Conditions of registration: 1. 2. A maximum of 40 persons in the OP category in need of nursing care may be accommodated. A maximum of 14 persons in the category DE/E (Dementia) to be accommodated. 6th July 2005 Date of last inspection Brief Description of the Service: Longfield Manor is a care home with nursing providing personal care and accommodation for up to forty older people and accommodation for fourteen service users with dementia. Forty eight service users are accommodated at this time. Dr. S. H. Sachedina and Mr. S Boghani privately own it. The home was opened in 1998 and is a two-storey building with wellmaintained grounds. The home has forty six single bedrooms and four-shared bedrooms all offering ensuite facilities. There is a passenger lift and comfortable sitting and dining areas, domestic in character. There is an attached day centre, which the service users can access. The home is located in Billingshurst close to shops and other local amenities. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out on the 07/12/05. The home was inspected against the National Minimum Standards. Residents, staff and visitors were spoken to. The registered manager was available throughout and made any documentation required for inspection available. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5 Prospective residents have the information they need to make an informed choice about where to live. Residents and their representatives know that the home they enter will meet their needs. Prospective resident service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Standard 6 is not applicable. Longfield Manor DS0000024173.V269723.R01.S.doc Version 5.0 Page 8 EVIDENCE: A Statement of Purpose and Service User Guide is in place to give prospective residents and their relatives information on the home. Prospective residents are invited to see the home prior to admission. All prospective residents have pre-assessment prior to admission. This was evident in the care plans. Some pre-assessments are carried out on the day the prospective residents visit the home. The home does not usually take emergency admissions but the registered manager discussed putting in place a policy to ensure speedy assessment should admissions be made from long distances and preassessment is not possible. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 & 10 The residents health, personal and social cares needs are set out in an, individual plan of care. Residents make decisions about their lives with assistance as needed. Residents, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. EVIDENCE: Are plans inspected showed all aspects of care being carried out and recorded. Policies and procedures are in place to ensure the safe storage, handling and disposal of medicines. Medicines are only administered registered nurses or, in the residential suite, staff who have had appropriate training. All medicines received, administered and disposed of are recorded. Controlled drugs are stored in an appropriate cupboard and a controlled drug register is maintained, although there are not any controlled drugs at this time. The staff liaise with the general practitioner on medication issues.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 & 15 Residents are helped to exercise choice and control over their lives. Residents receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. EVIDENCE: Residents spoken to stated that they have a certain amount of freedom to come and go as they please. Relatives spoken to stated that they are free to visit the home at any time and are made welcome. Residents are free to go out on outings with friends and relatives. Several residents were spoken to in the dining room and in their rooms. Two stated that they were not happy with the food but all others spoken to expressed satisfaction, but stated that tea is sometimes cold. Several residents stated that the chef tries to please them and that the food has improved since he arrived. One resident stated that she was 100 happy in the home. There were however some shortfalls noted in the kitchen. Some food was noted in the 1st fridge, not dated or labelled. The 2nd fridge door was very dirty. Freezer 1 was disorganised and an ice cream was stored with the lid off. The freezer itself was dirty, particularly the rim which also had a hair stuck to it. The bottom of the food trolley was dirty with old food residue.
Longfield Manor DS0000024173.V269723.R01.S.doc Version 5.0 Page 11 The floors were dirty with caked residue particularly around the doorsill and under the trolleys and kitchen equipment, also around the dishwasher. Fridge and freezer temperatures and the cleaning schedule were not up to date. The unacceptability of these findings was discussed with the new chef. They were also discussed with the registered manager. The last environmental health report was 17/11/04 when deep cleaning was required. The manager stated that this had occurred. The need for this practice to be ongoing alongside an effective daily/weekly cleaning schedule was discussed. It was noted that during the inspection the home was suffering a gas provision problem which was being adequately addressed and every effort was being made to ensure a supply of hot meals from a sister home. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Residents and their relatives and friends are mostly confident that their complaints will be listened to, taken seriously and acted upon. Residents are protected from abuse. EVIDENCE: Residents spoken to stated that they felt that they could complain and that it would be acted upon. One stated that her complaints on food made little change, however this was not the experience of the majority. One resident and relative have made many complaints mainly regarding the food. The manager discussed strategies that have been put in place to overcome this difficulty. A second chef has been employed, food wastage is monitored and these issues are discussed at residents meetings thus evidencing that complaints are listened to and efforts made to address them. The homes policies and procedures and comments made by residents show it are the homes intention to protect residents from abuse. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 26 These standards were fully inspected at the last inspection and the recommendation of that report has been met. Residents have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. EVIDENCE: A visitor stated that a relative had been taken out that morning but they could not get very far as the tyres of the wheelchair were flat. This was discussed with the manager who stated that she would address this so that residents had the equipment necessary to promote independence and freedom. The home in general was neat clean and free from offensive odours, with the exception to the hygiene issues cited on page 13-14 of this report.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 Residents are supported and protected by the home’s recruitment policy and practices. EVIDENCE: Staff files inspected had all of the necessary documentation to ensure that they can meet the assessed needs of residents and ensure their safety. Staff turnover at this time is quite high but the home does not employ agency staff. Two staff recently the subject of an adult protection investigation have left, disciplinary proceedings had been instigated and their names have been referred for consideration of inclusion on the Protection of Vulnerable Adults list. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 35 & 38 Residents are safeguarded by the accounting and financial procedures of the home. Residents’ financial interests are safeguarded. The health, safety and welfare of service users and staff are mostly promoted and protected. EVIDENCE: Resident’s financial interests are safeguarded by the policies and procedures in place. Any monies held are held securely and suitably documented. The manager handles a profit and loss account for the home annually and has quarterly account meetings with area management.
