The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The care residents receive is of a high standard and most of the people spoken with were satisfied with the care that they, or their relative, were receiving. Visitors are made welcome in the home. All of the comment cards received were positive about the care provided. There are plenty of varied activities happening in the home as well as regular outings to the seaside or shopping centres etc. to keep residents stimulated and active. Several residents were seen enjoying singing hymns with the activity organiser in the conservatory area. Staff were seen to work well with the residents and the atmosphere in the home is cheerful and friendly. The home has a competent and experienced manager; staff are very well supported in their training and development so they can provide better quality care for residents. All of the residents said that the quality of meals is good and that they are given a choice. Regular meetings are held with residents to find out what they think of the way the home is run and action is taken to resolve any concerns they have. The home is clean and tidy. Medicines are generally well managed.

What has improved since the last inspection?

The carpet has been replaced in the reception, lounge and corridor areas and these areas have also been redecorated. The manager has introduced a new system of care planning and this appears to be working well. The manager has achieved the Registered Managers Award NVQ Level 4 and now meets the national minimum standard. The hours for the activity organiser has increased to 30 hours per week.

Staffing must be provided to meet the dependency needs of the service users at all times and will comply with any guidance which may be issued through the Commission for Social Care Inspection. 3 Date of last inspection 17/01/05 Brief Description of the Service: Lawton Manor is a care home with nursing accommodating up to 50 older persons. It is located just off the A50 between Kidsgrove and Alsager, close to the village of Church Lawton. Lawton Manor is a two storey Georgian detached property, which has been converted and extended into a fifty bedded care home. The home is situated in its own grounds which consist of landscaped gardens, walkways and patio areas. Bedroom accommodation is on three floors and comprises of 44 single rooms and three double rooms. Twenty eight of the rooms have en-suite facilities. There are two passenger lifts and staircases. Wheelchair access is good within the building and grounds. There is a very spacious lounge and dining room, and in addition there are smaller quiet areas and a conservatory. There are a variety of assisted bathing/showerving facilities and assisted toilets. This announced inspection took place over 6 hours. The inspector spoke to seven residents, four relatives, two members of care staff, one nursing staff, the activity organiser and a member of domestic staff as well as the manager of the home during the course of the inspection. Eight completed CSCI comment cards were received from residents and three from relatives as well as a letter from a relative. The comments are reflected throughout the body of the report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 & 6 Assessments of people’s care needs are carried out before they move into the home so there is information to show that their needs can be met at the home. EVIDENCE: Four residents files were examined as part of the inspection. One of the files contained details of the assessment done by the manager before admission to the home. The deputy manager stated that the assessments for the other residents were archived as they had been admitted some time ago. One relative spoken with said that the home manager had been out to visit her mother before she moved into the home. Intermediate care is not provided at this home so standard 6 does not apply. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,& 10. The care plans include information on all residents’ care needs and how these are to be met so residents are safe and well at all times. Residents’ health care needs are monitored closely so that any deterioration in condition is recognised and addressed quickly. The medicines are generally well managed so that residents receive their medicines as prescribed. EVIDENCE: Individual plans of care are in place for every resident in the home. These were well maintained. They contained detailed information for staff to follow on how to care for the resident. The care plans are reviewed regularly and are changed as residents needs change. There were detailed risk assessments in place. There were records of other professionals visiting the residents in the home as needed. There was evidence that staff monitor residents condition and seek advice from other professionals as necessary. Two relatives spoken with said that staff had sat down with them and their relative and had gone through the plan of care. They were very happy with the staff and their attitude and felt their relative was well cared for at the home. Her relative is now walking with a zimmer. It went on to say that the care her relative received has been ‘wonderful’. Another comment card stated that staff inform relatives regularly of ‘any developments in the management of healthcare and general welfare’ With the exception of one resident, all stated they felt well cared for and that their healthcare needs are met by staff. One resident spoke with the inspector as she was concerned about the action taken by one member of staff in response to a request made on the morning of the inspection. The resident then spoke with the care coordinator who agreed to investigate the issue and let the resident know the outcome. All of the residents and relatives comment cards were positive about feeling well cared for. Although most of the staff were seen to treat residents with respect, knocking on doors before entering, one member of staff entered a residents room without knocking to turn off her call bell. The manager was informed of this incident at the end of the inspection and she agreed to address this issue. Medicines are generally well managed. Staff record on the Medicine Administration Record sheets when residents are given their medicines. A system is in place to make sure that staff address any issues that might arise regarding medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 &15. The activity organiser is well qualified and provides plenty of varied activities to keep residents stimulated and active. Regular outings take place. Visiting can take place at any reasonable time so that residents can keep in touch with their relatives and friends. Meals are well presented, varied and balanced providing adequate nutrition for residents to keep them healthy. EVIDENCE: There is an activity co-ordinator employed at the home. Her hours have recently increased to 30 hours per week. She is qualified to Level 3 City and Guilds in therapeutic care for the elderly. Residents said there are plenty of activities provided at the home. One resident said that he was going to Llandudno the day after the inspection, and that he was looking forward to the planned barbeque on Saturday. Two other residents told the inspector they were looking forward to the trip the following day. One resident wrote on the comment card that there are lots of activities in the home, such as knitting, sewing, occasional exercises etc. They also have outings to the seaside, shopping centres etc. LAWTON MANOR F51 F01 S18727 Lawton Manor V231427 190705 Stage 4.doc Version 1.30 Page 12 One relative wrote that there is something going on all of the time, staff work extremely hard to keep residents occupied. Relatives spoken with said that there were plenty of activities provided by the staff at the home. Residents were seen singing hymns with the activity organiser in the conservatory on the morning of the inspection. A notice board is kept in the main entrance with photographs from recent outings. One resident who spoke with the inspector said that she ‘can please herself’ in what she does at the home. Generally she likes to get up early in the morning and staff help her achieve this. Another resident said that she has recently been discharged from the local hospital and felt as if she was ‘home’. She said that visitors can come and go as they please. All of the residents spoken with said that the food was good. All said they are given a choice. One resident said ‘you can’t match the food it has made me the woman I am today, I was a little old woman when I first come into the home’. The menu was submitted to the CSCI before the inspection. This was varied and nutritious and offered residents choice. Lunch consisted of ham and mustard or Cornish pasty with carrots and cauliflower followed by bread and butter pudding or semolina. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 &18 Information about the complaints process for the home is readily available so residents and their relatives know how to make complaints and who to make them to. Staff knew what action to take if an allegation of abuse was made so that residents are not placed at risk. EVIDENCE: There is a clear complaints procedure both on display in the entrance hall and in the service user guide, which identifies the action to be taken in the event of a complaint. Residents knew who to go to if they wished to make a complaint. Members of the care staff spoken with knew what action to take should an allegation of abuse be made. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26. Although there are signs of wear and tear in some parts of the home, the carpets in the lounge, corridor and reception areas have been replaced and these areas decorated since the last inspection. The home is well maintained, safe and comfortable and provides suitable living space and facilities for all of the residents. The nurses station do not provide an ideal environment for staff to have confidential conversations or telephone calls. EVIDENCE: There are signs of wear and tear in some parts of the home, however the carpets in the lounge, corridor and reception areas have been replaced. A partial tour of the building took place on the day of the inspection. Rooms were well maintained and had adequate furniture. The home was found to be clean and tidy. The lighting appeared adequate in most areas of the home, however some parts of the corridors seemed to be quite dark. The manager said that they were considering painting these areas in a lighter colour to brighten them up. LAWTON MANOR F51 F01 S18727 Lawton Manor V231427 190705 Stage 4.doc Version 1.30 Page 15 The nurse stations which are situated in the corridors around the home are open and not conducive to the management of confidential telephone calls or to confidential discussions with relatives, relatives and other staff. See Recommendation 1. The temperature of the bath water in one bathroom was found to be 48 degrees Celsius which is higher than the recommended temperature of 43 degree Celsius. The temperature was taken with the bath thermometer. The manager informed the maintenance man who agreed to recheck the temperature and address it as necessary. See Requirement 2. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 &30 There are adequate numbers of both qualified and care staff on duty so that the needs of residents are met. Staff are supported in their training thus making sure that they have the knowledge and skills to care for residents. The recruitment procedures include thorough checks of new staff to make sure they are suitable to work in care and that residents are protected. EVIDENCE: Most residents spoken with said that staff attend to them in a caring manner. Relatives were also complimentary about the staff. One comment made by a relative stated that they were particularly impressed that the manager is ‘hands on’ so that she knows the personal needs of the residents in the home. Another residents comment card stated that they enjoyed living at the home as it is friendly and comfortable with good staff who look after the residents well. A relative’s comment card stated that they appreciate the commitment and liaison from the staff. One resident expressed concern about the attitude of one of the staff working in the home. The manager agreed to address this. A sample of personnel files were examined. All the necessary checks had been done before the individual was employed to work at the home. One member of staff new to the home discussed the comprehensive induction programme she undertook. She was knowledgable about the residents and felt supported in her training needs. A comprehensive list of training courses offered to staff was submitted to the CSCI before the inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,37 & 38 The manager is experienced and qualified so the home is run well for the benefit of the residents. Equipment is serviced regularly so that the safety of residents is maintained. The care plans are not kept in a locked cupboard so confidential information could be compromised. Although the health and safety of residents is maintained, the door to the cellar was unlocked and could be a health and safety hazard to residents or young children visiting the home. EVIDENCE: The manager has recently completed the Registered managers award (NVQ level 4). There is a residents’ committee which meets regularly and minutes are recorded. Relatives were complimentary about the management approach and felt it was open and inclusive. The operations manager carries out monthly visits on behalf of the owner and produces a report on the conduct of the home. The manager carries out regular audits including accident audits so that staff can identify trends and try to minimise re-occurrence. The records seen on this inspection were well kept and up to date. However care plans are stored underneath the nurses station which could be accessed by a member of the public. See Requirement 1. Staff have attended mandatory training such as fire safety training and moving and handling in addition to various courses offered by the management of the home. The door to the cellar which is situated to the side of one of the nurses station was unlocked and could be a health and safety hazard to residents. See Recommendation 2. There was evidence to suggest that the equipment in the home is serviced regularly to maintain the safety of the residents.
