The accommodation consists of 21 single rooms and one shared room, which are spread over three floors. Three of the single rooms and the one shared room have en-suite facilities. There is one large dining room, which has a small seating area overlooking the garden, two further lounges and a conservatory. The home is owned by Mr and Mrs Munif and is managed on a day-to-day basis by one of the registered providers, Mrs Munif. Lynwood is registered to care for 23 older people. The registration also allows for up to ten service users who are suffering from a dementia type illness and three service users who may have a mental health problem. The home is located in the Heaton Mersey area of Stockport and is close to local shops and other amenities. Stockport town centre, motorway network and public transport are easily accessible. The fee structure at the home ranges from £408-£418. The previous CSCI inspection report was available on request. Care Homes for Older People Page 3 of 8 What we found:
The lead inspector requested a pharmacist inspector to look at the medicines to see if they were managed safely. The visit took about an hour and three quarters and involved examining residents medicines and medicine records. The manager accompanied us throughout the inspection and two members of care staff, who manage medicines, also took part. We found that, on the whole, medicines were managed well, which helps ensure that residents health is protected. We looked at how medicines were stored and found them to be secure. A trolley was used to take the medicines to the residents. The rest of the medicines were in a large metal cupboard. Each residents medicines were stored seperately in baskets. This means that unauthorised people cannot get to the medicines and they are secure. The law about how to store controlled drugs (like strong painkillers that are liable to misuse) has changed. This means that all care homes need to have the type of cupboard that is specified in the regulations. The cupboard being used is not suitable now. It is important to store these medicines correctly to comply with the law. We looked at how medicines were handled and recorded. The pharmacy supplies the medicines, where possible, in a 28 day calendar pack. It is easy to compare these with the records to account for all the doses. There is no system to easily account for all the doses of medicines packed in conventional containers. Recording the amounts held regularly will ensure that all doses are accounted for and the residents have had their medicines safely. The medicines we examined were in good condition and items with a reduced open shelf life marked with the date of opening. We found the medicine records to be well done. The pharmacy provides printed sheets with the medicines that cover each 28 day supply. They showed when medicines were received and residents taking or refusing their medicines. The pharmacy also provides a book to record items to be sent to pharmacy to be thrown away. Staff made notes on the back of the record sheet to explain refusals in more detail and dose changes. Another record showed how staff tried to help a resident by varying the dose of a sedative to help the anxiety whilst avoiding over sedation. We found a medicine pot with an unidentified tablet in it in the trolley. Potting up tablets before the resident is ready to take them is bad practice as there is a risk that they might be given to the wrong person. When we looked at the record it had been signed as given. This is an example of how medicines can be mishandled. We were told that this was an exceptional case as the resident had gone out and would be back later. The same staff would give the tablet safely. What the care home does well:
Medicines were stored safely in an organised way. This helps ensure there is no mishandling. This means that residents get the most benefit from their medication, so protecting their health.
