GP surgeries or social workers. Comments were positive about the standard of care provided by the staff and management of Lake View. Records of three members of staff were also examined. A tour of the premises was undertaken. Examination of the homes documentation, policies and procedures formed the basis of the inspection process. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3.Quality in this area is excellent. This judgement has been made using available evidence including a visit to this service. The admission and assessment procedures were clear and precise to ensure the needs of the residents are met EVIDENCE: The records of three residents were examined and had full assessment information recorded in detail. One resident is funded by social services and been assessed by social workers with information on file for the care staff at the home to develop a care plan to ensure all health, welfare and social needs are identified and recorded. A staff member spoken to said, “We ensure all the information is provided by social workers before anyone is admitted”. Another member of staff said, “Only senior staff undertake assessments”. Residents spoken to confirm they and their relatives were involved in the assessment process and were able to give their views on the care they required. One relative survey said on how thorough the home was before her relative was admitted ensuring they had the correct information to provide the right care, “The manager was very caring and helpful”. Standard 6 was not assessed, as Lake view does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Promotion of health is taken seriously. Resident’s welfare is closely monitored and health needs are identified and met. EVIDENCE: Records of three residents were examined and accurately reflected the individual’s health and social care needs. Care plans were up to date and regular reviews taking place with involvement of the residents and relatives and good information of the care provided ensuring the welfare and general wellbeing of residents is continuously monitored. One of the residents spoken to said, “Yes I am involved in my monthly review and sign the form”. Records examined confirmed risk assessments have been completed and are reviewed monthly and updated reflecting any changes that have occurred individually and in the environment ensuring the resident’s needs are being monitored. Residents spoken to give their opinion of the care they received comments included, “I was in a right state when I first came here, now I feel a lot better because of the way the staff are”. A relative of a resident in the dementia unit said, “I am involved in all the care planning the manager keeps me well informed”. Medication practices observed were safe and good records had been kept ensuring residents health is maintained. A nurse spoken to said, “ Only trained staff administers medication”. The three medical records looked at were being accurately maintained to ensure the safety and protection of the residents. At the time of the inspection controlled drugs were being administered and examination of records, storage and handling of medicines confirmed good systems are in place with experienced trained staff to ensure the safety and health of the residents is maintained at all times. The pharmacist visits on a regular basis to provide advice and training so care staff and nurses are well informed of any changes in medication practices. A staff member spoken to said, “We have a great relationship with the local chemist he is extremely helpful and available any time day or night”. Resident’s dignity and privacy was observed during the visit ensuring residents are treated with respect. This was confirmed by observing staff members knocking on doors before entering rooms and the way staff talked to the residents. One resident spoken to said, “The staff are wonderful”. Another said, “Always respectful to me”. A member of staff spoken to said, “We discussed privacy and respecting residents during our induction training”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Social activities and meals are both well managed, creative and provide daily variation and interest for people living in the home. EVIDENCE: Breakfast and lunchtime meals were seen being prepared, and were wholesome, home baked with fresh produce providing a nutritious meal. Menus examined are balanced and interesting. Meal times are set although flexible enough to accommodate preferences. One resident spoken to said, “If I want I can have meals in my bedroom”. Resident surveys and residents spoken to commented on the high quality of food at the home. Comments included “Great choice of meals”. And, “Plenty to eat”. Discussion with cooks and staff confirmed diabetic diets, liquidised foods and food from different cultures could be provided ensuring residents from any religious faith can be accommodated. Staff spoken to said, “Liquidised foods are done separately so the presentation is good on the plate”. Also, “I have been trained in Indian cooking and aware of meals from different religions”. A member of staff spoken to said, “ A newsletter is produced monthly and residents contribute towards it”. A resident spoken to confirmed “I enjoy the meetings so we can put forward our ideas for activities and social events”. Activities are centred on each individuals preferences ensuring flexibility and residents can enjoy their own personal interests, which are recorded on their care plan. A resident spoken to said, “You can join in if you want to or just relax”. Observations of residents rooms showed personal belongings are allowed into the home so to provide a homely atmosphere for each individual. One resident spoken to said, “I have a cat the staff love it”. Residents spoken to confirmed visitors can visit at any time of the day or night. One said, “They can come and go as they like”. A staff member said, “We welcome relatives and friends at any time”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The arrangements for recording and reporting of complaints are good ensuring people feel listened to. The manager and staff have good knowledge and understanding of safeguarding adult issues, which protect residents from abuse. EVIDENCE: The home has a detailed complaints procedure, which is made available to all residents on admission and contained in the Statement of Purpose and Service User Guide. Complaints records examined confirm complaints investigated by the home have been managed in line with their complaints policy with details of the complaint, the subsequent investigation and the outcome. One complaint since the previous inspection was received by the Commission for Social Care Inspection (CSCI) and referred back to the home to investigate. Examination of the documentation confirmed this was dealt with appropriately. The manager spoken to said, “All complaints major or minor are taken seriously and investigated thoroughly. Comments from residents confirmed they know who to speak to should they wish to make a complaint. Staff spoken to are aware of the complaints procedure, one said, “Complaints are covered in induction training”. The home has a procedure in place for dealing with allegations of abuse. The manager and staff spoken to had a good understanding of the procedures to be followed in the event of any allegations or suspicion of abuse or neglect. One staff member spoken to said, “Abuse issues and training of awareness is covered in our National Vocational Qualification (NVQ)” . Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25 and 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The environment and building is safe and clean maintained to a good standard providing comfortable surroundings for the residents. EVIDENCE: A tour of the building found the home to be clean and tidy. The home is maintained to a good standard and maintenance records confirmed there is a rolling programme of general repairs and renewal of the premises ensuring the comfort and safety of the residents is maintained. One survey returned from a relative commented, “The room was recently redecorated and I was involved in every decision especially to remove the carpet and replace it with washable floor covering”. There have been some improvements to the bedrooms of residents with redecoration and some new flooring to ensure residents are comfortable and in pleasant surroundings. One member of staff spoken to said, “We are currently in the middle of replacing carpets in bedrooms”. Radiators must be of a guaranteed low surface temperature or guarded properly so that the risk to residents is removed ensuring their safety and protection is maintained. Since the previous inspection work has been carried out to ensure radiators are safe and have low surface temperature, however guards for all the radiators would ensure residents safety. There are policies and guidance for laundry processes and for the control of infection ensuring the home is kept clean, pleasant and hygienic. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The policies and procedures for the recruitment of staff are good ensuring the safety and protection of the residents. Training for staff is good ensuring they have the skills and competencies for their roles. EVIDENCE: Examination of staffing rotas confirms sufficient nursing and care staff on duty at the time of the site visit. Residents said they were happy with the care they receive from the staff and management and were well treated. Residents spoken to said, “Wonderful there is always someone to help you”. And, “Very helpful people”. Training for staff is good, records shows the target of 50 of care staff to complete National Vocational Qualification (NVQ) level 2 in care has been achieved with some staff on level 3. One member of staff said, “Yes I found it useful”. Examination of three staff files confirmed the recording procedures of the home are good. Staff records include, application forms, confidentiality statements, individual photographs, Criminal records Bureau (CRB), disclosures and references were in place to ensure the residents are protected. To improve the recruitment procedure dates on recruitment documentation received on the font of staff records would evidence all required checks are in place prior to commencement of employment. Examination of staff records confirmed each member has an ongoing training record, discussion with personnel confirmed they had induction training when they started working for the home, carried out by the management and senior nursing staff. Comments from staff included, “Access for training is excellent”. And, “There is support by management for all courses and training we want”. One resident spoken to said, “The girls are always doing some courses or other”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is managed well and systems and policies in place for the protection and safety of staff and residents are good. EVIDENCE: The registered manager has the necessary skills and experience required to support the staff and residents and enable the home to meet its stated purpose and objectives. However there remains a recommendation for the registered manager who should complete the Registered Managers Award Lake View Nursing Home DS0000025566.V326627.R01.S.doc Version 5.2 Page 23 equivalent to NVQ level 4 in management and care. Discussion with the manager confirmed the course is ongoing. Staff members said they found the manager and senior staff supportive and provided a clear sense leadership which enabled them to carry out there duties to the best of there ability. One staff member said, “ We can always pop in and see the manager if we have a problem”. Another said, “Always approachable”. A resident said, “The manager is always around if I want to discuss something”. Records show the management has good systems to gather staff, residents and relative’s views to enable ongoing improvements to the home. Regular staff and resident meetings are recorded and suggestions are carried out if agreed by both parties. One staff member said, “At the meeting a suggestion by a resident for a path to be provided down to the lake was made and now it is being done”.
