Conditions of registration: 1. Only adults (18-64) with learning disability may be accommodated. Date of last inspection 14th December 2005 Brief Description of the Service: Seventy seven Lang Lane South is a three bed roomed detached bungalow that provides accommodation and personal care for three adults with learning disabilities. Seventy seven Lang Lane South is owned by Alternative Housing and managed by Alternative Futures. The home is located on the Wirral in a residential area that is situated on the top of a hill and subsequently benefits from panoramic views of the Mersey estuary. Communal space within the home comprises of a large lounge/dining room, a small quiet area, a study and a kitchen. Adapted and accessible bathing, toilet and shower facilities are also shared. This is an overview of what the inspector found during the inspection. The home did not know about the visit which took two and half hours. The inspector spoke with the service users, manager and care staff. The inspector read files relating to the service users and the home and toured the premises. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,5 The homes Statement of Purpose and Service User guide are excellent providing service users and prospective service users with details of the services the homes provides enabling an informed decision about admission to the home. EVIDENCE: The home’s Statement of Purpose and Service Users Guide contains all the relevant information as required in The Care Homes Regulations 2001 and is presented in a format that is service user friendly. The organisation has a written format for pre-admission assessments that includes compatibility criteria. In addition to a written assessment, prospective service users have a number of introductory visits to the home, which can include an overnight stay. It is understood the last service user admitted to the home had an introductory period of five months. The organisation has a new licence agreement, which is split, into three sections: - Finance, Terms and Conditions and Signatures. The document is clear and comprehensive and presented in a format that is service user friendly. The Service User Guide is also well presented and is service user friendly. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Service users needs are reflected in their care planning and they are supported in making some decisions and taking risks as part of their lifestyle and routines. Visitors are welcomed at the home. EVIDENCE: The home operates a key worker system and care plans are documented in service users daily files. The care plans are detailed and include appropriate information to enable care staff to meet service users individual needs. Each service user has an Essential Lifestyle Plan. In addition each service user has a daily diary sheet which details who was on shift, what went well what did not go well, what could be done differently and other things to share. This is excellent documentation. Each service user has a Personal Communication Dictionary, which was compiled with the help of a Speech and Language Therapist. This is an excellent tool to aid communication and will help new members of staff. It is understood that a service users bedroom was redecorated in their favourite colour. The Service User Guide is service user friendly. Risk assessments are undertaken and recorded for all situations that are potentially hazardous for service users and staff. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Service users engage in community and leisure activities appropriate to their age. Service users are encouraged to meet with their friends. Dietary needs of service users are well catered for with a balanced and varied selection of food available that meets service users tastes and choices. EVIDENCE: None of the current service users are in employment; one service user attends college one day a week. Staff support service users to be part of the local community and evidence of this practice was demonstrated on the day of inspection. The service users went out shopping. The manager stated the home does not have daily routines; service users have individual choice and freedom of movement that is appropriate to their needs. The service users like to go out whenever possible. The manager said that he would monitor a situation if a service user were choosing not to go out. Lang Lane South (77) DS0000018992.V281907.R01.S.doc Version 5.1 Page 11 One service user has a close relationship with someone who lives at another home and they are supported to visit, visitors are welcomed to the home. The staff team open the mail with service users present and read the mail with them. The members of staff on duty were observed knocking on service users doors before entering and asking permission to go in the bedrooms. The staff were observed talking with the service users in a respectful manner. Service users are offered a choice of suitable meals and alternative choices are always available. Separate menus have been developed as part of a healthy eating plan for two of the service users who have health problems. The menus, which were viewed by the inspector, have been devised with the service users involvement and are based on the weight watchers point system. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 The service users receive the appropriate personal support in the way they prefer and require. The service users’ physical and emotional health needs are well met. The medication at this home is well managed promoting good health. EVIDENCE: There were records on morning, night, bath and grooming routine and how each service user wanted these activities carrying out in the two service users files that were examined. Service users physical and emotional health care needs are met. This is achieved by accessing appropriate support from the G.P. and primary care services. In addition service users receive support from a local community resource centre for people with learning difficulties (Ashton House). The home is developing a Health passport, which will record all visits to any health professional. The inspector suggested that the health records are kept within the care plan. Due to profound learning difficulties none of the current service users are able to self medicate. On the day of inspection the management and administration of medication was found to be in accordance with the Care Homes for Younger Adults and Adult Placements National Minimum Standards 2001. All the staff team receive instruction from the manager regarding recording and security of medication. Some staff have received formal medication training, this will be given to all the staff. The stock sheet that has been introduced reduces the

The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 The complaints process in the home is good with complaints information available to service users. EVIDENCE: The organisation has a complaints policy and procedure that meets the requirements referred to in the Care Homes for Younger Adults and Adult Placements National Minimum Standards 2001. There have been no complaints/concerns received since the last inspection. There is an Adult Protection policy that including a whistle blowing procedure. Physical intervention is not used in the home. Not all the staff team have received adult protection training, the inspector recommended that all the staff team have this training. The manager said that at the next staff meeting training regarding abuse will be given. The finances at the home are being revised. One service user has their own bank account; the home is looking into the other service users having their own accounts. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,27,30 The overall quality of the furnishings and fittings is good The home is comfortable and creates a pleasing and pleasant environment for the service users to live in. EVIDENCE: Service users live in a homely, comfortable and safe environment, which is suitable for its stated purpose. A tour of the premises indicated that the service users bedrooms are decorated individually with consultation. The bed rail in one bedroom was purchased through the occupational therapist who assisted with the risk assessment. The inspector suggested that the manager confirm this in writing with the occupational therapist. The paint in the bathroom was peeling and should be redecorated. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,35,36 Staff morale appears high resulting in an enthusiastic workforce that works positively with service users to improve their whole quality of life. EVIDENCE: There are nine staff working at the home, five staff members have an NVQ level 2, three staff are attending an NVQ induction and one staff is completing Learning Disability Award Framework training. The manager has completed the NVQ 4 Registered Managers Award. The staff team appears motivated and there is low sickness record within the team. Some of the staff team have not received 5 days paid training and development days (pro rata) this year. A sample of staff files indicated that the staff receive regular, recorded supervision. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 The record of self-review by the registered provider is good and provides the home with adequate quality assurance. EVIDENCE: The manager has recently become the registered manager for the home. Due to the profound learning disabilities of the service users written surveys are not used. Service users views are sought through daily verbal/non-verbal communication and via relatives/representatives. The manager stated that in addition to the above, service users views are always considered during the Essential Lifestyle Plan review process and personal achievements are celebrated informally. The locality manager monitors the process of managing and running the home via monthly visits, which incorporate a quarterly environmental check. Fire safety records viewed by the inspector demonstrated that not all staff had received a two fire drills in the last year. Weekly fire alarm/emergency lighting tests were appropriately recorded and up to date. Water temperatures are also tested and recorded on a monthly basis.
