What the care home does well

Residents are supported by a caring and hard working staff team.

What has improved since the last inspection?

The improvements to the garden area have now been completed save the laying of some grass and borders. Residents have access via ramped pathways to a large patio and decked area. Areas of the home have been redecorated.

What the care home could do better:

Residents reported that on many occasions the call bells were not answered promptly. Recording of medication administered to residents could be improved.

The home can also admit up to eight people above the age of 55 in the category of terminal illness and physical disability. The home has twenty-six single rooms, of which twelve have ensuite bathrooms and seven shared rooms, one of which is ensuite. The home is located in a residential area and the building is on two floors with a shaft lift to access the first floor. The home has a large car park at the front of the building and a garden accessible to the residents at the rear. Latham Lodge Nursing and Residential Care Home. A tour of the building was made and introductions made to the residents and staff. The registered manager, the regional manager and a peripatetic manager all assisted with the inspection. The majority of the residents were spoken with as well as three relatives and the staff on duty. Two residents were tracked through the inspection in respect of the documentation of their care at the home. Staffing levels were discussed and the recruitment records relating to two staff were viewed. Issues raised by residents during the inspection were discussed with the manager. One requirement about the call bell system from the previous inspection remains in force. The other requirement about providing access for residents to the garden has been met. A new requirement was made with respect to the recording of the medication administration records. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2 & 4 Residents are informed of their rights through a signed contract and available Statement of Purpose. EVIDENCE: A Statement of Purpose was available if the front reception area for residents and relatives to read. The files for two residents were tracked through the inspection. A copy of a contract containing the terms and conditions of residence, signed by the resident were in each file. In general the residents spoken with said that the staff team were caring met their needs. The one area of concern voiced by many of the residents was that their call bells on occasion were not answered promptly. This matter is addressed later in the report. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 & 9 Health needs of the residents are met in general, however there are potential health risks resulting from some recording omissions by staff who administer medication to residents. EVIDENCE: The home operates a named nurse key worker system. Each resident is registered with a GP. Within the two personal files for the residents tracked through the inspection there were assessments of needs for each resident. On the day of inspection the peripatetic manager for the organisation was working with the nurses at the home on how to carry out assessments and completion of the company recording formats. Detailed care plans were also within the files and a sheet signed by the residents informed that their care plan had been discussed and agreed with them. Each file also contained a photo of the resident and appropriate risk assessments in relation to moving and handling, skin care, mobility and falls and nutrition. The home also records the wishes of residents in the event of their death. The medication administration records for the two residents tracked were viewed and it was found that there were some gaps within the records. The Latham Lodge Nursing and Residential Care Home. It was reported that the procedure was for medication to be administered, witnessed as taken and then a record made. It was noted that one of the residents spoken with had their medication on their table in an administering pot. A requirement was made that the procedure for the home is to be followed with respect to administering and recording. Residents spoken with said that their health needs were attended to and that the staff treated them with dignity as individuals. Latham Lodge Nursing and Residential Care Home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 15 The management of the home has a strategy for improvements in food provided, from which residents should benefit. EVIDENCE: The home employs an activities co-ordinator and a list of the activities for the month was on display in the hallway. The interests of residents and a short life history are taken with residents as part of the assessment of need. This then links in with activities arranged in the home. During the inspection the inspector spoke with three relatives who said that they were able to visit at any time. The inspector was able to see when visiting residents within their rooms that they were able to personalise their rooms with their own possessions. All of the residents spoken with were asked about the food provided in the home. As well as some favourable comments some residents made negative comments about the quality. This was discussed with the home’s managers and it was reported that the home had recently appointed a new cook as part of a strategy to raise the standard and quality of food provided in the home. It was reported that there was ongoing liaison between the residents and staff on providing a menu that suited the resident group. H54 S11509 Latham Lodge V230973 010605.doc Version 1.30 Page 12 management were aware of difficulties in this period of change and it was agreed that the views of the residents would be canvassed at the next inspection as to improvements that have been achieved. EVIDENCE: It was noted that a copy of the complaints procedure for the home was on display in the main reception area. These standards were not inspected in depth. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 22 Residents would benefit from the instalment of a modern call bell system that is accessible to all the residents who need this means of contacting the staff. EVIDENCE: On the day of inspection the home was found to be clean and in the main free from adverse smells. The paths and patio area that allow residents access to the garden have now been completed as agreed at the last inspection. Flowerbeds have yet to be made and some areas seeded with grass. The dining room has been decorated since the last inspection and the inspector was told that there were plans to improve the front of the home. The home has a schedule for the maintenance of the building, thus meeting the requirement made at the last inspection. At the last inspection a requirement was made that more call bell alarm points are provided in the main sitting area, this was also a requirement from the inspection before that. H54 S11509 Latham Lodge V230973 010605.doc Version 1.30 Page 15 provided, although the home had tried to rectify this with the use of walkietalkies, which had not proved successful. On the weekend before the inspection there had been an intermittent fault with the alarm call system and some residents relayed their fear of the system breaking own again. This was discussed with the managers and a requirement was made that an action plan be submitted within one month as to how the home address these deficiencies. It was evident from visiting residents in their rooms that they were able to personalise their rooms with their personal possessions. Latham Lodge Nursing and Residential Care Home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 & 29 Residents would benefit from the management undertaking a staff review of staffing requirements with the residents and the staff. EVIDENCE: The home is owned by Latham Lodge Limited and the registered manager has the support of a regional manager and for specific issues, a peripatetic manager. At the time of inspection, the regional manager and the registered manager were available, whilst the peripatetic manager was working with the nursing staff in the home to train them in the corporate systems. The care staff work in pairs in three designated areas. It was reported that usually there are two nurses on duty at all times. A proportion of the residents spoken with said that at times they felt it took too long for the staff to answer their call bells. The staff on duty were also spoken with who said that at times they were stretched to meet the needs of the residents and it was frustrating for them that they knew people were waiting for their assistance. This was discussed with the managers is respect of whether there were sufficient numbers of staff or whether the staff could be deployed more efficiently to meet the needs of residents. It was agreed that the management would carry out a staffing review with resident and staff participation to ensure that the needs of the residents are met.
