The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

What has improved since the last inspection?

Since the last visit, a new storage area has been built which is of sound construction. This is to replace the previous building, which was made of wood and was several years old. To ensure that residents are protected in case of fire, alarm systems tests in Locharwoods are now carried out every week and these are recorded in the fire book. Medication administration records were in good order. In instances where no pharmacy printout is provided and it is necessary for staff to write the drug, dose and time to be administered, they are aware of the procedure. The writer must sign the additions and have a colleague check and sign also. This is necessary to avoid errors such as the resident receiving the wrong drug or dose. Staff files which were read, were well organised in a standardised format, following a recommendation made during the last key inspection. The information required was on the files and easily accessible under the new arrangements.

The service should, at all times, employ a suitably qualified and experienced manager who is registered with the CSCI. 18th August 2006 Date of last inspection Brief Description of the Service: Locharwoods is a care home for 18 older people. The home is owned by Mr. S. and Mrs. C. Pearson. Mrs. Linda Anderton is the registered manager. Locharwoods is a large converted house in pleasant and secluded gardens. Situated in a quiet residential area of Formby, the home is close to shops and local amenities. Locharwoods is staffed throughout the day and night and provides personal care, single accommodation, meals, laundry and in-house social activities. The weekly charge for Locharwoods is £398.00. Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The methods used during this un announced inspection were, discussion with residents, the manager Mrs. Linda Anderton, the registered provider, Mr. Stuart Pearson and staff who were on duty. Records compiled in the home regarding care, staffing and health & safety were read and a tour of the premises and gardens was carried out. What the service does well: Before moving into Locharwoods, residents have been given a copy of the home’s service user guide, they have had their needs assessed and have been assured that their needs can be met in the home. A copy of the service user guide is on view in the reception area for residents and their visitors to read as they wish. All residents have contracts of residence and these are written in plain language and are easy to follow. Contracts include terms and conditions of residency, notice periods and fees payable. Locharwoods is registered for eighteen residents. A sample of the care plans of residents was read and these were satisfactory. The needs identified in their assessments were checked against action plans and had been addressed. The care plans are supported by risk assessments and monthly reviews of support needs with adjustments made accordingly. Diversity is recognised in the care planning process through recording each individual’s religious beliefs and arranging access to visiting ministers. The rights of those who do not wish to participate are also respected. Staff were showing respect for residents’ privacy and dignity during the visit. Bedroom and bathroom doors were kept closed and staff were respectful in the way in which they approached and spoke with residents. A resident said, “It is so relaxed here and I have no problems at all.” The lifestyle in Locharwoods meets residents’ expectations and preferences. A number of in–house leisure events are on offer and residents choose whether or not to take part. Residents said that their meals are very good. Two who commented said that they are consulted daily about what they would like to eat. Staff confirmed that there are alternative menus available each day, in accordance with the choices expressed. Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 6 Locharwoods has a complaints procedure and, (with the service user guide), a copy is provided in the reception area for the convenience of residents and visitors. Residents who were spoken with said they would tell the manager if they had any complaints. They said she and the owners (who visit regularly) are approachable and helpful. The complaints procedure outlines timescales and lines of responsibility in managing the process. Locharwoods is well maintained and in good decorative order. There is an ongoing re-decoration programme. The home is very clean and well organised, whilst maintaining a homely and comfortable appearance and atmosphere. The communal areas are comfortable, well maintained and domestic in furnishings and décor. The gardens are well planted and very well maintained. There is a conservatory off the dining room, with good views of the garden, and there are lounges at the front and side of the home. Over 50 of staff have NVQ qualifications and there is an ongoing mandatory training programme and induction training for newly appointed staff. Mrs. Linda Anderton, has five years experience as registered manager of Locharwoods and holds a management qualification. The manager is well supported by the registered providers, Mr. & Mrs. Pearson, who live in the area and visit several times a week. What has improved since the last inspection? Residents have the information they needs before moving into Locharwoods, and their needs have been assessed. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 2 and 3. The home does not provide intermediate care and will not be assessed against standard 6. All residents have contracts of residence and these are written in plain language and are easy to follow. Contracts include terms and conditions of residency, notice periods and fees payable. Those who have been referred to Locharwoods are provided with the information they need before deciding to move in. They are given a service user guide, which includes a copy of the home’s complaints procedure. A range of needs is assessed including, health, social and personal care, sensory abilities, continence, mobility and cognition. The identified needs form the basis of the individual’s care plan. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. Residents are treated with respect and their health, personal care and social needs are met. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 7,8,9.10. Locharwoods is registered for eighteen residents. The care plans of three residents were tracked. The needs identified in their assessments were checked against action plans and were satisfactory. The care plans were supported by risk assessments and monthly reviews of support needs with adjustments made accordingly. All residents are registered with local general practitioners when they move in to Locharwoods and there was evidence in their health care records of referrals Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 12 to doctors, district nurses, local hospitals and paramedical services. Records of residents’ weight are maintained and monitored. All admissions to Locharwoods are subject to a twenty-eight day mutual trial period and prospective residents are encouraged to visit Locharwoods to meet residents and staff, before making the decision to move in. Diversity is recognised in the care planning process through recording each individual’s religious beliefs and arranging access to visiting ministers. The rights of those who do not wish to participate are also respected. Residents’ leisure interests are recorded in their care plans. A resident said, “I like to do the quiz, it is amazing how much I can remember. There is bingo sometimes. They have got it right for me, there is enough going on to keep me interested.” Locharwoods has a system for managing residents’ prescribed medication and staff who administer medication have received relevant training. Medication is stored securely and administered from pharmacy containers. Medication administration records for the current month were satisfactory at the time of visit. There are systems in place for the audit of all medication brought into the home and for monitoring returns to the pharmacy. Staff were showing respect for residents’ privacy and dignity during the visit. Bedroom and bathroom doors were kept closed and staff were respectful in the way in which they approached and spoke with residents. One resident said, “I like to stay in my bedroom most of the time, I like to have my meals with the others but want time to myself just now. I come and go as I please and no one interferes. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. The lifestyle experienced in Locharwoods meets the residents’ expectations and preferences. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 12, 13, 14.15. Locharwoods provides a number of in-house activities for residents and there was evidence in their care plans of their preferences and participation in leisure events. One resident said she has an interest in art and likes to watch the birds from her bedroom window. Another said, “It is so relaxed here and I have no problems at all.” Residents said that their visitors are made welcome by staff and their privacy is respected when entertaining. A resident said her family visits regularly and she felt that it is important to keep contact with loved ones and not feel Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 14 forgotten. The home has links with the community through a local church group and access to advocacy services if necessary. Residents said that their meals are very good. Two who commented said that they are consulted daily about what they would like to eat. Staff confirmed that there are alternative menus available each day, in accordance with the choices expressed. Since the last visit, a new food store has been built and this provides space for fresh, chilled and frozen foods. Food stocks were high and were appropriately stored, particularly the meat, which was appropriately labelled and dated. The cook said there are deliveries of fresh fruit and vegetables as required and there were in store choices of breakfast cereals, juices, tea and coffee. There is a bright and comfortable dining room, which provides ample accommodation for all residents and is well decorated with freshly laid tables. The dining room is adjacent to the kitchen, and meals are served directly through a hatch. Residents’ complaints are listened to and they are protected by procedures and training provided to staff of Locharwoods. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 16, 18. Locharwoods has a complaints procedure and with the service user guide, a copy is provided in the reception area for the convenience of residents and visitors. Residents who were spoken with said they would tell the manager or owners, if they had any complaints. The complaints procedure outlines timescales and lines of responsibility in managing the process. There have been no complaints received at CSCI about Locharwoods since the last visit. The home has adult protection and whistle blowing procedures. Staff who were spoken with had received POVA training and were aware of the indicators of abuse and procedures to be followed in the event of abuse of a resident being suspected. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. Locharwoods is well-maintained and hygienic and provides comfortable accommodation for eighteen residents. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 19, 26. Locharwoods is well maintained and in good decorative order. There is an ongoing re-decoration programme, the most recent works being decoration of one of the lounges and construction of a new food store, which is situated in the rear garden. Future plans include replacing the existing kitchen units. The communal areas are comfortable, well maintained and domestic in furnishings and décor. The gardens are well planted and very well maintained. There is a conservatory off the dining room, with good views of the garden, and there are lounges at the front and side of the home. Residents appear to move freely Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 17 between all the communal spaces and their bedrooms. One resident said as spoken with in her bedroom, she said, “I have a lovely view of this tree and I spend ages looking at the birds. My bedroom is comfortable and I will put more photographs around when I have settled in. For now I am very comfortable.” There are toilets and bathrooms situated throughout the home for residents’ convenience. Locharwoods was very clean and well organised during the visit, whilst maintaining a homely and comfortable appearance and atmosphere. The home employs domestic staff and they are supplied with protective clothing (gloves and aprons). The home has procedures for infection control and the control of substances hazardous to health. Cleaning materials are locked away when not in use and COSHH procedures were being followed at the time of the visit. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. Residents’ needs are met through staff numbers and skills and they are protected through the home’s vetting procedures. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 27, 28, 29, 30. Staff rosters are maintained in Locharwoods and these provide evidence that staffing levels are consistently maintained. There were no residents of high dependency at the time of this visit and care staff numbers and skills were meeting presenting levels of need at that time. Ancillary staff, (cooks and cleaning staff), were on duty and the home employs a contract gardener. Residents said that staff are always there when needed and answer calls promptly and staff said they did not feel rushed in carrying out their duties. Staff are provided with job descriptions and contracts of employment and they said they were well supported by their employers, the registered providers, Mr. & Mrs. Pearson. The home has a procedure for recruitment and selection, which includes, advertising posts, interviewing candidates and taking up references and vetting clearances for those who are appointed to posts. Three staff files were read and were satisfactorily maintained and well organised. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The home is managed with residents’ best interests and safety as a priority. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Standards 31, 33, 35, 38. Mrs. Linda Anderton, has five years experience as registered manager of Locharwoods and holds a management qualification. During the visit, Mrs. Anderton showed a very good level of knowledge of resident’s needs. Staff said they are well supported by Mrs. Anderton and a resident said of her, “She is a very caring person. She has been of great support to me since I moved here.” Locharwoods DS0000038689.V336857.R01.S.doc Version 5.2 Page 21 Mrs. Anderton confirmed that the home has a quality assurance system, carried out by a consultancy agency. The most recent quality questionnaires were read and gave positive responses to questions about Locharwoods, including those on care and support, staffing and the environment. Mrs. Anderton said that remedial action would be taken to address any negative responses received in the questionnaires. The home holds small sums of money (personal allowances) in safe keeping, on behalf of some of the residents. Records of transactions were seen and these had been satisfactorily maintained. The manager said the home does not become involved in residents’ personal financial affairs. These are selfmanaged, or family members have responsibility. For residents who need assistance or representation, and who do not have power of attorney in place or have any family, the manager would arrange access to local advocacy services to advise. Legionella : Mr. Pearson confirmed that he has checked with the engineer and has been advised that the home does not require a legionella certificate due to the system in place for water storage in the building.
