Conditions of registration: Date of last inspection 06th February 2006 Brief Description of the Service: Lands House is registered to provide nursing and personal care for up to 30 older people. The establishment, a spacious detached property, is situated on New Hey Road in Rastrick with easy access to the town centres of Halifax, Brighouse and Huddersfield. Local community facilities are nearby. The establishment is generally well maintained throughout and provides accommodation in single and double rooms. Safe and accessible garden areas with seating are available for the service users to en joy, especially in the summer months. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 17th March 2006. Over an inspection year from April until March, care homes have a minimum of two inspections a year. During the course of this inspection several service users were consulted as to their views of the standard of care and facilities provided within Lands House, and all spoke very highly of the service provided. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3,6 Service users needs are assessed prior to admission. EVIDENCE: Records seen indicated that service users are assessed prior to admission to ensure that the home can meet their needs. Intermediate care is not provided within the establishment. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Service users needs are assessed and the home has a good approach to promoting the service users physical health care. EVIDENCE: A selection of case records were examined and were found to contain detailed information regarding the physical nursing and personal care needs of the service users. A manual handling plan is in place for each resident and care plans are reviewed on a monthly basis. The care plans, however, contain very little information about the service users prior to admission, there are no social or family histories and no indication of the service users interests and preferences. The daily records are very brief and give no indication of how the service user has spent their day, and only refer to the physical care given. Medication held within the establishment is securely and appropriately stored and administered by the qualified nursing staff in accordance with NMC guidelines. The medication is supplied by a local pharmacy and supplied in individual cassettes.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15 A range of activities are arranged which the service users are encouraged to participate in, if they choose to do so. EVIDENCE: Some form of activity takes place each afternoon and the activity calendar included the following: gentle exercises, old time music hall, reminiscence, video, movement to music, painting and crafts, needlework, bingo, baking, church services, table games and outings. On the day of the inspection Guinness and soda bread was being enjoyed to celebrate St. Patrick’s Day. Records seen indicate that a varied and nutritious diet is taken by the service users. The main meal of the day is served at lunchtime and on the day of the inspection the luncheon being served consisted of: baked fish in butter sauce, potatoes, carrots and cabbage, followed by a choice of either jam sponge and custard or rice pudding. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 The home has robust systems to ensure that service users are safeguarded from abuse and that complaints are dealt with promptly. EVIDENCE: The establishment holds a complaints procedure which is displayed within the home. All staff receive training in adult protection procedures which is updated annually. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Sufficient staff are deployed to meet the needs of the service users. EVIDENCE: From records examined and following observation and discussion at the time of the inspection sufficient staff are employed to meet the needs of the service users. Rota’s seen indicate that there is always a registered nurse on duty and staffing levels are as follows: morning: 1 nurse plus 5 carers, afternoon and evening: 1 nurse plus 4 carers, nights: 1 nurse plus 2 carers. In addition, catering and domestic staff are also employed. At the time of the inspection there was no programme of NVQ training in place and out of a total of 17 care staff only 3 hold their NVQ II award. The owner was reminded of the requirement that 50 of the care staff to be qualified to NVQ II by 2005. Recruitment practices within the home have been strengthened and staff files examined indicated that all recently recruited staff had completed an application form, and two written references and a satisfactory POVA and CRB disclosure had been received prior to commencing employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,38 The acting manager is implementing new systems to ensure that the service users receive consistent quality care. EVIDENCE: The acting manager is qualified and experienced and is committed to ensuring an open and positive atmosphere is prevalent within the home. She has submitted an application to the CSCI be registered as manager of the home. The establishment places a high priority on health and safety issues and policies and procedures are in place to support this.
