Conditions of registration: none Date of last inspection 04/11/04 Brief Description of the Service: Longlands is a care home providing personal care and accommodation for up to 43 older people. the home is part of Mimosa Healthcare Ltd.The home is situated on the outskirts of central Middlesbrough and is close to shops, pubs and other amenities.It is a purpose built 2-storey building with an en-suite facility in all rooms. There is a paved area to the front of the home and a garden to the rear that is well maintained. A passenger lift is available to access the first floor. This is an overview of what the inspector found during the inspection. A positive inspection, whereby all of the National Minimum Standards inspected were met. The inspection lasted four hours with one inspector and six residents were interviewed. A tour of the home showed that it was clean, tidy and well decorated throughout. The six residents interviewed were very satisfied with the care they received. They said they felt happy and well cared for. Residents were complimentary about the quality of the food they were offered. They all expressed they were highly satisfied with the care and also said they wouldn’t be anywhere else. Praise was given for all of the staff working in the home and the residents said they couldn’t wish for better and kinder staff. What the service does well:
The home is well managed and the staff delivers a high standard of care to the residents. All the residents were clean and the clothes were appropriately laundered. The care records were comprehensive and detailed to reflect the residents well being. Staff are well trained in health and safety issues and care practises. The environment is pleasing with new carpets in the corridors and some bedrooms. There is a budget available for the furniture and decoration to be on going. Residents’ views are considered when redecoration is planned particularly in their bedroom. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Each service user’s needs are assessed prior to their move to the home and periodically thereafter. This will help ensure that each service user’s needs are met at the home and inappropriate admissions avoided. EVIDENCE: Four assessment records were viewed, they included a care manager assessment and the homes assessment. This ensured that the home could provide the care each resident needed. Detailed information about the resident’s needs and abilities are included, this is a holistic approach to care and benefits the resident. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The health and personal care needs recorded in the care plans fully reflect the resident’s current level of need. As such the resident’s welfare is fully promoted and safeguarded. Staff at the home successfully support service users with their social, health and personal care needs in a way which treats them with respect and promotes their rights and privacy. Arrangements are in place to help preserve service users’ privacy and dignity, however insufficient staffing levels have impacted the ability of staff to promptly address service users care needs. EVIDENCE: The residents care plans were detailed and comprehensive. They gave a true reflection of the needs of each resident and were also tailored individually for their health improvement. All care plans were appropriately evaluated and amended as required. Following a discussion with the resident’s they all confirmed they were treated well and with dignity. The staff were polite and kind towards them and would assist them with anything they required them to do. Privacy was maintained and respected at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,15 Residents are offered the opportunity of participating in a wide range of leisure and social activities enabling them to lead active and fulfilled lives. Service users are offered and receive a varied, wholesome, nutritious and wellpresented menu. This can contribute to their general health and wellbeing. EVIDENCE: The residents enjoy an active social time within the home should they choose to participate. Some of the residents go out to the shops independently or with their families. An activity co-ordinator is employed 20 hours per week and organises trips to places of interest. Some places the residents have visited include Albert Park, Dorman Museum, York Railway Museum and shopping at Cleveland Centre and Hill St shopping centre. In house activities include bingo, cinema nights, manicures and dominoes. During lunch the residents confirmed they enjoyed the meals and they were well presented and appetising. A choice of menu was available and the residents said they could have what they wanted and also the portion size was to there liking. JUDGEMENT – we looked at outcomes for standard(s) 16 Complaints in the home are handled objectively and openly with the manager and staff encouraging service users, their friends and families to offer comment on the services that are offered. Service users are confident that any complaints made would be acted upon. EVIDENCE: All complaints are taken seriously and records are well maintained containing the appropriate information. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 The home is clean, well decorated and maintained. This can help promote a positive image for service users, and ensure they remain safe. The building is clean, warm, homely and attractive offering service users a pleasant environment in which to live. EVIDENCE: The home is well maintained and has recently had new lights fitted in the foyer and the lounge, dining rooms, conservatory has been refurbished. Carpets in the upstairs corridors have been replaced. Portable electric appliance testing is undertaken annually. The home is clean and tidy throughout; laundry staff are employed 42 hours per week. The laundry facilities are satisfactory and the equipment is serviced periodically and is up to date. Policies and protective clothing for staff is in place. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 There are sufficient staff on duty to meet the needs of the residents’ The procedures for the recruitment of staff are robust and provide the safeguards to offer protection to people living in the home. The residents’ needs are met by a well trained staff team. EVIDENCE: There are 42 medium-high dependant people currently living in the home. The senior management team have increased the night staff from 3 –4 carers two of the carers are seniors and have 1st aid certificates and an NVQ in care. Meticulous record keeping in the home shows a robust policy for recruitment is applied. Staff training is in place for all staff and detailed records show that training is consistent and appropriate for the staff this enables the carers to carry out the care equipped with the information that will enhance care. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,36,38 The manager offers clear leadership and direction to the staff and this has resulted in well-trained workforce who offer consistency of care to service users. The care practices, which are in place and the regular staff training, ensure that at all time service users health and welfare is promoted. The manager ensures that all staff have guidance and support so that service users needs can be most effectively met. Service users are asked by the organisation and by staff about how they want their support to be provided so that their rights and best interests are promoted. The health and safety of the service users is promoted by a wellmanaged staff team. Longlands Care Home B51-B01 SN56178 Longlands VN236652 180805 Stage 4.doc Version 1.40 Page 16 EVIDENCE: An annual survey is undertaken and the views of the residents, social workers and other health care professionals are included. All staff have received regular supervision and have had training in infection control and 60 of the staff have had 1st Aid training. The majority of staff have an NVQ in care qualification and the remaining staff are working towards achieving this. There is a rolling programme for training that covers health & safety and care practises. Protective equipment is also available for the staff and there are up to date policies available for the staff to refer too. The staff are equipped with appropriate hoists and slide sheets and there are two lifting and handling coordinators employed in the home. The health & safety representative tests the fire alarms weekly and it is commendable that this person has received an award from Middlesbrough Borough Council for good practise and for maintaining accurate health and safety records. There is a carer with a first aid qualification on duty at all times and the first aid boxes are checked and restocked monthly.
