Conditions of registration: 1. 2. No more than 7 service users between the ages of 55 - 65 can be accommodated at any one time within the categories of PD and TI Dispensation has been granted for 3 service users under the age of 55 whose dates of birth are 08/03/57, 28/04/52, 19/02/53 to be accommodated at the home. 22nd April 2005 Date of last inspection Brief Description of the Service: Langdale Nursing Home has been converted from a large old house and is situated in a quiet residential area of Gosport, close to all local amenities. The home is registered to accommodate 35 service users over the age of 65 years who are in need of nursing care and who have physical disabilities. The home is also registered to care for those service users who are terminally ill. There is a condition to the registration that 7 service users may be accommodated at any one time between the ages of 55-65 years who are within the stated categories. This is an overview of what the inspector found during the inspection. This inspection was unannounced and was the second statutory inspection for the year 2005/2006. The inspection was conducted by one inspector and lasted four hours. The registered manager was on duty throughout the inspection. During the inspection the inspector spoke with both residents and members of staff. The residents spoken to by the inspector told her that they were very happy with the care they received in the home and confirmed that the staff were very caring and always prepared to give assistance when it was required. They also confirmed that they had freedom of choice over many aspects of their daily lives. One resident commented, “It was just like home, but in many ways better as I am not so lonely and I have the support of the staff when I need it”. Staff members spoken to said that they enjoyed working in the home. They said they were well supported by colleagues and senior staff members and training provided in the home enabled them to maintain a high level of care to the residents in the home. Staff also confirmed that regular supervision enables them to address any issues they had about their job in private and that the manager and her deputy were very supportive. The home has implemented an emergency and crisis policy. This provides staff with guidance of what to do in the event of an emergency or crisis, which may require full or partial evacuation of the building. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Standard 6 does not apply to this home. Arrangements for assessments are in place, prior to admission, which ensure that the needs of the resident can be met. EVIDENCE: Prior to admission to the home the registered manager or her deputy undertakes a comprehensive assessment of the prospective resident. This assessment is carried out either in the person’s home, the hospital or previous residential placement. All the physical and psychological needs of the resident are addressed during this assessment. The inspector saw three copies of these assessments in residents files and evidence was seen that information had been obtained from other health professionals and members of the resident’s family. This assessment identifies the health care needs of the resident and forms the basis for care plans that are written to address all the care needs of the resident. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 The arrangements for meals ensure that residents have a nourishing and balanced diet. They are able to choose where to eat their meals. EVIDENCE: The home provides residents with a choice of food for each meal. The menus are changed weekly over a five-week period. The meals are varied and include fresh vegetables and fruit. The catering staff keep records of alternatives that are requested by residents and the care staff records in a resident’s daily record if any meals are refused. Nutritional assessments are in place and all residents have their weight recorded monthly. Any continued weight loss identified is reported to the resident’s GP and a dietician consulted for advice. Specialist diets are provided for residents where necessary, again, in consultation with the dietician. Resident’s spoken to say that they were always provided with a choice of food each day and that the meals were well presented and appetising. The inspector discussed with the manager for the home the possibility of the catering staff attending a study day for the nutritional needs of elderly people in a care home setting. This would provide them with more information about dietary needs of the residents and enable them to ensure that the nutritional needs of the residents’ are maintained. The manager agreed that she would arrange this.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 29 The staff undertakes training that enables them to be competent to do their jobs. Adherence to the recruitment policies and procedures for the home ensures that staff is employed in such numbers and the skill mix is satisfactory as to meet the needs of the residents. EVIDENCE: The home provides staff with training for NVQ levels 2 and 3 Two staff has achieved NVQ level 3 and one has achieved level 2. At the time of the inspection, five staff members are undertaking training for NVQ level 2. The home has a very robust recruitment procedure. Samples of four staff recruitment records seen by the inspector and were found to be in order. There was evidence that written references had been obtained and checks with the Criminal Records Bureau had been undertaken. The date of interviews of new staff are documented but details of what has been discussed during these interviews are not recorded. This was discussed with the manager for the home and the inspector has recommended that records of interviews should be made. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35 and 38 The appropriate qualifications of the manager ensure that the home is well managed. Procedures are in place to ensure that residents’ financial interests are safeguarded. All safety checks are completed on a regular basis to promote the safety of residents and staff. EVIDENCE: The manager and her deputy have both successfully completed their NVQ4 in management and their registered managers’ award. The manager of the home has managed the home for six years. She is a Level 1 Registered General Nurse and works one shift every two weeks in the emergency department of a local hospital thus ensuring that her clinical skills and knowledge are maintained.

The inspector saw records of recent financial transactions; these were found to be in order. The records detailed all outgoings and income. Receipts are kept for all purchases made on his behalf. Since the last inspection there has been a fire in a resident’s bedroom. The prompt action of the staff, following the fire procedures for the home, brought the fire under control quickly. The resident was unharmed. The resident was able to be accommodated in a vacant bedroom. The fire damaged room has now been completely refurbished and the resident was moving back to the room on the day of the inspection. All risk assessment for this resident have been reviewed. Fire safety records demonstrate that the staff in the home has undertaken regular fire safety training and all fire safety equipment is tested and maintained regularly.
